[Tricuspid regurgitation induced by transvenous right ventricular pacing: echocardiographic and pathological observations].

To assess tricuspid regurgitation (TR) in patients with permanent transvenous right ventricular (RV) pacing, we performed phonocardiographic, contrast and pulsed Doppler echocardiographic studies in 18 patients with transvenous leads for RV pacing. In addition, a pathological study was performed on 26 autopsy cases with transvenous leads for RV pacing. None of the patients had right-sided heart failure. The previous phonocardiograms revealed regurgitant murmurs of TR in one clinical case and five autopsy cases. In the clinical study, definite TR was diagnosed both by contrast and pulsed Doppler echocardiography in five cases (28%). Probable TR was diagnosed only by one technique in three cases (17%), and the absence of TR was confirmed by both techniques in 10 cases (55%) (non-TR group). The average right atrial dimension was 59 +/- 5.3 mm in the definite TR group and 39 +/- 2.4 mm in the non-TR group (p less than 0.01). The average inferior vena cava dimension was 19 +/- 1.7 mm in the definite TR group and 15 +/- 0.8 mm in the non-TR group (p less than 0.05). Right atrial and inferior vena cava dimensions showed a significantly positive correlation (r = 0.58, p less than 0.05). In the pathological study, the presence of TR, which was explained by the position of the pacemaker lead in relation to the valve structure, was confirmed in 11 cases (42%). Valve motion interference was classified as type I (two cases), in which the lead was suppressed and the leaflet immobilized, type II (4 cases), in which chordae tendineae were involved by a pacemaker lead, and type III (five cases), in which both mechanisms contributed to valvular regurgitation. In conclusion, TR may follow transvenous RV pacing in approximately half of the cases with RV pacing. Contrast and pulsed Doppler echocardiography are sensitive noninvasive techniques for detecting this valvular abnormality and they should be used in the follow-up of such pacemaker recipients.

A consideration on histopathologic variability of diffuse pleural mesothelioma based on DNA cytophotometry.

Nuclear DNA cytophotometry of the tumor cells was performed for an autopsy case of malignant diffuse pleural mesothelioma diagnosed histologically as a mixed type. The DNA distribution of these tumor cells confirmed bimodality, one being situated in the triploid and the other in the pentaploid DNA range, thus indicating two stemline cell populations. In the former, the majority were epithelial mesothelioma cells resembling the normal mesothelial cells. The latter were mesenchymal tumor cells with spindle and occasionally bizarre shaped nuclei. This cytophotometric data strongly suggests that the histologic variability in mesotheliomas results probably from the change in number and ploidy level of the stemline cell population accompanied by neoplastic development.

[Assessment to fixed and variable ROI (region-of-interest) settings in the determination of left ventricular ejection fraction by multiple gated cardiac pool imaging: the benefit of variable ROI setting].

The accuracy of left ventricular ejection fractions (EF) determined by multiple gated cardiac pool imaging (RI-EF) and by M-mode echocardiography (Echo-EF) (Teichholz's method) was evaluated by comparing with those derived from angiocardiography (LVG-EF) in 19 patients including 9 with myocardial infarction, 3 with rest angina, 2 with effort angina, 2 with valvular disease, 1 with hypertrophic cardiomyopathy, 1 with congestive cardiomyopathy and 1 with hypertensive heart. Measurement of RI-EF was carried out by the fixed ROI method using a single end-diastolic ROI, and the variable ROI method where end-diastolic and end-systolic ROI were used.

[Giant intracranial aneurysm visualized by prolonged injection angiography--case report (author's transl)].

A case of giant intracranial aneurysm which almost completely occluded the internal carotid artery and was diagnosed angiographically by prolonged injection technique was reported. A 62-year-old house-wife was admitted to the Department of Neurosurgery of The Asahi General Hospital, complaining of headache, nausea, and left ptosis. Neurological examination revealed left IInd, IIIrd, IVth, and VIth cranial nerve palsy. Films and laminagrams of the skull demonstrated the double floor of the sella trucica. Plain CT scan showed an oval high density area without surrounding low density area in the left middle cranial fossa. A left carotid angiogram revealed almost complete occlusion of the internal carotid artery at the foramen lacerum. Good cross filling through the anterior communicating artery was seen in the right carotid angiogram. Pre-operative diagnosis was pituitary tumor which extended to the middle cranial fossa. Left fronto-temporal craniotomy was performed and the tumor was suspected to be a giant intracranial aneurysm. Post-operative carotid angiogram by prolonged injection technique also showed a blood channel in the aneurysm and a part of the aneurysm. Left carotid ligation was carried out following intracranial surgery and her symptoms were remarkably improved. Apart from left VIth cranial nerve palsy she showed no abnormal neurological findings at discharge. Usefulness of the prolonged injection technique for differential diagnosis of the parasellar tumor was stressed.

Partial purification and immunological aspects of carboxylesterase from rat liver microsomes.

Carboxylesterase (CEase) was solubilized from rat liver microsomes by autolysis followed by cholate treatment and then purified by the combination of ammonium sulfate fractionation, gel filtration, chromatography on DEAE Sephadex A-50 and hydroxyapatite and preparative Disc electrophoresis. The overall purification was 25-fold with a yield of 6% of the original enzyme activity. Analytical Disc electrophoresis of the final enzyme preparation showed a single band. However, SDS polyacrylamide gel electrophoresis revealed one main band of 93% and three other minor bands. To investigate the interaction between CEases of rat, monkey, pig and rabbit liver microsomes, rabbit antibody to the above enzyme preparation was prepared and immunological analyses, i.e., Ouchterlony's test and immunoelectrophoresis, were performed. In the comparative double diffusion test, the partial fusion of precipitation line between anti-rat CEase and the enzymes of other species was observed. In the second analysis, sharp arc precipitation lines also could be seen in all specimens and, furthermore, mobilities of each enzyme were different. These observations suggest that rat liver CEase seems to be immunologically related in part but not completely identical with the CEases of other species and the charge difference may exist in these specimens.

Fibromuscular dysplasia of the cervico-cephalic arteries.

Six cases of fibromuscular dysplasia of the cervical and cephalic portions of the internal carotid arteries, including their intracranial branches are reported. It should perhaps be pointed out that one of the cases was from the Sudan. As far as we know, the condition has never before been reported in a male African. The condition was associated with an intracranial aneurysm in four of our cases. To our knowledge only three autopsied cases of fibromuscular dysplasia involving intracranial arteries are on record. In our six cases the diagnosis was based on angiographic evidence, and three of the cases, two with intracranial involvement, were verified post mortem.
