[Enhancing the therapeutic potential of the physician-patient-relationship].

The dependence of the patient-doctor-relationship on social change is briefly pointed out. The special challenge for the therapeutic relationship patients with somatoform disorders give rise to, is thoroughly outlined. The fact that patients with somatization are prone to use unconventional therapies is the reason for an analysis of the placebo effect. This analytical consideration allows to demonstrate that the success of the unconventional medicine relies on characteristic healing factors usually hidden in the blackbox placebo. Acqainted with such healing factors the science oriented physician may recognize new possibilities to improve his therapeutic impact with a kind of supplementary placebo effect. Eventually a practical procedure based on modern psychology is proposed to deal with patients with somatoform disorders.

[Emergency: the internist as psychiatrist].

Aspects of psychiatric emergencies of special interest to the general physician are discussed. The perception that a problem constitutes an emergency is often subjective and sometimes reflects the anxieties of others rather than a real need for urgent treatment. Keeping calm and unhurried in front of an agitated patient might be as important as the use of a neuroleptic. The rather unusual psychopathological phenomena catalepsy and psychogenic twilight state (psychogenic trance) are mentioned in view of their importance for urgent assessment of mental status. The scope of the problem of suicide is outlined as far as the general physician is involved. Special attention is paid to panic disorder and attacks, a relatively new concept in medicine. Since the symptoms mimic those of cardiac, endocrine and convulsive disorders, patients with such symptoms often come to the attention of general physicians and frequently use the services of emergency departments. Early identification and treatment of panic disorder might help to avoid unnecessary repeated emergency department visits, and at the same time reduce the associated high risk of suicide and other psychiatric disorders such as depression as well as drug and alcohol abuse.

[Vulvovaginal mycoses-stort-term treatment with Econazole (author's transl)].

The short-term treatment (3 days) of mycologically demonstrated vulvovaginal candidiasis with econazole vaginal suppositories 150 mg was tested in a population of 194 women. Substancial prerequisites for thorough and complete performance of the therapy are high cure rates (93,3%), prompt therapeutic results, minor burden for the patient and good tolerability. In view of the vital danger for the newborn provoked by candidiasis morbidity, this short-term therapy is also indicated for prenatal prophylaxis.
