[Mastery of conflictual relationship patterns in the course of a psychodynamic focal therapy].

Mastery is a psychological construct rooted in the psychodynamic psychotherapy research. The Mastery Scale provides a reliable content analytic method to measure the mastery of maladaptive interpersonal conflicts. With the help of model-sentences verbatim transcripts of relationship episodes are assessed regarding to six hierarchical so-called levels: lack of impulse-control, introjection and projection of negative emotions, difficulties in understanding and cognitive control, interpersonal consciousness, understanding of the self and self control. Further, mastery capabilities can be quantified by a mastery score. For the first time, the instrument is introduced in German by a study of a rather well known successful psychodynamic short-term psychotherapy which previously has been investigated with several empirical methods. The results verify the possibilities of the Mastery Scale in subtly describing changes in the course of a psychotherapy.

Stereotypical relationship patterns and psychopathology.

BACKGROUND: We explored the relationship between the consistency of relationship patterns and the severity of psychopathology. METHOD: Relationship patterns were assessed by means of Relationship Anecdote Paradigm interviews rated according to the Core Conflictual Relationship Theme (CCRT) method. The repetition of the same type of CCRT components across relationship narratives indicated stereotypical patterns. RESULTS: Subjects treated in an inpatient setting (n = 25) told narratives with more consistent patterns than subjects in an outpatient setting (n = 32). Relationship episodes of normal adults (n = 23) were more flexible compared with the two clinical groups. Especially repetitions of the wish component were closely associated with the severity of psychopathology assessed by SCL-90R. CONCLUSIONS: The consistency of relationship patterns seems to be connected with the severity of psychopathology.

[A multicenter study of expenditure and success in psychodynamic therapy of eating disorders. Study design and initial results].

UNLABELLED: Evaluation of psychodynamically oriented treatment of anorectic and bulimic patients, taking into account both quality and cost of therapist ("How much therapy is applied to which patients for a successful treatment?"). Criteria for indication of the kind of therapy and of amount of therapy ("dosage") are provided. METHOD: Prospective naturalistic longitudinal study (comparable to phase IV of effectiveness studies on pharmaceuticals) including 2.5 year follow-up. Consecutive sample (Anorexia nervosa and bulimia nervosa, DSM-III-R). Multidimensional outcome measures including self-rating and expert-ratings. Operationalisation of outcome is layered. Differentiated measurement of amount of therapy and treatment elements. Survival analysis, logistic regression. RESULTS: 1171 completely documented treatment episodes of 43 institutions (30.3% Anorexia nervosa, 55.3% bulimia nervosa, 14.4% double diagnoses). Patients are chronically ill (M = 7.6 years; SD = 6.3 years) and at the beginning of (mostly inpatient) treatment of the index episode 25.5 years of age (SD = 6.0 years). Therapies last appr. 11 weeks (median) and encompass a broad range of therapy measures. DISCUSSION: Study conduction as well as data collection and analysis of this worldwide largest study on treatment of eating disorders is discussed by presenting preliminary results.

[Diagnosis of attachment disorders].

After an introduction about the basic concepts of attachment theory the diagnostic classification systems ICD, DSM and ZERO TO THREE are reviewed for descriptions and diagnosis of attachment disorders. It is demonstrated how over the last few years attachment theory has been partly adopted in those diagnostic systems. In addition different types of clinically relevant attachment disorders are described. The authors argue that it would be necessary to integrate attachment disorders on a broader base into the current diagnostic classification systems. The diagnostic importance of attachment disorders and their impact on the technique of child psychotherapy are finally discussed.

[Development, attachment and relationship: new psychoanalytical concepts].

Authors present the evolution of psychoanalytical views of how a child perceives and contruct the world. They begin with Freud's thought and go through works of Klein, Spitz, Mahler, Bowlby, ending with modern thought of Stern. Developments in understanding of the infant by observations taken through the last thirty years are outlined and implications for psychoanalytical theory and treatment of mental problems are described.

[Music therapy single case process research--development and organization of a research field].

Music therapy yet to be discovered as a target of psychotherapy research. A first step consists in creating adequate samples of significant musical dialogues. Video-recording is an essential requirement for establishing such data. To test the clinical view that the emotional content of the musical dialogue in music therapy reflects the basic relationship issues of a patient was the objective of this study. By using ratings of significant musical dialogue sequences with the instrument of semantic differential comparing groups with varying clinical expertise (music therapists, psychoanalyts, lay persons) the results demonstrate that there is considerable stability of the judgments; however, systematic differences exists among the three groups of raters.

[Coping with bone marrow transplantation. A contribution to adaptive vs. maladaptive coping].

The coping strategies of 36 adult leukemia patients admitted for bone marrow transplantation have been investigated systematically by trained raters using audio-taped material from semi-structured interviews. So-called long term survivors with a median survival time of 978 days after BMT displayed significantly lower passive-stoic acceptance and resignation than non-survivors who died within a median time of 229 days after BMT. These copings were of high prognostic value prior to all treatment regimes in strong correlation with a negative hematological prognosis and age. Non-survivors showed significantly higher passive-stoic acceptance and resignation coping. Form of leukemia and other sociodemographic variables or stress had neither a significant impact on coping intensity nor on coping quality. Consequences for further coping research and possible treatment interventions prior to or immediately after BMT are being discussed.

[The core conflictual relationship theme of core relationship patterns. A further methodology-critical development of the "core conflictual relationship theme" method].

The Core Conflictual Relationship Thema Method (CCRT) by Lester Luborsky is based on the analysis of narratives about relationship episodes within transcripts of therapy sessions. Wishes, responses of the other and responses of the self are identified and counted. The critical explanation with the CCRT-Method conducted to alternative methods of data analysis. In the result we have not only one Core Conflictual Relationship Theme (CCRT), but we analyse the structure of Central Relationship Pattern (CRP). We set an example with a completely judged brief psychodynamic psychotherapy with about 300 relationship episodes.
