Homeopathic treatment of migraine: a double blind, placebo controlled trial of 68 patients [see comment].

To evaluate the efficacy of homeopathy in preventing migraine attacks and accompanying symptoms, a randomised, double-blind, placebo-controlled clinical trial was conducted. There was a one-month registration period without treatment, followed by four months individualised homeopathic treatment or identical placebo. Patients were stratified for common or classical migraine. Seventy-three patients were randomised, 68 completed the trial. Baseline values were similar in the two groups. Both the homeopathy and placebo groups had reduction in attack frequency, pain intensity and drug consumption, with a statistically non-significant difference favouring homeopathy. Migraine diaries showed no difference between groups. The neurologists' trial evaluation showed a statistically significant reduction in attack frequency in the homeopathy group (P= 0.04) and non-statistically significant trends in favour of homeopathy for pain intensity and overall evaluation. Further research, with improved trial design, on the possible role of homeopathy in migraine prophylaxis is justified.

[Dostoevsky's epilepsy--a new view on a great writer's illness].

Dostoevsky's epilepsy was discussed by his first biographers and is still a matter of debate. The latest development in this discussion is a series of articles published by Russian authors which conclude that Dostoevsky did not suffer from epilepsy at all. Build our hypothesis on new research probably not known by the Russians, we refute their conclusion. A description of the history and etiology of Dostoevky's illness is given and a new hypothesis presented: that Dostoevsky contracted secondary syphilis with infection of the surface of the meninges or the cerebrum. This infection is what caused his lifelong epilepsy.

[Johan Sebastian Welhaven's parkinsonism. An insight into the history of literature].

The article is meant to give new insight into the history of literature: During the last ten years of his life, Welhaven produced neither poetry nor prose and lived withdrawn in his own house, mainly because he had Parkinson's disease. He understood himself, as his doctors did--and his biographers--that he suffered from a brain disease. However, nobody was able to make a Parkinson diagnosis in Norway in the 1860s. This was perhaps possible for a handful of French neurologists, who at this time "rediscovered" James Parkinson's "An Essay on the shaking palsy" from 1817--and named the disease Maladie de Parkinson, or Parkinson's disease. When a neurologist today reads Welhaven's own observations or those of his contemporaries, and his biographies, he has little doubt about the diagnosis.

[Diagnosis of migraine in general practice].

195 patients with headache were examined by general practitioners and neurologists, and their diagnoses were compared. The general practitioners diagnosed migraine in 116 cases, the neurologists in 144 cases, and in 104 cases their diagnoses agreed. Thus, migraine can be difficult to diagnose, and this study indicates underdiagnosis of this common condition by general practitioners. This may result in overuse of analgesics and alternative therapies, and too little prescription of specific migraine medication. Greater familiarity with the diagnostic criteria of the International Headache Society should make it easier to arrive at a correct migraine diagnosis.
