Adult, acquired reactive perforating collagenosis. Report of a case including ultrastructural findings.

A case of reactive perforating collagenosis in an adult male patient with chronic renal failure. Pruritic, umbilicate papules, showing extrusion of collagen fibers through the epidermis were detected histopathologically. Electron microscopy showed absence of basal membrane beneath the perforation and collagen fibers with preserved periodicity passing through widened intercellular spaces with islands of cytoplasmic material.

Atopic dermatitis, house dust mite allergy and month of birth.

An analysis of the month of birth in 210 patients with atopic dermatitis and a positive prick test towards the house dust mite (Dermatophagoides pteronyssinus) showed a significantly higher prevalence of birth in the interval May to November when compared with the expected distribution (p less than 0.01). A subdivision of the AD patients due to the concomitant presence of asthma showed no differences as the relative risk was nearly the same in the two subgroups. This observation indicates that a consistent seasonal preference of birth in patients with atopic dermatitis only applies to the skin disease and not to coexistent asthma.

IgE in atopic dermatitis: a study of the intercellular fluid.

In 33 adult patients with atopic dermatitis of mild to severe degree the concentration of IgE in serum (S) and intercellular fluid (IF) was studied by suction blister technique. The median value of IF/S-IgE was 0.20 and not different from the ratio 0.21 found in controls. Furthermore, by subdividing the patients into groups with asthma/rhinitis or different severities of eczema no significant differences in the IF/S-IgE ratios were found compared with the controls. In the controls the IF/S-IgE ratio (0.21) was significantly different from the IF/S-IgG ratio (0.32). The median ratio of IF/S-IgE divided by IF/S-IgG was 0.63, which might indicate a reduced passage of IgE into IF compared with IgG presumably dependent on differences in the molecular size or tertiary structure of the proteins.

Penicillin concentrations in serum following weekly injections of benzathine penicillin G.

Twelve patients with syphilis were treated weekly with injections of 1.44 g (2.4 X 10(6) IU) of benzathine penicillin G for up to 3 weeks. Almost daily, serum penicillin concentrations were measured by a sensitive microbiological agar cup method. An individual and interindividual variation was found. Concentrations below the recommended 0.018 micrograms/ml were found 7 days after the first or second injection in 5 samples. Shorter intervals between injections are recommended.

Parathyroid hormone and calcium metabolism in generalized scleroderma. Increased PTH level and secondary hyperparathyroidism in patients with aberrant calcifications. Prophylactic treatment of calcinosis.

Parathyroid hormone (PTH) in serum and biochemical parameters of calcium metabolism were analysed in 45 patients with systemic sclerosis. Calcification of the skin and subcutaneous tissue was assessed by X-ray examination of the hands. Analyses disclosed secondary hyperparathyroidism (increased PTH in serum, low calcium 'ion' in serum, decreased urinary excretion of calcium and phosphate), in particular in patients with calcinosis (P less than 0.05) as compared to those with no calcinosis. The duration of systemic sclerosis was longer in patients with calcinosis (P less than 0.05). The calcinosis type of systemic sclerosis is characterized by secondary hyperparathyroidism developed during the progression of the disease. A hypothesis is made regarding calcium metabolism in the early no-calcinosis (with increased synthesis of Vitamin D) and late calcinosis types. PTH may stimulate aberrant calcification. The hypothesis implicates that prophylactic treatment with Vitamin D in low dose may prevent calcinosis.

Increased central cornea thickness in systemic sclerosis.

Central cornea thickness (CCT) was measured in 32 patients with systemic sclerosis by the Haag-Streit pachymeter with improved centrality. Results were compared with measurements in 29 healthy adults matched with respect to sex and age. CCT was increased (P less than 0.001) in patients with systemic sclerosis (mean 0.56 mm, average SD right and left side 0.0297 mm) as compared to the controls (mean 0.51 mm, SD 0.0109 mm). CCT of right and left eye was increased in 69% and 72% of patients with systemic sclerosis as compared to controls (mean 0.51 mm +/- 2 SD). CCT increased during the first 8 years of the disease (correlation coefficient 0.593) reaching a plateau after 8 years (correlation coefficient 0.005). CCT did not increase during medical treatment with collagen inhibitors. Measurement of CCT may be useful as a supplement to other quantitative methods for diagnosis and control of systemic sclerosis.

Age at menopause of females with systemic sclerosis.

Clinical symptoms of menopause were recorded in 27 females with systemic sclerosis, mean age 53.5 years (range 22-68). Five had regular menstruation, and 22 were in the postmenopause, 2 with climacterium praecox. Mean age at menopause was 47.8 years (range 30-59) among the 22 females, and 49.3 years (range 40-59) among the 20 females with a normal climacterium. Eleven had physiological subjective symptoms of menopause. The 22 females had 34 living children, and 13 abortions. It is concluded that the menopause is normal in females with systemic sclerosis. This indicates that the ovaries are normally not involved in the disease.

Hair changes due to zinc deficiency in a case of sucrose malabsorption.

A 7-year-old girl suffering from chronic diarrhoea due to sucrase deficiency was referred because of poor hair growth. Her scalp hair had a poor, colourless appearance and was much thinned in the occipital region. Her skin was dry, but otherwise normal. P-zinc was low (7.9 mumol/l), whereas P-albumin was normal. Oral zinc therapy, 40 mg daily, had a marked beneficial effect on her scalp hair, eyebrows and eyelashes, which became thicker and pigmented. Beau lines appeared on thumb-nails and 4th left finger-nail. A rise in P-zinc and S-alkaline phosphatase levels was observed during the zinc supplementation. Microscopic examination of her poor scalp hair, using polarized light, revealed well-defined abnormalities of the hair shafts, as reported by others in a case of acrodermatitis enteropathica: 1) a marked individual variation in diameter, 2) narrowing often associated with waving or sharp bending and broken ends, 3) striation with a tendency to trichonodosis. Such changes were absent in the pigmented hair appearing after the start of zinc therapy.

Pachymeningitis cervicalis hypertrophica syphilitica.

A rare form of neurosyphilis with hypertrophic cervical pachymeningitis is presented. It is concluded that serological tests for syphilis are still indicated in patients with unexplained neurological or psychiatric symptoms.
