[Peripheral aneurysms-frequency, etiology, and localisation (author's transl)].

Peripheral aneurysms (p.a.) took only 4% of the 8.461 reconstructive operations in vascular surgery (1966-March 15, 1981). 44% of the p.a. were secondary to arteriosclerosis, while 41% were anastomotic aneurysms. 2/3 of all arteriosclerotic peripheral aneurysms were localized in the iliac or femoral artery; the popliteal artery was involved in 25%. 25% of the patients had 2 and more aneurysms. The popliteal aneurysm was found to be bilateral in 32%. Rupture occurred only in 3.5% whereas 27% of all peripheral aneurysms were associated with distal occlusive disease. In the asymptomatic stage amputation rate was 4%, the operative letality 2%, after complications both increased up to 16%.

[Enhanced enzymatic activity of Crohn's disease and ulcerative colitis (author's transl)].

1. An enhanced collagenolytic activity in the bowel tissue samples of M. Crohn and ulcerative colitis is demonstrated. 2. No inhibition of the collagenolytic activity was found when the specific collagenase inhibitor EDTA was applied. 2. In vitro, the application of EGLIN B resulted in a significant decrease of collagenolysis. EGLIN is a recently detected inhibitor of serine proteases such as Elastase.

Similarity between a kininogenase (kallikrein) from human large intestine and human urinary kallikrein.

A human colon kininogenase (kallikrein) was isolated by gel filtration on Sephacryl S-200 and affinity chromatography on Trasylolbound Sepharose, yielding a material with a specific activity of 1.3 U/mg (substrate: AcPheArgOEt). The molecular weight of the enzyme as estimated by gel filtration is approximately 70 000. After reduction with mercaptoethanol two bands were obtained in dodecyl sulfate eletrophoresis with molecular weights of 27 000 and 70 000. The bimolecular velocity constant for the inhibition by diisopropyl fluorophosphate was determined as 4 l x mol-1 x min-1. The preparation was characterized by immunological and enzymatic methods. Using the radioimmumoassay for human urinary kallikrein cross-reactivity and parallel binding curves were obtained. Kinin liberation from human high Mr-kininogen was totally inhibited by antibodies directed against human urinary kallikrein. Trasylol and diisopropyl fluorophosphate, but not by antibodies directed against human trypsin and plasma kallikrein. The effect on dog blood pressure was comparable to that obtained with human urinary kallikrein. The amino acid composition of human large intestine kallikrein is very similar to that of human urinary kallikrein.

Demonstration of a collagenolytic enzyme in postmortal human colon tissue.

A collagenolytic enzyme was extracted from the 6000 g sediment of homogenized postmortal human colon tissue. The enzyme digests native collagen at neutral pH. The enzyme has been concentrated by ammonium sulfate precipitation. The collagenolytic activity was measured using [1-14C]acetylated calf skin collagen, solubilized in 0.1 g/1 acetic acid. There was a significant inactivation of the enzyme solution after the addition of mumol EDTA to the test volume.

[Bursting pressure of colon in the rats and proteinase inhibition (author's transl)].

In the early postoperative period (third postoperative day) the colonic enterotomies show 45 per cent higher bursting pressure following administration of Aprotinin than the control group (p less than 0,02). On the fifth postoperative day no difference was noted between both groups. The interpretation of the results is difficult because specific parameters such as collagen content and collagenase activity were not determinated. The relationship between colonic anastomotic breakdown and collagenase and therefore the question of collagenase inhibition have to be discussed. It is suggested that activation of procollagenase is prevented because trypsin and kallikrein are inhibited by Aprotinin.

Abdominal aortic aneurysms. Risk factors and complications and their influence on indication for operation.

From January 1982 to June 1986 475 patients underwent operation for abdominal aortic aneurysm (AAA) with reconstruction by tube graft or bifurcation graft. Patients were subdivided into 2 groups, those operated upon either electively or those operated upon urgently. The overall hospital mortality following elective intervention was 4.9%, following emergency intervention 36.5%. In patients operated upon electively preoperative risk factors such as history of myocardial infarction or coronary artery disease did not influence mortality. In patients operated upon urgently, however, the postoperative mortality was significantly higher (p less than 0.005) in those with a history of myocardial infarction or coronary artery disease. Postoperative morbidity in the emergency group (2.7 complications per patient) was significantly higher than in the elective group (0.94 complications per patient). These results show that early elective operation on asymptomatic aneurysms and younger patients with few risk factors can prevent rupture and reduce postoperative mortality to an acceptable level.

[Abdominal aortic aneurysms: diagnostic and surgical problems in advanced age].

Own experience in operative treatment of abdominal aortic aneurysms (AAA) in the over-70 age group during the last 22 months are presented: there was no mortality in the asymptomatic patients (n = 51). If the AAA was symptomatic (n = 20) mortality was 25%, and if rupture occurred (n = 26) mortality raised to 42%. The wide-spread use of ultrasonography detected many more people with asymptomatic AAA than in former years. The high incidence of coronary artery disease in the over-70 age group raises the operation risk. By early detection of asymptomatic AAA, intensive preoperative treatment, and a meticulous monitoring during the operation and the intensive care total mortality was lowered from 44% to 16.5% in last years.
