[Correlation between laser tyndallometry and protein concentration in the anterior eye chamber].

The laser flare cell meter (LFCM) is introduced as an instrument that quantifies noninvasive anterior chamber protein. The relationship between laser flare measurement and actual protein concentration in vivo and in vitro was assessed. The intensity of light scattering by helium neon laser beam is measured by LFCM and depends on protein concentration and molecular size. Total protein, albumin and transferrin were measured using nephelometry and the Coomassie method. We tested 63 patients undergoing routine cataract surgery. Laser flare measurements were made before surgery. The mean in vivo flare was 8.25 photons/ms, standard deviation 9.57 photons/ms. Before surgery paracentesis was performed in 61 patients. In vitro aqueous flare was 10.55 +/- 7.68) photons/ms. Biochemical analysis showed a mean anterior chamber protein concentration in 51 patients of 33.65 (+/- 27.36) mg/dl, a mean albumin concentration in 38 patients of 15.78 (+/- 11.03) mg/dl, and a mean transferrin concentration in 33 patients of 2.01 (+/- 0.88) mg/dl. Up to a "cell" count of 40/0.075 mm3 there is a statistically significant correlation. Laser flare values compared with biochemical analysis showed for total protein a correlation coefficient of r = 0.695 in vivo and r = 0.753 in vitro. A "cell" count higher than 40/0.075 mm3 produces marked overestimation of protein concentration by laser flare values. There is a statistical significant linear correlation between photon count by LFCM and total aqueous protein concentration by biochemical analysis in vivo and in vitro in normal cataractous eyes.

[Friedrich Ludwig Urban--from blacksmith to veterinarian. The history of his life].

Friedrich Ludwig Urban lived from 1808 to 1879. There is documentary evidence that he worked as a veterinary surgeon in Berlin and fought here on the barricades in 1848. Little is known about either his professional career or his private life. The author uses hitherto unpublished facts to present a detailed picture of the professional work and private life of Friedrich L. Urban, making a contribution to the historiography of veterinary surgery.

Tangle-bearing neurons show more extensive dendritic trees than tangle-free neurons in area CA1 of the hippocampus in Alzheimer's disease.

To elucidate the pathogenetic significance of neurofibrillary tangles in Alzheimer's disease, the dendritic tree of tangle-bearing and unaffected pyramidal cells of area CA1 of the hippocampus was morphometrically examined. Golgi-stained neurons were assessed which were deimpregnated and counterstained with Congo red to visualize neurofibrillary tangles. The study revealed that tangle-bearing neurons have more extensive apical dendritic trees than tangle-free neurons. It is concluded that metabolic processes associated with the formation of neurofibrillary tangles may increase neurotrophic activity on a single cell level and counteract the cellular degeneration process.

Acute and chronic neuroborreliosis with and without CNS involvement: a clinical, MRI, and HLA study of 27 cases.

Of the 96 serologically confirmed neuroborreliosis cases seen in our clinic between 1983 and 1988, 11 patients had mild to moderate and 4 patients had serious cerebral and/or spinal cord symptoms. Nine of these 15 patients with CNS involvement exhibited a primary chronic course of the illness. After high-dose intravenous therapy with penicillin, doxycycline or cefotaxime, given mostly in combination with cortisone, gradual recovery occurred with normalization of CSF findings characteristic of neuroborreliosis, and normalization of significantly elevated Borrelia burgdorferi IgG antibody titres in CSF and serum. Brain MRI and CT showed evidence of or were suggestive of vascular involvement which correlated with clinical symptoms in 11 of the 15 patients with CNS involvement. Brain MRI changes that were similar but much slighter in number and intensity were seen in 5 of 12 neuroborreliosis patients without clinical signs of CNS involvement (lymphocytic meningoradiculitis; Bannwarth's syndrome). The frequencies of the HLA-DR7 (75%), HLA-B44 (50%) and HLA-A29 (33%) antigens in 12 neuroborreliosis patients with clinical symptoms of CNS involvement were significantly different from the frequencies in 12 neuroborreliosis patients without CNS involvement and in 100 control subjects. Diagnostic criteria of active neuroborreliosis are proposed.

[The distaff philosophy in folk medicine animal science].

Spinning, distaffs and spinning-rooms are used to be closely linked with superstitious thinking and behavior. The so-called distaff philosophy reflects the wisdom of the old black magic spinners. Their wisdom was known by the popular veterinary medicine. With the help of the collections of distaff philosophical thoughts the relation between distaff philosophy and popular veterinary medicine is shown.

[Belief in magic and magical medicine. Characteristics of its origin and consolidation in folk medicine for animal medical science].

In the evolution of mankind and therefore in the veterinary historiography as well you can find the belief in magic and in magic medicine. In the primitive society our early forefathers began with the magic medicine by domestication and the applying of magic medicine also to animals. Because of witch-hunt and printing the belief in magic was systematized and authorized in the veterinary medicine. With the freedom of trade in the second half of the 19th century the belief in magic and magic medicine flourished also in Germany.

Protein kinase C inhibition by calmodulin and its fragments.

Inhibition of protein kinase C (PKC) by calmodulin is investigated and we describe the localization of inhibitory sequences within the calmodulin molecule. We present evidence that calmodulin inhibits PKC through an inhibition of the activation of PKC associated with lipid membranes: Binding of PKC to lipid vesicles is not affected, but activation is abolished. The potent calmodulin antagonist R24571 (calmidazol) did not affect the inhibition of PKC by calmodulin at concentrations up to 10(-5) M. Two tryptic fragments of calmodulin were isolated which inhibited PKC. They were only slightly less potent than intact calmodulin with an IC50 of 6 microM compared to 1 microM of intact calmodulin. They were identified as Ser38-Arg74 and His107-Lys148. Each of the inhibiting fragments contains an intact Ca2(+)-binding domain with complete helix-loop-helix structure ("EF hand"). Other calmodulin peptides showed only weak inhibitory activity. Both fragments did not stimulate cAMP phosphodiesterase even at concentrations 100-fold higher than the calmodulin concentration needed for maximal stimulation. None of the fragments acted as a calmodulin antagonist.

Organic, including symptomatic, mental disorders (section F0)--results of the ICD-10 field trial.

The natural history of classification systems for the organic mental disorders from ICD-8 through ICD-10 and DSM-III is presented. Out of a total of 31 cases the 6 with a main diagnosis of the F0-section of ICD-10 are described and field trial results analysed in terms of classification errors, goodness of fit, difficulty, and interrater-reliability. Specific categories (instead of "collective categories") were used more frequently and fewer classification errors made on the basis of ICD-10 as compared with ICD-9. The authors suggest that an operationalized differentiation of dementia as one of the core categories would further increase the reliability and clinical practicability of this section.

Follow-up of antibiotically treated and untreated neuroborreliosis.

Follow-up of 57 patients who suffered from antibiotically untreated acute, monophasic neuroborreliosis 5 to 27 years ago shows no significant difference in comparison with the follow-up of 66 patients who suffered from antibiotically treated acute, monophasic neuroborreliosis during the last 5 years. In both groups, the involution of clinical symptoms and the normalization of pathological CSF findings were nearly identical. We found no significant difference of sequelae between the groups. Following acute neuroborreliosis, neither the antibiotically untreated nor the antibiotically treated patients developed chronic neuroborreliosis. Only in rare cases of primary chronic neuroborreliosis with CNS involvement did we observe convincing effects of antibiotics, which were given mostly in combination with glucocorticosteroids.

Panencephalopathic type of Creutzfeldt-Jakob disease with primary extensive involvement of white matter.

The case of a 66-year-old woman with atypical Creutzfeldt-Jacob disease (CJD) presents several peculiarities. Pathologic examination confirmed a rare CJD case with primary generalized spongiform changes of the white matter and only moderate, but typical changes of the gray matter. Besides an essentially typical clinical course, the patient developed temporary diplopia and vertical eye movement paralysis. Intoxication had been excluded. Isoelectric focusing revealed oligoclonal CSF-IgG. Magnetic resonance imaging studies showed periventricular accentuated flat and striped hyperintense structures. EEG had CJD-typical periodic 1/s synchronous discharges. This case illustrates for the first time that in panencephalopathic type of CJD the cerebral white matter can be involved primarily and more extensively than the gray matter.

Stability of cell size and nucleolar size in tangle-bearing neurons of the hippocampus in Alzheimer's disease.

Samples of 14 brains of autopsied patients suffering from dementia of the Alzheimer type were examined. In Congo red-hemalum stained 14-microns-thick sections of the hippocampus 15 tangle-bearing and 25-30 unaffected nerve cells of area CA1 per case were measured for cell size and nucleolar size utilizing an interactive image processing unit. In affected cells tangle size was measured too. There was no significant difference in neuron size and nucleolar size between affected and unaffected cells. In tangle-bearing cells nucleolar size and cell size were not correlated with tangle size. Since nucleolar volume can serve as an indicator of RNA synthesis of a cell, tangles themselves seem not to disturb nerve cell metabolism.

Meningoradiculitis and encephalomyelitis due to Borrelia burgdorferi: a follow-up study of 72 patients over 27 years.

In 1987, follow-up studies were conducted on 72 patients who had had meningoradiculitis and encephalomyelitis (8 patients) due to Borrelia burgdorferi 5-27 years previously. These patients had not been treated with antibiotics, either during the acute disease or during the interval prior to follow-up studies. The patients had exhibited the typical symptoms of Bannwarth's syndrome during the acute phase. At the follow-up studies, 33 patients showed no, and 23 only mild, clinical residual symptoms including normal CSF findings and low-positive serum IgG borrelia antibody titres (IFT; ELISA). Three patients without sequelae exhibited persistent intrathecal secretion of oligoclonal B. burgdorferi-specific CSF IgG antibodies (Immunoblot; positive borrelia CSF IgG antibody titres). Thirteen patients exhibited mild-to-medium sequelae with persistent intrathecal formation of oligoclonal B. burgdorferi-specific CSF IgG antibodies, up to 21 years after the acute illness. This persistence can be interpreted as an "immunological scar syndrome". Our follow-up studies appear to indicate that neurological manifestations of B. burgdorferi infections are generally (with few exceptions) of a benign nature. Most patients can be classified as having been cured without antibiotic therapy. No late manifestations of chronic progressive CNS borreliosis comparable to that of neurosyphilis have been seen following acute untreated neuroborreliosis.

MRI study of Baló's concentric sclerosis before and after immunosuppressive therapy.

An intra vitam diagnosis of Baló's concentric sclerosis in a 39-year-old male is described. To our knowledge this is the first reported case of ante-mortem diagnosis based solely on magnetic resonance imaging (MRI) findings. MRI showed features unique to this process. After high-dose intravenous therapy with prednisone, the clinical symptoms improved, and MRI showed marked changes of the concentric lesions.
