Malignant neuroepithelioma (peripheral neuroblastoma). A clinicopathologic study of 15 cases.

A clinicopathologic study of 15 cases of malignant neuroepithelioma (peripheral neuroblastoma) of soft tissues is reported. The patients were chiefly young Japanese adults with a median age of 21 years. The tumors arose mainly in the soft tissues of the lower extremity (seven cases) and the trunk (four cases). Microscopically, there were sheets of closely packed, small round or oval cells, and Homer Wright-type rosettes were seen in all cases, one of which also had Flexner-type rosettes. Immunohistochemical cytoplasmic localization of neuron-specific enolase (NSE) was demonstrated in six of the eight cases, using the peroxidase-antiperoxidase (PAP) method. In no case, however, was there any staining reaction for S-100 protein. Of the 14 patients for whom follow-up information could be obtained, nine died within a period of 2 years and two were alive and well for over 5 years after the initial treatment. Differential diagnosis from other soft-tissue round-cell sarcomas, such as embryonal or alveolar rhabdomyosarcoma, extraskeletal Ewing's sarcoma, and others, are briefly discussed, on a clinicopathologic basis.

Histopathologic maturation of juvenile xanthogranuloma in a short period.

We present a case of solitary juvenile xanthogranuloma (JXG) on the scalp of an 8-month-old girl. The initial biopsy specimen showed a dense collection of small histiocytes as evidenced by CD68 staining without either lipidization or giant cell formation, admixed with a small number of lymphocytes. On the other hand, sections from the excised specimen obtained 2 weeks after the initial biopsy from the same site showed a mixed proliferation of abundant foam cells together with Touton giant cells, some small histiocytes, and small numbers of lymphocytes and eosinophils. Mitotic figures were fewer in the excised nodule than in the initial biopsy specimen. Fascicles of spindle-shaped cells arranged in a vague storiform pattern were additionally found in the deep portion of the nodule. Our case findings suggest that xanthomatization of the JXG could have been accelerated by the inflammation associated with the biopsy, based on the histopathologic fact that the change from an early phase to a mature form occurred within the very short period of 2 weeks.

Vitamin D3 analogues improve café au lait spots in patients with von Recklinghausen's disease: experimental and clinical studies.

Topical application of vitamin D3 analogues for 6 months was found to be effective in improving the pigmentation of café au lait spots in patients with von Recklinghausen's disease. Treatment of café au lait spots grafted onto nude mice with a vitamin D3 analogue caused suppression of bromodeoxy- uridine uptake in the cells of the basal layer. Vitamin D3 analogues also decreased melanin pigment in café au lait spots after 4 weeks of treatment. Thus, it is considered that long-term application of vitamin D3 analogues is effective both for improving the pigmentation of café au lait spots and suppressing the development of neurofibromas in patients with von Recklinghausen's disease.

Erythema induratum in a patient with active tuberculosis of the axillary lymph node: IFN-gamma release of specific T cells.

A 57-year-old woman with tender nodular lesions on her legs, arms, buttocks and face is reported as a case of erythema induratum (EI) with active tuberculosis of axillary lymph nodes. Both skin nodular lesions and lymph nodes responded positively to antituberculous therapy. The patient's peripheral blood mononuclear cells showed a high proliferation and produced interferon-gamma (IFN-gamma) in response to purified protein derivative (PPD). These findings indicate the possibility that PPD-specific T cells, capable of producing IFN-gamma, are likely to be involved in the formation of EI as a type of delayed-type hypersensitivity response to mycobacterial antigens at the site of skin lesions.
