Effect of RU 486 on luteal function in the early pregnant rat.

A dose of 30 mg RU 486/kg, an antiprogesterone, was administered to pregnant rats on Day 2 (Group 1) or Day 4 (Group 2) of pregnancy. RU 486 significantly changed serum progesterone and oestradiol concentrations and luteal 3 beta-HSD and 20 alpha-HSD activities in Group 1, and implantation was significantly inhibited. The luteal 3 beta-HSD activity in Group 2 rats on Day 6 was significantly (P less than 0.01) lower than the control value (7.5 +/- 0.6 and 10.1 +/- 0.6 mU/mg protein respectively). This decline in the 3 beta-HSD activity was followed by a marked decrease in the serum progesterone concentration, resulting in a significant decrease of the progesterone/oestradiol ratio and implantation was completely inhibited. The 20 alpha-HSD activity, which could not be detected on Day 6 in the control rats, was twice as great in Group 2 than in Group 1 rats (17.5 +/- 1.2 and 7.4 +/- 3.1 mU/mg protein respectively). Ultrastructural examination of corpora lutea of Group 2 rats confirmed luteolysis. These results suggest that RU 486 has a luteolytic effect and its anti-implantation effect is concomitant with luteolysis of the corpora lutea of pregnancy.

Adult T-cell leukemia/lymphoma in pregnancy.

Adult T-cell leukemia/lymphoma, a lymphocytic leukemia caused by human T-cell lymphoma virus-I (HTLV-I), is prevalent in southwestern Japan. A Japanese woman with adult T-cell leukemia/lymphoma in the third trimester of pregnancy was delivered of an infant by cesarean section. She did not breast-feed the child, who has remained free from HTLV-I infection.

Characterization of transplant generations of primary rat adenocarcinoma and some chemotherapeutic observations.

Rats (Wistar) bearing subcutaneous implants of tumor fragment of DMBA-induced tumor were used in this study. Studies have been made of their proliferative characteristics, and their response to Cis-dichlorodiammine platinum (CDDP) has been measured. The transplanted tumor appears to be on homogeneous solid structure throughout all transplant generations, although the primary site is concomitant adenocarcinoma showing both glandular and solid structure. Microangiography showed that the transplanted tumor was more hypervascular than the primary tumor. Compared with the previous reports using the primary autochthonous tumor, the antitumor effect of CDDP is excellent and the histologic effect is observed in all areas of the sectioned surface. The transplanted tumor could provide a useful experimental system for the laboratory study of the chemotherapy of ovarian adenocarcinoma and the possibility of predicting ovarian adenocarcinoma seemed to be great because of the morphological similarity.

Small cell carcinoma of the endometrium associated with adenosquamous carcinoma: a light and electron microscopic study.

This report describes a 64-year-old woman with a primary small cell carcinoma of the endometrium associated with adenosquamous carcinoma. The light microscopic features resembled those of small cell carcinoma of the lung and those of the uterine cervix, and foci of adenosquamous carcinoma lay scattered sparsely in the small cell carcinoma. Electron microscopy revealed cytoplasmic neurosecretory type granules. The neoplasm behaved in a very aggressive manner such that at 3 months after surgery a metastatic neoplasm appeared in the vagina. This case is a rare example of an endometrial carcinoma with differentiation toward endocrine as well as adeno and squamous cell carcinoma.

Histologic origin of rat ovarian cancer induced by direct application of 7,12-dimethylbenz(a)anthracene.

A direct application of 7,12-dimethylbenz(a)anthracene (DMBA) to the ovary successfully produces an ovarian epithelial cancer in rat. To identify the histogenic process, the DMBA-treated rats were serially examined from the 15th to the 40th week following DMBA application. Furthermore, from the anticipated effect of estrogen on surface epithelial proliferation, the rats were put into an hyperestrogenic condition for immunohistochemical characterization of the tumor tissue. At the 20th week, the surface epithelial proliferations were seen to show remarkable multistratification with cellular pleomorphism. The proliferated surface cells began to infiltrate into the stroma to then form irregular gland structures. In immunoperoxidase stainings, both the adenocarcinoma cell and the surface cell has the same character in the immunohistochemical reaction for estradiol. On the base of these results, the induced cancer in rat was believed to have a similar histogenic process to the common epithelial tumors in human ovary.

Partial trisomy for the distal part of the long arm of chromosome 15--a new syndrome?

A malformed male neonate with partial trisomy 15q dist (q22----qter), because of malsegregation of a balanced translocation (7; 15) (p22; q22) in his mother, is described. Comparison of this patient with thirteen previously published cases of this trisomy reveals a pattern of common features including: peculiar craniofacial dysmorphism--facial asymmetry, antimongoloid slant, narrow or short palpebral fissures, prominent nose, long upper lip, micro or retrognathia, high arched palate, low set ears, malformed ears, protuberant occiput--, abnormal fingers and toes, short neck, mental and growth retardation, cardiopathy, respiratory distress etc.. The main clinical findings seem to be similar enough to justify the establishment of a new entity of partial trisomy 15q dist syndrome.

A case of eclampsia during the 18th week of gestation.

We report a rare case of eclampsia which occurred during the 18th gestational week in the absence of hydatidiform disease or Rh isoimmunization. In spite of extensive examinations during pregnancy and postpartum, we were unable to find any underlying disease, and the patient returned to being normotensive and non-proteinuric postpartum.

[Clinical studies on aztreonam in infections in obstetrics and gynecology].

Clinical effect was studied for aztreonam (AZT) on 2 patients being treated for malignant tumors. One-two grams of AZT was administered b.i.d. by intravenous drip infusion for 6-7 days. In case 1 with pyometra, there were excellent improvements in WBC, CRP and fever pattern. Unfavorable BUN and S-Cr. values were found, but these were considered to be side effects due to cisplatin. These improved finally, and clinical effect was also excellent. Case 2, a urinary tract infection + prophylactic postoperative administration case, was treated in combination with another drug presuming mixed infection and AZT was shown to be effective clinically and overall. No side effects in cases 1 or 2 were considered to be due to AZT.

Intrasplenic grafting of 7,12-dimethylbenz (a) anthracene-treated ovarian piece.

Immediately after a direct application of a chemical carcinogen, 7,12-dimethylbenz (a) anthracene (DMBA), the ovarian piece containing DMBA was implanted into the spleen of Wistar strain rats with or without gonadectomy. Six months after these treatments, an intrasplenic neoplasm was found in four of nine gonadectomized rats and in two of ten rats without castration. The volume of the induced tumor in the former group was 1.85 cm3 in average and that in the latter was 0.25 cm3. Histologic examination indicated that all of the induced tumors were not in the sex cord-stromal category, but adenocarcinomas. No morphological differences between the tumors in the castrated group and those in the noncastrated group could be detected by light microscopy. From the difference in the volume and the incidence of the induced tumors in the two groups, it appears that the hyperstimulation of the pituitary gonadotropins in the castrated rats promoted the tumorigenic process of the induced 'epithelial' ovarian cancer caused by the DMBA application to the grafted ovarian pieces.

Digital subtraction angiography for the evaluation of persistent uterine trophoblastic disease.

In three patients with persistent uterine trophoblastic disease, follow-up after primary chemotherapy was undertaken by digital subtraction angiography. It was found that the angiographic appearance of the intramural uterine lesions was comparable with that obtained by conventional pelvic angiography. Digital subtraction angiography may be applicable as an easy, safe, and valuable diagnostic method for gestational trophoblastic disease in place of conventional pelvic angiography because it is a relatively noninvasive procedure that can be performed on outpatients.

[Evaluation of cancer chemotherapy in primary ovarian cancer in rats--observation of histological effect by light and electron microscopy].

In order to evaluate the effects of ifosfamide as an anticancer agent, ifosfamide was administered to rats with ovarian cancer induced by DMBA (7,12-dimethylbenz (a) anthracene). Four to five weeks after the chemotherapeutic treatment 16 rats were sacrificed and then tumor sizes were compared before and after the treatment, and in addition morphologic changes were investigated by light and transmission electron microscopy. Of 16 rats, the reductions in tumor size were recognized in six cases. A histologic effect was observed in both the tumor decreased group and the increased group. One of the latter group showed excellent histological chemotherapeutic effect. On light microscopic observation, the histologic effects seen were reduction in the amount of neoplastic tissues and proliferation of connective tissues. On transmission electron microscopic observation, destruction of tumor cells and a reduction in the amount of ribosome and chromatin were recognized as the effects of the anticancer drug. Multinuclear giant cells, presumably derived from the mesenchym, were found among the connective tissues in one case showing a remarkable histologic effect.

[Immunochemotherapy of recurrent and advanced uterine and ovarian cancer using cisplatin].

A combination immunochemotherapy regimen containing cisplatin (20 mg, days 1-5), peplomycin (20 mg, days 2, 9, 16), +/- vinblastine (5 mg, days 1, 2), and picibanil (3-5 KE/week) was performed in twelve patients with advanced or recurrent uterine and ovarian cancers under intravenous hyperalimentation (IVH), except one patient receiving peplomycin by a continuous infusion method using IVH bag (10 mg/day for 5 days). This regimen was repeated every three weeks. Five (71.4%) of seven evaluable patients showed partial response. No patients yielded the complete disappearance of disease. No severe and lethal pulmonary or renal dysfunction occurred and all was well tolerated. In a regimen without vinblastine, myelosuppression, especially thrombocytopenia, occurred later compared to the regimen including vinblastine.

[A hormonal impact on DMBA-induced rat ovarian cancer--detection of estrogen receptors, and resultant morphologic changes after estrogen application].

Hormonal dependencies of 7,12 dimethylbenz (a) anthracene (DMBA)-induced rat ovarian cancer were studied. To reveal intratumoral estrogen receptors, dextran coated charcoal (D.C.C.) methods were utilized. In order to change the hormonal level in the rat during the period of tumor induction, serial weekly administrations of estrogen were performed from 5 to 20 weeks after the application of DMBA. Resultant morphologic changes in the induced tumor after the hormonal impacts were observed by light and electron microscope and the results obtained were as follows; 1) Estrogen receptor existed in 80% of the rat ovarian cancers induced by DMBA 2) In the estrogen injected group, a higher incidence and larger growth of the induced tumor were observed than in the hormonally non-treated group 3) A major difference between the histologic features of estrogen group and non-treated group was that the ovarian adenocarcinoma of the former, but not the latter, was associated with a significant proliferation of connected tissues and contained many signet ring cells involving intracytoplasmic canaliculi 4) Ultrastructural findings of these cells were characterized by well developed microvilli, mitochondria, golgi complexes and abundant lipid droplets 5) The proliferation of the DMBA-induced cancer cells in culture was promoted by 17 beta-estradiol application, suggesting its hormonal dependence.
