[Value of cardiopulmonary function tests in the evaluation of dust-exposed patients].

On the basis of many years of own experience and evaluating a large number of judgements on exposure to silicogenic dusts-the results of those are reported at a different place-, a scale of function tests for judgement of silicosis is presented and discussed. It consists of largely applicable tests (examination of ventilation and arterial blood gases at rest and with standardized exercise). The results, in a modification of Knipping's method, are proposed in form of a table for scaling of bodily impairment. The importance of a solidly based first judgement is stressed.

[Interpretation of pneumoconiosis--results of cardiopulmonary function tests in dust-exposed patients with and without silicosis].

The retrospective evaluation of 472 judgements of professional disease in persons exposed to dusts showed that all observed restrictions of cardio-pulmonary function (obstructive lung disease, increase of residual volume, respiratory failure, pulmonary hypertension) occurred almost independently from the presence and stage of silicosis, but correlated first of all with the duration of exposure. Frequency and degree of functional disorders in exposed persons without radiologically demonstrable silicosis indicate that the fraction of nonspecific dusts, which is by far larger than the SiO2-fraction, produces chronic obstructive lung disease and its sequelae (emphysema, pulmonary hypertension). The term "pneumoconiosis" should therefore be used in a much broader sense than hitherto. This leads to considerations of the present practice of judgement on silicosis which are presented and discussed in detail.

[In memory of Oskar Wandel].

The 50th anniversary of Oskar Wandel 's death gives rise to the recognition that the county hospital St. George can look back on a venerable tradition, the continuation of which is a high obligation for those who are working in this institution at present.

[Methods and evaluation of clinical ergometer tests].

Taking into consideration biological calibration of ergometers the authors suggest load steps to be related to oxygen uptake. A pedalling rate of 45 rev/min should be used. At each stage of load an almost steady state should be reached, at which there is no further increase of the pulse rate. Thus, one load period will extend over 3--6 minutes. Intermittent rest pauses are required only for special reasons of method. The parameters to be measured depend on required data. In evaluating test results it should be considered that poor performance in an exercise test cannot only be caused by heart or lung diseases, but also by functional disturbances.

Cardiovascular Diseases↗

[Recent aspects of pharmacotherapy in obstructive airway diseases with special regard to bronchial asthma (author's transl)].

After demonstration of the pathophysiologic principles bronchial obstruction a review of the pharmacotherapeutic possibilities is presented. The main of therapeutics comprehends beta-2-receptor stimulants in combination with theophylline derivatives. Further therapeutic efforts with anticholinergic drugs (ipratropium bromide) preponderant in chronic bronchitis or intrinsic bronchial asthma respectively and with cromolyn sodium in extrinsic asthma are valuable. Glucocorticoids are allowed to be given only after exhaustion of all other modes of treatment, if possible as intermittent long term treatment. The stimulation of expectoration is an essential part of pharmacotherapy. The complex application of less specific methods of pharmacotherapy and physiotherapy belongs to the comprehensive treatment of patients. The threatening life asthmatic status demands the appropriate use of all possibilities of pharmacotherapy and intensive care if necessary.

[About the clinical picture of pulmonary heart disease with special regard to correlations between pulmonary function and pulmonary hemodynamics (author's transl)].

After introducing heart catheterization especially the right heart catheterization according to Grandjean the direct measurement of right ventricular as well as pulmonary artery pressure has been made possible in a relatively simple manner. Thereby the diagnosis cor pulmonale chronicum can be secured or omitted respectively with high confidence. In this regard the exercise--induced increase in pulmonary artery pressure has evident importance especially in case of clinical uncertainty. But because the right heart catheterization is not available in each patient suffering from a pulmonary disease distinct parameters (PaO2, FEV1, pulmonary residual volume, ECG, heart rate) can be used for recognition of the not yet decompensated state of the pulmonary heart disease considering the clinical situation as well as the correlation calculations pointed out by us. With increasing number of implicated functional parameters the security of diagnosis raises.

[Latest state of pharmacotherapy in the pulmonary heart disease (author's transl)].

With the exception of the decompensated state there is no special therapy in the pulmonary heart disease. Therapy is mainly directed to the original lung disease. Regarding the frequency of the ventilative-hypoxic type of the cor pulmonale chronicum the main thing of this lecture is the improvement of oxygen supply and thereby the lowering of pulmonary artery pressure pathologically raised by drug-induced bronchial dilatation.

[Improvement of drug therapy for essential hypertension].

Only 20-30% out of all the patients suffering from hypertension have sufficient treatment. In spite of full treatment a small part of the patients (about 5%) shows an insatisfactory result. Subjective factors are more important: on the part of physicians doubtfulness about choice of medicaments and imperfect consistency to long-term treatment, on the part of patients inadequate co-operation. The use of 9 selected antihypertensive medicaments is suggested in a decided order under rational aspects (mode of action, side effects, expense for controls, cost). In 1975-1978 development of consumption of antihypertensives and diuretics in the district of Leipzig corresponded to this conception. The respective medicaments placed at disposal in the district of Leipzig were sufficient to treatment of 30% out of all persons suffering from hypertension in 1975 and of 40% in 1978.

[Role of life style and nutrition in the etiology, prevention and therapy of coronary disease with special reference to lipid metabolism].

Issuing from the epidemiological situation of the angiocardiopathies the importance of the factors of risk for the coronary heart disease are emphasized with special consideration of the disturbances of lipometabolism. Essential constituent in the reduction of the factors of risk, particularly obesity, hyperlipoproteinaemia and hypertension, is the change of the habits of nutrition. An increased supply of manifold unsaturated fatty acids, an adequate to the disturbances of lipometabolism type-specific restriction of the cholesterol and carbohydrate intake and a reduction of the saline supply are to be recommended. The importance of measures in the field of health education which should begin already at school age is emphasized.

[Compulsory health education in invasive diagnostic procedures in cardiology].

The medical and juridicial side of the information duty concerning the basic character of a disease as well as diagnostic and therapeutic interventions are briefly discussed. By means of information paper for cardiological invasive examinations it is possible to deepen and to confirm the references given in the medical information talk. Apart from this the patient confirms by his sign that he was informed about the planned examination and that he agrees with it, which possibly is of importance for expertises.

Cardiac Catheterization↗

[Results of stationary management of patients with hypertension with reference to previous ambulatory care].

The case histories of 100 patients who were hospitalised in the Department of Internal Medicine of St. George's Hospital in the period from January 1975 to March 1976 are analysed mainly to study their treatment as outpatients (information of the patient, previous diagnoses and treatment, and diagnosis at the time of hospitalisation) and the results of their in-hospital treatment. Most of the patients were admitted to hospital at an advanced stage, usually when out-patient treatment had failed to produce results, but in 80% of the cases the result of in-hospital treatment was satisfactory or good. Besides improvement in drug combination and treatment of cardiac insufficiency, the most frequent complication, observation of general therapeutic principles under conditions of in-hospital treatment was found to be of special importance. Over and above this, we regard close cooperation of hospital and out-patient clinic as essential in the optimisation of the treatment of hypertension.

[Mechanics of breathing and exertion dyspnea in silicosis (author's transl)].

Mechanics of breathing as studied in patients with different stages of silicosis in rest and during exercise show the following: dynamic compliance and work against elastic and viscous resistances could not be found to be decisive indices of exertion dyspnea. Evidence of exertion dyspnea could only be achieved by means of the ration VT/PT (VT equals tidal volume, PT equals tidal esophageal pressure). Exertion dyspnea was observed at less than 0,081 VT per cm H20 PT. In accordance with Campbell and Howell, dyspnea is seen as an inappropriateness between ventilation and pressure needed.
