Palmar fibromatosis (Dupuytren's contracture). Ultrastructural and enzyme histochemical studies of 43 cases.

Forty three cases of palmar fibromatosis were studied by light and electron microscopy, enzyme histochemistry, and ultrastructural immunohistochemistry. By electron microscopy most of the cells composing the nodules in both the proliferative and the involutional stages were identical to myofibroblasts. The myofibroblasts in the involutional nodules often possessed microfilament aggregates probably representing contraction of micro(actin)filaments in the cytoplasm. The proliferative nodules revealed small perivascular haemorrhages and haemosiderin deposits accompanied by accumulation of macrophages and some lymphocytes; these inflammatory cells possibly secrete a certain growth factor inducing proliferation of genetically abnormal fibroblasts and myofibroblasts. Diaminopeptidase IV was detected in myofibroblasts and fibroblasts by enzyme histochemistry and ultrastructural immunohistochemistry; the enzyme may play a role in the metabolism of intercellular substances. Some perivascular mesenchymal cells, interpreted as variants of myofibroblasts, had moderate activity of alkaline phosphatase.

Surgery of the primary tumor of metastasizing renal carcinoma.

40% or 116 of 350 patients with renal carcinoma had distant metastases at the time of hospital admission (M1, N0-4). Women fell ill less often than men did (3:7). 82 of 116 patients (71%) received nephrectomy primarily. Lethality within the first 30 days was 6%. Patients with nephrectomy survived longer than those without, women with nephrectomy survived longer than men. Patients with a grade II tumor survived longer than those with a grade III tumor. Palliative nephrectomy can therefore be recommended as treatment of choice, if the general condition of the patient allows it, the more so as there are no alternative ways of treatment.

Acute vascular rejection treated by plasma exchange.

Since 1980 we have treated in 14 patients 19 steroid resistant acute vascular rejection (AVR) episodes by plasma filtration (PF). Each episode was treated by six PF with 5L plasma being filtered and simultaneously replaced by 3.5 per cent Albumin-Ringer-lactate. After each PF 20g immunoglobulin (Intraglobin) was given intravenously. In all cases renal function improved following PF. In the long term one graft was lost due to chronic irreversible vascular rejection, all others are still functioning, nine of them six months and eight of them 12 months post PF treatment. They show no signs of rejection. PF with repletion of immunoglobulins after treatment is a simple and safe procedure, which in our hands proved to be quite effective in reversal of AVR.

Reversal of acute vascular rejection by plasma exchange.

The vast majority of grafts with acute vascular rejection (AVR) undergo irreversible loss of function, since this type of rejection usually does not respond to increased levels of immunosuppression. Since 1980 we have treated with plasmafiltration (PF) 14 steroid resistant episodes of AVR in 10 patients. In the treatment of each episode 6 X 5 L plasma were filtered and simultaneously replaced with 3.5% albumin-Ringer-lactate solution. After each PF, 20 g immunoglobulins (Intraglobin) were replaced i.v. In all cases following PF renal function improved. One graft was eventually lost as a result of chronic irreversible vascular rejection; all others are still functioning. Eight of the 10 patients have been observed now for at least 6 months since beginning PF treatment and 5 of these 8 have been followed for more than 12 months. None of them shows signs of rejection. PF with replacement of immunoglobulins after treatment appears to be a simple and safe procedure, which in our hands proved to be quite effective in reversing AVR.

Improved graft prognosis by treatment of steroid resistant rejections with ATG and plasmapheresis.

Conventional therapy of acute rejection is almost exclusively based on increased steroid dosage, however, a considerable number of grafts undergo irreversible steroid resistant rejection (SRR). We investigated in a prospective study the effects of antithymocyte globulin (ATG) and plasmafiltration (PF) in cases of SRR. Acute interstitial rejections were treated with ATG, acute vascular rejections with PF. Thirty-nine of 42 (93%) cases of SRR were reversed by these forms of therapy. In 68 recipients of first cadaveric renal allografts actuarial one year graft survival has improved to 88 (+/- 5) per cent since the introduction of ATG and PF for SRR. Severe side effects or increased mortality were not observed, none of the patients with either form of therapy died. Thus ATG and PF are valuable adjuncts in the treatment of SRR.

Acute vascular rejection treated by plasma exchange.

The vast majority of grafts with acute vascular rejection (AVR) undergoes irreversible loss of function, since this type of rejection usually does not respond to increased immunosuppression. Since 1980 we have treated in 22 patients 27 steroid resistant AVR by plasmafiltration (PF). Per episode 6 x 5 L plasma were filtered and simultaneously replaced by 3.5% Albumin-Ringer-lactate. After each PF 20 g immunoglobulins (Intraglobin(R] was substituted i.v.. In all cases renal function improved following PF. In the long run 1 graft was lost due to chronic irreversible vascular rejection, in 2 patients rejection was not reversed by PF, all others are still functioning, 19 of them 6 months and 8 of them 12 months post PF treatment. They show no signs of rejection. PF with repletion of immunoglobulins after treatment is a simple and safe procedure, which in our hands proved to be quite effectful in reversal of AVR.

Acute Disease↗

[Liver vein obstruction. Clinical aspects and diagnosis of the Budd-Chiari syndrome].

In recent years we have observed 9 cases of Budd-Chiari syndrome. The diagnostic, etiological and therapeutic aspects of this rare hepatic vein disease are discussed. 3 patients had a neoplasm, and 1 female had polycythemia; she had been on oral contraceptives. 1 patient had osteomyelosclerosis. 4 females exhibited thrombosis of the hepatic veins; 2 of them had been on oral contraceptives. Liver scan has proven to be an adequate method for follow-up studies in demonstrating the hypertrophied caudate lobe. Very rarely, the small and medium-sized hepatic veins are involved, most often after ingestion of certain plant toxins. This syndrome is termed the Stuart-Bras syndrome or "veno-occlusive disease".

The origin of the dendritic reticulum cell. An experimental enzyme-histochemical and electron microscopic study on the rabbit spleen.

To obtain more information concerning the origin of dendritic reticulum cells, the development of germinal centers in the spleens of rabbits was investigated by conventional light microscopy, enzyme histochemistry, and electron microscopy. Washed sheep erythrocytes were used as antigen. Splenic tissue was examined on the 13th, 18th, 21st, 27th and 48th day after antigen administration. Electron microscopic investigations revealed transitional forms between typical fibroblastic reticulum cells, which formed the framework of the entire splenic white pulp, and typical dendritic reticulum cells. During this transformation, the enzyme histochemical pattern of alkaline phosphatase disappeared and a positive alpha-naphthylacetate esterase reaction appeared in the transformed cells. On the basis of these findings, it is highly likely that dendritic reticulum cells develop through transformation of fibroblastic reticulum cells during the development of germinal centers in rabbit spleens. The characteristic folding of the surface membrane of dendritic reticulum cells is probably caused by the conspicuous increase in size of the Golgi apparatus, the detachment of vesicles, and the uptake of such vesicles by the cell membrane observed electron microscopically during the cellular transformation. Receptors that are of significance in antigen trapping might reach the cell surface in this manner, i.e., with the Golgi vesicles.

Electron microscopic studies of the innervation of the human spleen.

The innervation of four normal human spleens was investigated by electron microscopy. Unmyelinated nerve fibers accompanied the arterial vascular system up to the arterioles of the red pulp. Neither myelinated nerve fibers nor ganglion cells were seen in the splenic hilum or in the splenic tissue itself. The nerve fibers terminated against the smooth muscle cells of the blood vessels in a manner that is typical of the autonomic nervous system. The terminal axons contained small and large granular vesicles and thus were adrenergic nerve fibers. In contrast to the results of previous studies using silver impregnation methods innervation of the red or white pulp could not be demonstrated. The findings on human spleens agree with those on mammalian spleens obtained by other authors using ultrastructural and fluorescence histochemical methods.

[Enzyme histochemical, histometrical and ultrastructural studies of spleens in vinylchloride-disease (author's transl)].

By means of histometric, enzyme histochemical, and electron microscopic investigations it was demonstrated that the pathological changes in the spleen in vinylchloride-disease are primary. Fibroblastic cells are the only specific splenic cells involved. Fibre-associated reticulum cells of the red pulp and fibroblastic reticulum cells in white pulp are stimulated to produce excessive amounts of the extracellular elements of connective tissue, especially collagen fibrils. The newly formed connective tissue causes obliteration of extracellular blood channels in the red pulp and thus a reduction in the number of pulp-cord macrophages, and scarring of the periaterial lymphatic sheaths. The results of this fibrosing process are characteristic quantitative changes in the splenic histologic structures. These changes are different from those structural alterations occurring in spleens following extrasplenic hemodynamic changes, such as thrombosis of the splenic veins or cirrhosis of the liver.

[Lymphographic investigation of the face-neck-region (author's transl)].

Indirect lymphography is shown to be a technically uncomplicated and easy to use method of displaying the lymph nodes and -vessels of the face and neck regions. "Lipiodol-Ultra-Fluid" an oily contrast medium which has used in experiments with minipigs can be radiologically and histologically demonstrated in nodes for months. The histological findings in the course of a year are, at the beginning a phase of non-specific reaction followed by a phase of granulomatous reaction between two weeks and six weeks and finally a third phase where in the medium is stored virtually without histological reaction and is only reduced very slowly. Irreversible destruction of lymph-node structure as not infrequently follows the direct method - due to high-pressure application of the fluid was not evident. It may be assumed that disturbance of lymph node function or changes in lymph flow do not follow the application of the indirect method of lymphography.

Comparative histochemical and electron microscopic studies of the sinus and venous walls of the human spleen with special reference to the sinus-venous connections.

Sinus and venous walls of normal human spleens were studied with enzyme histochemical and electron microscopic methods. Particular attention was paid to the connections between sinuses and veins. Histochemically the sinus lining cells revealed a distinct naphthol-AS-acetate-esterase activity but no reaction for alkaline phosphatase. Venous endothelial cells were positive for the latter but negative for the former enzyme. In the sinus-venous junctional area there were no endothelial cells with reactivity for both enzymes. Electron microscopically both the sinus lining cells and the venous endothelial cells could be clearly characterized and therefore easily distinguished from one another on morphological grounds. There were no clear ultrastrural indications of transitional forms between sinus lining cells and venous endothelial cells in the sinus-venous area. According to these findings, sinus lining cells represent a specialized endothelium, but one with practically no morphological similarities to the venous endothelium.

[On the etiology of spindle-shaped acid-fast bodies in the human spleen (author's transl)].

Spindle-shaped acid-fast bodies have been identified only in lymph nodes till now. Their nature and their origin remained unclear. We have also found these bodies in the human spleen. Their staining reactions and their correlations to age, sex, weight of spleen, and to hemosiderin deposits were examined. They have been encountered in 50% of cases of hereditary spherocytosis and in 41.7% of cases of traumatic rupture of the spleen--but only in 2.3% of all other cases of groups of various other disorders. We conclude from our results that: 1. The spindle-shaped acid-fast bodies are made of ceroid. 2. They are not causative organisms and therefore cannot be of importance in the etiology of sarcoidosis. 3. They derive from increased destruction of erythrocytes. 4. They originate due to oxidative polymerization of membrane lipids. 5. They may also be found in absence of any fat-metabolism disturbance.
