Treatment of infected joint arthroplasty.

Sixty-two patients presented with an infected total hip arthroplasty. Their management depended on their general medical condition, the clinical signs of infection, the type of infection, the degree of fixation of the components and the available bone stock. Treatment consisted of one of the following: debridement and lavage without removing the prosthesis, one or two stage revision arthroplasty, or excision arthroplasty. There were 11 early and 51 late infections. The commonest bacterium isolated was Staphylococcus epidermidis (30%). Primary revision of the femoral component was slightly more successful with a cemented prosthesis than with an uncemented prosthesis. The overall success rates for cemented and uncemented femoral components were roughly comparable (91.5%:90%). We make various recommendations for the management of infected total hip arthroplasties based on our experience.

[Treatment of infected hip joint arthroplasty. Results of treatment of 62 infected total prosthesis arthroplasties].

The treatment of 62 cases of infected total joint arthroplasty of the hip is reported. The treatment regimen for each patient was dependent on the general medical condition of the patient and the clinical signs of infection, as well as the type of bacteria, the bony anchorage of the prosthesis and the bone stock of femur and acetabulum. Surgical treatment was one of the following: treatment of the infection leaving the prosthesis in situ; one-stage or two-stage revision arthroplasty; or excision arthroplasty (Girdlestone procedure). There were 11 early and 51 late infections. The commonest bacterium isolated was Staphylococcus epidermidis (30%). The primary success rate of revision of the shaft was somewhat better with cemented than with uncemented prostheses. The overall success rates for cemented and for uncemented shafts were similar (91.5% versus 90%).

[Therapeutic concepts in infected hip prosthesis].

In the period from January 1980 to December 1987, 62 patients with infected hip joint arthroplasties were treated. The duration of observation was on average 37 months (minimum 12 months, maximum 91 months). The choice of treatment modality depended on the type of infection, the state of anchorage of the endoprosthesis, the general condition of the patient, the type of bacterium and the state of the tissue surrounding the implant. 42 arthroplasties (67%) healed primarily. The remaining 20 were subject to 46 further operations (2.3 relapse operations/patient). 14 subsequently healed (23%) and six cases remained definitively infected.

[Anterior plating of the cervical spine with the hollow screw-plate system of titanium].

A hollow screw-plate system made of titanium for osteosynthesis of the cervical spine is presented. This system is designed to achieve a stable fixation for fusion of the vertebra. The posterior corticalis of the vertebral body need not and indeed cannot be perforated by the screws. Loosening of the screws is not longer possible because of growth of bone into the lumen of the screw and direct attachment of bone to the surface of the screw.

[Small cell carcinoma of the lung (author's transl)].

Of the various types of bronchogenic carcinoma small cell carcinoma has the poorest prognosis. It is the most malignant of the histological subtypes of lung cancer owing to its rapid growth and early, widespread metastases. Small cell carcinomas are radiosensitive and respond to cytotoxic therapy. In the Second Surgical Department of the Vienna University Hospital, 70 "early-stage" small cell carcinomas were resected between 1955 and 1974. 27.5% of the patients with stage I and II carcinomas survived for more than 5 years without symptoms. "Curative" surgery is selected cases leads to prolonged survival in patients with localized lesions.

Replacement of the anterior cruciate ligament. a comparative study of four different methods of reconstruction.

Eighty-seven patients have been examined 2 years on average after knee ligament reconstruction for a torn anterior cruciate ligament. The patients were divided into four groups according to the type of operation that had been carried out. In the first group an extra-articular lateral repair ("MacIntosh tenodesis") had been performed, in the second group an intra-articular "over-the-top" repair using the quadriceps and the patellar tendon, in the third group a modified "Eriksson" procedure using the patellar tendon, and in the fourth group a combined intra- and extra-articular repair using carbon fibres as a graft. The results of the operations in the different groups are compared. The best results were obtained with the "Eriksson" procedure, closely followed by the "over-the-top" repair. Limited range of motion and retropatellar pain resulting from changes in the alignment of the patella were the main problems. The results after the use of carbon fibres were less good. In two cases the graft tore without further trauma, and there were also problems because of restricted range of motion and retropatellar pain. The worst results were found after extra-articular lateral repair, due to insufficient stability in many cases. However, the best results with regard to the range of motion were found in this group.

[Long-term survival after pulmonary resection in bronchogenic carcinoma (author's transl)].

25 patients with bronchogenic carcinoma, who had undergone pulmonary resection between 1950 and 1964, were re-examined after a survival time of between 15 and 29 years. There was no relation between the long survival time and the duration of preoperative symptoms, the smoking habits prior to the operation or the localization of the tumour. The histological classification revealed that 14 patients had suffered from squamous cell carcinoma and 11 from large cell carcinoma. There was no case of adenocarcinoma nor of small cell carcinoma. In 7 patients tumour cells were demonstrable in the resected lymph nodes and in at least 2 cases the edge of the resected tissue was not tumour-free. In one case the blood vessels were invaded by tumour cells.

Clinicopathologic correlations in pseudocapsule formation after breast augmentation.

In 2,000 patients who underwent augmentation mammoplasties with different types of prostheses, the thickness of the pseudocapsules around gel-filled implants was greater than that of the pseudocapsules forming around inflatable implants. This observation was corroborated by an independent histologic study. Deposition of liquid silicone into the pseudo-capsules as well as the adjacent breast tissue and migration into capillaries was demonstrated. Until an impermeable shell or a non-transgressive gel can be manufactured, gel-filled implants should not be used.

[Treatment of esophageal strictures caused by reflux (author's transl)].

Chronic gastroesophageal reflux may cause intensive esophageal stenosis. Terminal distal strictures are rare, more frequent are strictures in the middle third of the esophagus. Neither conservative nor palliative measures offer satisfactory results in such cases. Only surgical intervention improves dysphagia and reduces complications. In some cases also the stenosis itself has to be corrected. Various surgical techniques as well as own experiences are reported.

[Three decades' experience in the treatment of bronchogenic carcinoma by pulmonary resection (author's transl)].

A review of the development of thoracic surgery is followed by a description of the results obtained by us in the surgical treatment of bronchogenic carcinoma from the difficult beginnings in 1947 up to the present day. The diagnostic investigations, indications for operation and the operative risks are discussed on the basis of 1532 pulmonary resections. The subsequent fate of this patients is known to us almost without exception over a period of three decades. Diagrams show the ten-year survival rates. The importance of early diagnosis and the priority of surgical treatment are two basic conclusions which still hold true today.
