[Orbital blow-out fractures. Therapeutic results].

37 patients treated surgically for orbital blow-out fractures were reviewed retrospectively. 29 had diplopia before and four after operation. The diplopia was present only in extreme gaze positions and was not troublesome to the patients. Enophthalmus was present in 19 patients preoperatively and in six postoperatively. Of six patients treated conservatively one developed a moderate enophthalmus. There were no significant postoperative complications.

Glaucomatocyclitic crises (Posner-Schlossman syndrome). A case report.

A white man aged 40 suffering from glaucomatocyclitic crises, was treated with trabeculectomy during an acute attack. The tissue specimen showed numerous mononuclear cells in the trabecular interspaces. The hypertensive attacks are supposed more to be due to blockage of the aqueous outflow pathway by these cells, and less to increased aqueous formation.

Scanning and transmission electron microscopy of Müller cells isolated from rabbit retina.

Proteolytic enzymes were applied in the dissociation of rabbit retina to obtain isolated Müller cells. Both trypsin and papain were used, and in some cases the trypsinization was shortly interrupted by tissue fixation before it was continued. Scanning electron microscopy mostly confirmed the previous descriptions of these cells, which were based on different techniques. In addition, some new cell extensions were observed that derived from the nuclear region. The papain procedure ensured the best cell preservation with a free surface and limited contaminant material attached. Roughly 90% of the cells excluded trypan blue. It is concluded that these cells should be preferred for biochemical studies, because their cytology also turned out to be intact as judged from electron microscopic sections.

Laser trabeculoplasty in open angle glaucoma.

We have treated 54 eyes that were considered candidates for trabeculectomy with argon laser trabeculoplasty. One hundred burns were placed in the trabecular meshwork right in front of the scleral spur in the lower two quadrants. Average prelaser IOP was 26.2 mmHg and average postlaser IOP was 18.3 mmHg. Four eyes (7.4%) went to further surgery. Mean follow-up time was 3.5 months. There were few complications. Most important was a transitory increase of IOP larger than 10 mmHg in 7 eyes (13.0%). There was one case of acute iridocyclitis.

Focal macular choroidopathy.

The clinical and angiographic findings in 2 patients with focal macular choroidopathy (FMC) are described. Follow-up fluorescein angiography revealed that the initial lesions consisted of multifocal, whitish spot-like areas of choroidal infiltration with hyperfluorescence in the late phase angiogram. At a later stage of the disease, the developing disciform macular lesion showed subretinal neovascularization. The disease was bilateral with poor visual prognosis. There was no evidence of infection with Histoplasma capsulatum. Early oral corticosteroid treatment could not prevent the progressive nature of the disease.

The missing rectus: a CT observation from blow-out fracture of the orbital floor.

A patient with a blow-out fracture of the right orbital floor presented with impediment of the inferior rectus muscle. Coronal CT disclosed a lateral hinged fragment from the orbital floor and herniation of intrabulbar content into the maxillary sinus. The inferior rectus muscle could not be identified intraorbitally in three sequential 3 mm coronal sections and it was concluded that the muscle was incarcerated, a presumption that was verified at surgery.
