Effects of diclofenac sodium and He:Ne laser irradiation on plasmatic fibrinogen levels in inflammatory processes.

OBJECTIVE: The aim of the present work was to determine the possible synergic effect on the concentration of plasma fibrinogen (PF) by injecting diclofenac sodium associated with laser therapy postsurgery. SUMMARY BACKGROUND DATA: Nonsteroidal anti-inflammatory drugs (NSAIDs) and He:Ne laser irradiation as a therapy were used to inhibit the effects generated by inflammation. Tissue injury produces significant increases in PF levels, which are reduced to normal values by administration of NSAIDs or laser irradiation of the injured zone. MATERIALS AND METHODS: Rate were divided into groups in which different presurgical and postsurgical treatment were used; the inflammation was induced by laparotomy. RESULTS: Parenteral diclofenac or He:Ne laser irradiation used separately in normal rats did not produce changes in the PF levels. Diclofenac and laser irradiation combined postsurgery produced a significant reduction of PF levels compared with normal values, or with groups that were injected or irradiated postsurgically. CONCLUSION: The inflammatory response could be reduced by the effect of the diclofenac sodium upon the COX-2/COX-1 inhibitory relation associated with the photobiological effect of the He:Ne laser.

In vitro synthesis of prostaglandin E2 by synovial tissue after helium-neon laser radiation in rheumatoid arthritis.

This paper reports the effect of helium-neon laser radiation (power of 5 mW and 632.8 nm wave length) on the synthesis of PGE2 in vitro in synovial tissue of biopsy samples of knee joints in patients with chronic rheumatoid arthritis stages II or III. Twelve patients were studied. Each patient received 15 applications of He-Ne laser. Eleven points for He-Ne laser applications were selected in one of the affected knees. The energy density used was 8 J/cm2 per application point. The He-Ne laser therapy reduced the synthesis of PGE2. The analysis of the data revealed a statistically significant difference between the levels of the synthesis of PGE2 before treatment (17.69 +/- 2.65 ng mg-1 of dry tissue h-1) and after treatment (13.85 +/- 2.73 ng mg-1 of dry tissue h-1), with p < 0.01 comparing mean values. This was also accompanied by relief of pain (91.6%), and a favorable subjective report from the patient. We conclude that PGE2 is a quantifiable parameter that could explain what causes pain relief in patients with rheumatoid arthritis that are treated with He-Ne laser.

Structural changes induced by He-Ne laser on the chick embryo ovary.

UNLABELLED: The morpho-histochemical alterations that occur in the chicken ovary at 7 days of incubation after irradiation with He-Ne laser of a potency of 5 mw and at a wavelength of 632.8 nm were studied. The embryos were irradiated for 5 minutes through a window opened in the eggshell and aseptically maintained in incubator for 24 hours. The gonads were dissected out and processed for the following techniques: H/E, PAS, alcian blue, and toluidine blue. CONTROLS: The ovaries were formed by a germinative or superficial epithelium, with germ and epithelial cells, and by primary sex cords compressed between them, although separated by a reduced stroma. The cords contained germ cells The surface coat of the germinative epithelium presented a thin layer of PAS positive, alcianophilic at pH 25 and orthochromatic material. Basement membranes and intercord extracellular substance were also PAS positive. PROBLEMS: Disorganization of the tissue structure was well manifest in irradiated gonads, accompanied by negativization of the histochemical reactions. A lymphocytic infiltration was also found. No structural alterations were observed in germ or epithelial cells. It is concluded that laser radiations would act producing decrease of the mucosubstances associated to the plasma membrane and basement membrane. They would also provoke the appearance of an inflammatory mononuclear infiltration.

Expert computer program for the management of laser surgery.

Since 1960 when the first laser was produced, different types of lasers were developed and over one hundred of them are actually used in medicine. These facts makes very difficult for a surgeon to optimally utilize different types of laser in varying surgical circumstances, unless a laser expert is actually participating in the procedure. As this presence is impractical, if not impossible in every case, we are offering a computer program which presents information in a flexible and friendly way; it is self perfecting and greatly expandable.

[Proposal for the teaching and application of informatics at medical schools].

Informatics is the discipline that process efficiently all the necessary data to obtain information. The data acquisition, processing and interpretation is realized through traditional as well as automated means. Medical Informatics is the union of all methods of informatics in medicine including the preparation of medical data required for the application of these methods. Due to the need to keep up with the increasing amount of data that modern medicine is receiving and efficiently process it to obtain meaningful information, we propose the creation of a department of Medical Informatics in our Medical School to: 1) Teach the basic principles of medical informatics to undergraduate and graduate students, including lectures in: Information technics, medical terminology, medical linguistics, international classification of diseases, Hospital informations Systems, practical application of computing in medicine as Oncocyn, Mycin, etc., as well as external data bases. 2) Help the health sciences personnel to obtain and transfer medical information through the National and International Electronic Networks of Medical Information.

Effects of helio-neon laser radiation upon cellular cycle in a plant model.

The scope of this study was to investigate possible relationships between He-Neon laser radiation and mitotic and phase indices in meristematic cells of Allium cepa L. bulbs. Our results indicate that mitotic index increased after irradiation depending this modification on the time exposure and the potency of the He-Neon beam. Phase indices were also modified: frequency of prophase increased, while inter- meta- and anaphase decreased: telophases remain unchanged. These variations were significative only when the preparations were irradiated a) with 5 mW for 10 min. or more, b) with 10 mW or c) when the preparations were processed 60 min. after irradiation. These findings could not be attributed to thermal changes. Modifications in RNA or protein synthesis could be responsible.

CO2 laser in decubitus ulcers: a comparative study.

This is a comparative study of conventional surgery (knife/electric knife) and CO2 laser surgery and their effect on the outcome of stage III (including muscle) and stage IV (including bone) decubitus ulcers with an average area of 400 cm2. It is a prospective clinical study done with two groups of patients of similar age, economic and social background, as well with similar types of decubitus ulcers. Thirty patients were studied in each group. The study demonstrated a statistically significant difference in favor of the patients treated with CO2 laser with regard to operative blood loss, 0.5 cc/cm2 or 20% (P less than .01), infection rate (chi 2 test, P less than .01), and difference (60%) in recovery time (P less than .01). This translated into an average saving of 23 hospitalization days per patient. It also showed a difference, though not statistically significant, favoring the group treated with laser with respect to survival and full recovery. This probably is attributable to the lowered blood loss and infection rate as well as improved operative time and analgesia.

The use of an integral tissue expander for primary breast reconstruction.

The authors emphasize the recent advances in breast reconstruction provided by use of the skin expander device. A Silastic percutaneous skin expander developed by the authors and the Dow Corning Corporation has been used by them in breast reconstruction for the past 7 years with satisfactory results. Important design differences in the device suggested by the authors include the use of a reinforced backing which consists of Dacron Silastic sheeting extending beyond the inflatable tear-drop-shaped envelope for approximately 3/8 of an inch, thus adding strength and rigidity to the envelope and the rim, and making it possible for the rim of the expander implant to be sutured in an appropriate orientation.
