New form of platyspondylic lethal chondrodysplasia.

We report on a sporadic case of hitherto unknown lethal skeletal dysplasia. The cardinal clinical manifestations consisted of frontal bossing, cloudy corneae, low nasal ridge, and micrognathia, hypoplastic thorax, and rhizomelic micromelia. Laryngoscopy and neck CT disclosed laryngeal stenosis, and brain CT demonstrated hypoplasia of the corpus callosum. Skeletal survey demonstrated hypoplasia of facial bones and short skull base, extremely severe platyspondyly, hypoplastic ilia, and delayed epiphyseal ossification and rhizomelic shortness of tubular bones. The long bones appeared overtubulated with exaggerated metaphyseal flaring. The humeri were particularly short and bowed. Bowing of the radii and ulnae with subluxation of radial heads presented as a Madelung-like deformity. Unlike the long bones, the short tubular bones were not short and normally modeled. The skeletal changes were superficially similar to those in a group of lethal platyspondylic chondrodysplasias, but were inconsistent with any known subtypes of this group or other lethal skeletal dysplasias.

Effect of orally administered heat-killed Enterococcus Faecalis FK-23 preparation on neutropenia in dogs treated with cyclophosphamide.

Dogs injected intravenously for 3 days with cyclophosphamide (CY) at a dose of 10 mg/kg were given 100 mg/kg of Enterococcus faecalis FK-23 preparation (FK-23) perorally for 14 days to confirm the beneficial effects of the latter drug in neutropenic dogs when orally administered. Although FK-23 treatment did not inhibit CY-induced neutropenia, it augmented neutrophil-reconstituting capacity in these dogs. Increases in the myeloid/ erythroid ratio and neutrophilic lineages were found in the bone marrow of FK-23 administered dogs. The oral administration significantly restored the reduced activity of neutrophil phagocytosis and chemiluminescence in dogs treated with CY. These findings indicate that FK-23 administered perorally not only augments neutrophil reconstitution through the activation of bone marrow but also functions in dogs treated with CY. It may thus be a useful supportive agent to reduce the adverse side-effects associated with the administration of chemotherapeutic agents such as CY.

Optimal dose of enterococcal preparation (FK-23) supplemented perorally for stimulation of leukocyte reconstitution in dogs treated with cyclophosphamide.

Optimal doses of orally supplemented heat-killed Enterococcus faecalis FK-23 preparation (FK-23) were evaluated in dogs treated with cyclophosphamide (CY). Leukocyte reconstitution was stimulated by the supplement of 100 mg/kg of FK-23 through an induction of neutrophilia, and the treatment of 10 mg/kg of FK-23 induced mild neutrophilia in dogs tested. However, the reconstitution was not stimulated by the treatment of 200 or 400 mg/kg of the drug. Although the effects of FK-23 appeared to be milder than those of recombinant human granulocyte colony stimulating factor (rhG-CSF), there was no significant difference in the leukocyte reconstitution between the two substances. An appropriate dose of FK-23 is 100 mg/kg for augmentation of leukocyte reconstituting capacity in dogs treated with CY.

[A clinical study on the efficacy of ceftazidime alone or in combination with aspoxicillin against infections in obstetric and gynecological patients].

We studied clinical effects of ceftazidime (CAZ) alone or in combination with aspoxicillin (ASPC) against various infections in obstetric and gynecological patients. 1. Obstetric and gynecological patients (n = 91) with various infectious diseases were treated with CAZ alone (1-2 g x 2/day, n = 54) or in combination with ASPC (1-2 g x 2/day, n = 37) administered via drip infusion. 2. CAZ alone or in combination with ASPC was efficacious in 50 out of 54 (92.6%) or 33 out of 36 (91.7%) patients, respectively. Overall, the efficacy ratios were 46/49 (93.9%) against gynecological infections, 21/25 (84.0%) against perinatal infections and 16/16 (100%) against other infections. The bacteriological efficacy ratio was 21/21 (100%) while clinical effectiveness in cases in which causative agents were known was observed in 20 out of 21 (95.2%) patients. In patients who had not respond to other treatments, CAZ alone, and in combination with ASPC were effective in 15 out of 16 (93.8%) and 6 out of 8 (75.0%) patients, respectively, hence the overall efficacy ratio was 21/24 (87.5%). 3. Abnormal values in clinical laboratory tests were obtained in 3 out of 91 (3.3%) patients. No other adverse side effects were observed in any of the patients.

Comparisons of the bacterial flora in genital regions at non-pregnancy.

In the development of infectious diseases at non-pregnancy and at pregnancy, correlations between bacterial flora in the vagina and portio vaginalis and the ascending infections of those bacteria have recently been discussed. To clarify the cause of those infectious diseases, we studied the localization of microorganisms in genital regions. Patients undergone abdominal total hysterectomy (n = 172) were employed as subjects, and microorganisms isolated from 4 genital regions were studied. In addition, the preventive effect of cefmetazole (CMZ) against postoperative infections was analyzed in 479 cases including the hysterectomy cases mentioned above. The results obtained are summarized as follows: 1. The isolation rate of microorganisms at non-pregnancy, from subjects of 30 to 69 years old, was 65.6% (82/125) in the vagina and portio vaginalis, 52.1% (25/48) in the cervical mucus, 7.3% (9/124) in the uterine cavity and 0% (0/47) in the ovarian surface. 2. Numbers of microorganisms isolated in each region were 99 strains in the vagina and portio vaginalis, 28 in the cervical mucus, 10 in the uterine cavity and none in the ovarian surface. Isolation of aerobic Gram-positive bacteria (60-89.3%) and anaerobic Gram-positive bacteria (7.1-30%) were varied in each region. Lactobacillus spp. (38 strains), Staphylococcus epidermidis (20 strains) and Propionibacterium acnes (10 strains) were isolated from vagina and portio vaginalis, and Lactobacillus spp. (17 strains) were the most often isolated bacteria from the cervical mucus.(ABSTRACT TRUNCATED AT 250 WORDS)

Embryonic heart rates: development in early first trimester and clinical evaluation.

One hundred and forty-three women in the early first trimester of gestation were examined 364 times using transvaginal sonography, and the development of embryonic heart rate was studied. In each case gestational age was revised retrospectively by either recorded basal body temperature or ultrasound crown-rump length dating between 9 and 10 weeks. Embryonic cardiac activity could be detected as early as 37 days of gestation. In 133 continuing pregnancies, embryonic heart rate rose from an average of 97.7 beats per min at 36-38 days to 174.7 beats per min at 60-62 days. A significant correlation was seen between gestational age and embryonic heart rate (p less than 0.001). The regression equation for heart rate was as follows: heart rate = 3.850 x gestational age (days) -54.64 (r = 0.908, n = 347), in short, embryonic heart rate continued to rise about 4 beats per min every day until 8 weeks of gestation. In this series, 10 pregnancies resulted in spontaneous abortion in the first trimester, and all of them showed relative bradycardia. Embryonic heart rate measurements in 8 of them were below the 95% prediction intervals for normal heart rate plotted against gestational age. This study suggests that embryonic heart rate measurement by ultrasound may be a new method for dating early first trimester, and that first trimester bradycardia may be associated with a poor prognosis for the pregnancy.

[Clinical study of intramuscular imipenem/cilastatin sodium in the field of obstetrics and gynecology].

We evaluated the clinical efficacy and safety of intramuscular (as a new route of administration) imipenem/cilastatin sodium (IPM/CS) in patients with intrauterine infections which are typical in the field of obstetrics and gynecology. The obtained results are summarized as follows. 1. Twenty-seven patients were treated with IPM/CS, 250 mg/250 mg b.i.d. (3 patients), 500 mg/500 mg b.i.d. (22) and other dosages (a change in dosing regimen, 2). The duration of treatment ranged from 3 to 11 days and the total dosage during an entire course of treatment varied from 1.5 to 9.0 g. The drug was suspended in a lidocaine solution and administered in the gluteal muscle of the patients. 2. Clinical efficacies were excellent in 7 patients (26%), good in 19 (70%) and poor in 1 (4%) and the overall efficacy rate was 96.3%. All of the 8 patients who had not previously showed improvements with treatment by other antibiotics responded well to this drug. 3. Bacteriologically, the clinical efficacy rate was 95.8% (23/24) and the eradication rate was 76.2% (16/21). 4. No adverse effects due to the drug were observed. As abnormal laboratory test results, transient elevations of GOT and GPT were noted in one patient.

[Clinical effect of the combined use of aztreonam, amikacin and clindamycin in infectious disease in obstetrics and gynecology].

Clinical effects of combined use of aztreonam (AZT), amikacin (AMK) and clindamycin (CLDM) in 46 cases with infectious diseases in obstetrics and gynecology were retrospectively studied in 2 groups, and the following results were obtained. 1. No significant difference in efficacy rates was noted between AZT plus CLDM treated group (n = 25) and AMK plus CLDM treated group (n = 21) (96.0% vs. 95.2%), while rate of excellent efficacy was slightly higher in AZT plus CLDM group than AMK plus CLDM group (24.0% vs. 14.3%). 2. No significant difference in bacteriological clinical effects was also noted between the 2 groups, while bacteriological eradication rate was higher in the AZT plus CLDM group than in the other group (76.2% vs. 50.0%), and the difference was particularly clear in the eradication rates of aerobic, Gram-negative bacteria (88.9% vs. 30.0%). 3. Subjective and objective side effects, and abnormalities of clinical test results were not found in either group.

Estrogen-binding protein in blood and follicular fluid, and its biochemical properties in human females.

Testosterone-estradiol binding globulin (TeBG) has been known to have specific binding to estradiol (E2) in blood, however an unknown binding protein having higher affinity to E2 than TeBG is thought to exist in blood. Therefore blood serum and follicular fluid were collected in normal females. Ammonium sulfate precipitation showed different maximum bound ratio between E2BP and TeBG. Concanavalin A (Con-A) Sepharose adsorption analysis showed that the Con-A-bound phase demonstrated estradiol-binding protein (E2BP). Isoelectric focusing showed TeBG in pH 4.9 and E2BP in pH 3.9. The substance giving the peak at pH 3.9 has a high affinity to dehydroepiandrosterone sulfate and E2, but the substance with peak at pH 4.9 shows a high affinity to 5-dihydrotestosterone and testosterone. E2BP changed concomitantly with total E2 in blood in the menstrual cycle, but in follicular fluid E2BP was found only in small amounts. It consisted of two components, one protein with low affinity binding and the other with high affinity binding. These results suggest that E2BP exists as a specifically bound fraction to E2, while TeBG, as a nonspecifically bound fraction to E2, is not directly involved in the E2-related biological function.

[Prophylactic effects of a combined use of a cephem antibiotic, latamoxef, and tobramycin on postoperative infection in obstetrics and gynecology].

To prevent postoperative infection in obstetric gynecology, latamoxef (LMOX) and tobramycin (TOB) were intravenously administered to 81 patients at daily doses of 2 g and 120 mg, respectively, for 5 postoperative days, and the preventive effect of the combined drugs on postoperative infection were evaluated in terms of the clinical effects and safety. 1. The fever index was low as a whole; 3.38 +/- 2.30 degree hours in patients receiving simple panhysterectomy (n = 61), 3.21 +/- 3.84 degree hours in those given cesarean section (n = 12), and 3.53 +/- 2.78 degree hours in those given other surgical procedures (n = 8). 2. There were no abnormalities in hematological findings or liver and kidney functions 14 days after the surgery. PIVKA-II was observed in 3 of 32 patients 14 days after the operation, at a rate of 9.4%. 3. Except mild diarrhea which was observed in 7 of the 81 patients (8.6%) in 4 to 6 days after the operation, no subjective or objective side effects or postoperative complications were observed. The combined use of LMOX and TOB seems useful for preventing postoperative infection in obstetrics and gynecology in terms of its efficacy and safety.

[Androgen binding components in the human placenta].

In the present study, characterization of the androgen binding components in human term placenta was undertaken. Scatchard analysis revealed the presence of high affinity binding components with dissociation constants (Kd) of 1.5 X 10(-10)M and 3.7 X 10(-9)M for dihydrotestosterone (DHT) in hormone-striped placental cytosol. Isoelectric focusing (IEF) after equilibration with 20 nM 3H-DHT indicated three binding activities with elution pH (Pe) of 4.6, 5.3 and 6.0. The placental cytosol was then fractionated into Concanavalin-A Sepharose (Con-A). The Con-A unbound fraction contained two heat labile binding populations for DHT with Kd's of 1.9 +/- 0.4 X 10(-10)M and 5.9 +/- 1.2 X 10(-9)M (mean +/- SE: n = 6), and two peaks on IEF with 20nM 3H-DHT with Pe's of 4.6 and 6.0. When IEF was performed with 2 nM 3H-DHT, only one peak was observed with a Pe of 6.0. The Con-A bound fraction contained a heat stable, single binding population for DHT with a Kd of 2.2 +/- 0.2 X 10(-10)M and a Pe of 5.0. It appears that the binding population is testosterone-estradiol binding globulin (TeBG). In competitive binding studies, androstenedione, dehydroepiandrosterone and dehydroepiandrosterone-sulfate had much higher affinities for Con-A unbound fraction than Con-A bound fraction. Therefore, it appears that there are at least two androgen binding components in human term placental cytosol, demonstrable after removal of TeBG contamination, that can be shown to have different affinities for DHT and, probably, separate Pe's.

A plasma/serum estrogen-binding protein distinct from testosterone-estradiol-binding globulin.

A plasma/serum estrogen-binding protein (E2BP) which is distinct from testosterone-estradiol-binding globulin (TEBG) has been documented. This high affinity, low capacity estrogen binder is readily detectable in the mannoglycoprotein fraction of serum or plasma. It can be distinguished from TEBG by 1) its isoelectric elution pH (pH 3.9 as distinct from pH 4.9 for TEBG); 2) its sedimentation value on sucrose gradients (a lower sedimentation value compared to TEBG); and 3) its steroid specificity (a high affinity for diethylstilbestrol with essentially no affinity for dihydrotestosterone or testosterone, in contrast to TEBG which has no affinity for diethylstilbestrol and a high affinity for dihydrotestosterone and testosterone).

Single administration of enterococcal preparation (FK-23) augments non-specific immune responses in healthy dogs.

Influence of a single administration of heat-killed Enterococcus faecalis FK-23 preparation (FK-23) on non-specific immune responses was evaluated in healthy dogs. The dogs received a single administration of FK-23 at a dose of 100 mg/kg perorally and thereafter a complete blood count (CBC), leukocyte differential count, phagocytic activity of peripheral blood neutrophils and a lymphocyte blast transformation (LBT) test were assessed. Although FK-23 had no effect on the CBC or leukocyte differential count, treatment of the drug caused a 1.4-fold increase in neutrophil phagocytosis compared to FK-23 non-treated healthy dogs. LBT activities of phytohemagglutinin (PHA) and pokeweed mitogen (PWM) were also activated to twice the control levels and that of concanavalin A (Con A) was increased to one and a half times by the FK-23 treatment. Thus, a single administration of FK-23 appears to augment host resistance through stimulation of the non-specific immune responses in vivo.
