Fluorimetric determination of dimethylaminohydroxypropoxytriamterene in biological fluids by direct evaluation of thin-layer chromatograms.

A sensitive and specific thin-layer chromatographic method for the determination of the new antikaliuretic compound dimethylaminohydroxypropoxytriamterene (RPH 2823) in plasma and urine of rats is described. After dilution of the samples with methanol the tlc-separation is performed on silica gel plates in chloroform-methanol and NH3-atmosphere. RPH 2823 is recovered quantitatively from plasma and urine. The limit of detection is 5 ng/ml plasma and 50 ng/ml urine. The standard deviation of the method is 5% or less.

Pharmacodynamics of acidic triamterene derivatives and their esters and amides.

The diuretic, natriuretic and antikaliuretic effects of two acidic triamterene derivatives, carboxymethoxytriamterene (C2) and carboxybutoxytriamterene (C5), as well as some of their esters and amides were studied in male Wistar rats. The acids and their esters possess similar natriuretic and potassium retaining properties. The esters are nearly completely hydrolyzed to the corresponding acids; therefore, the acids themselves represent the active principle. Compared to the acids and esters the amides have a stronger effect on natriuresis and antikaliuresis. As the amides are excreted partially unhydrolyzed in urine, their pronounced natriuretic and potassium retaining effect must be due to the intact molecule.

Pharmacodynamics and pharmacokinetics of the basic triamterene analogue dimethylaminohydroxypropoxytriamterene.

The diuretic, natriuretic and potassium retaining effects of dimethylaminohydroxypropoxytriamterene (RPH 2823), a basic triamterene derivative, were studied in male Wistar rats. Compared to triamterene (TA) RPH 2823 has a pronounced effect on urine volume and on the excretion of sodium; in addition it possesses a higher antikaliuretic potency than TA. In combination with furosemide, 2.5 mumol/kg RPH 2823 can avoid the kaliuresis of 25 mumol/kg furosemide. The pharmacokinetics of RPH 2823 after i.v. application of 1 mg/kg and 5 mg/kg were studied in female rats. The substance is not metabolized. RPH 2823 has a terminal elimination half-life of 3 h. About 47% of the given dose are excreted unchanged with urine. Furthermore, the volume of distribution VD beta and the total body clearance were calculated.

Studies with triamterene derivatives for oral application.

The diuretic effects, expressed as the urinary Na+/K+ ratios, after oral application of triamterene (TA) and three representatives of a series of TA derivatives - dimethylaminohydroxypropoxytriamterene, carboxybutoxytriamterene and its ethyl ester - are investigated in male Wistar rats. Although the TA analogues are potent diuretics when they are given intravenously, very high oral doses are necessary to produce slight diuretic effects. This result is due to an insufficient absorption of the compounds in the gastrointestinal tract of the rat.

[Balint seminars in clinics with assistant physicians in neurology continuing education--a workshop report].

The article describes the problems of Balint seminaries held internally in hospitals, basing on literature and on the authors' practical experience, with particular reference to the effectivity of Balint group work. On the one hand, it is evident that this entails a number of disadvantages compared with Balint groups not linked to the institution; however, these drawbacks are no fatal obstacle to Balint group work. On the other hand, we can safely reject the argument that it is not possible to topicalise within the framework of a Balint seminary the pathology of reference in psychiatric and particularly in schizophrenic patients.

[Schizophrenia, creativity and art--a contribution to the theoretical understanding of pictures of schizophrenics (author's transl)].

Several opinions on the paintings and drawings of patients with mental disorder are referred. In a historical summary the authors who have a more diagnostical approach are confronted with those who are more interested in creative aspects. Finally the psychodynamical analysis of creative processes is stressed. From this point of view it is attempted to describe the relations between schizophrenia, creativity and art.

[The drawings of Augustin Wilhelm. The psychopathological aspects and cultural and historical comparisons].

This is a presentation of drawings of encephalopods by a schizophrenic patient of ours, Augustin Wilhelm. They will be compared with works of other patients as well as with encephalopod figures known from the history of art and culture. This comparison should enable a better understanding of the subject known in schizophrenic painting as the 'regressive phenomenon', which until today has been insufficiently grasped.

[Reintegration of collective inhibitions. Mark Prent's "Esthetics of ugliness" from the medical and psychosocial point of view].

In the light of sculptures by the Canadian Mark Prent a comment is made on the 'Esthetics of Ugliness' from the medical and socio-psychological point of view. The focal point of interest lies in the interaction between the work and the onlooker. The onlooker's concern with the problem of the 'Esthetics of Ugliness' is considered as a possibility of understanding better and, therefore, preventing or as the case may be curing mental disorders of the individual and society as well as the impedding and even disease-causing factors in human relations.

[Effect of dopaminergic substances on the antinocifensive action of arecoline].

Based on earlier studies that revealed the analogical influence of noradrenaline and serotonin on morphine analgesia and the antinocifensive effect of arecoline, it was tested whether under the influence of dopaminergic substances the inhibition of nocifensive reactions is being altered by arecoline like the action of morphine. In fact, arecoline could be amplified in a dose dependent manner by apomorphine and amphetamine, which alone had no antinocifensive effects. On the other hand, no antagnoism of the dopamine receptor blockers, chloropromazine ahd haloperidole, against arecoline was demonstrable; only the additive effect of apomorphine and amphetamine could be abolished. Therefore it is concluded that dopamine is not directly involved in the spinal mechanism of the antinocifensive effect of central cholinomimetics, but that only supraspinal influences can be varied by the function of dopaminergic neurons.

Effectiveness of topically applied antibiotics on anaerobic bacteria in the pilo-sebaceous duct.

An unspecific cream vehicle containing 1% erythromycin base was applied once a day for 10 days to one side of the forehead of 16 healthy subjects and the same vehicle without the active agent to the other side. Using the same procedure a solution containing 1.19% clindamycin phosphate (corresponding to 1% clindamycin base) and the corresponding unspecific vehicle was administered to another group of 17 healthy males. Prior to the treatment and after the end bacteria were obtained from the pilosebaceous ducts by means of a special sampling technique. Both ointments containing the antibiotics lead to a significant reduction of the bacterial growth in the pilosebaceous ducts regardless of whether evaluation is made of the total bacterial count or of Propionibacteria. Thus, it could be demonstrated that topical application enables some antibiotics to penetrate into the pilosebaceous duct in an adequate concentration to ensure antibacterial effectiveness if a suitable vehicle is used.

[Indirect portraits. An attempt of non-verbal communication in a psychoanalytic group].

A collection of 7 own drawing and 15 related sketches are presented. They have been produced during the time of my participation in an analytic group. Fairly detailed information can be given about how the drawing process developed (first inspiration, initial situation, considerations during drawing). The formative and constituent elements of the drawing conceived consciously during its production are contrasted with those that have only later become conscious. The discussion of this own series of drawings and sketches should facilitate the comprehension of, and approach to, the drawings of the patients, which can also be considered to be 'indirect portraits' of their own personality and of their current environmental situatiion. The drawings were also an attempt at better integration in the group, with regard to group interaction. Advantages and disadvantages of this trial in nonverbal communication are discussed.

[Objective graded increase of stenosing coronary artery processes using quantitative analysis of pathologico-anatomical data and their relationships. 3. Extension and discussion of the model].

On the basis of the demonstrated mathematical valuations of pathologo-anatomical influence sizes on the defined myocardial infarction the model is extened by the calculation of total numbers for the purpose of simpler use in medical practice. The statistical influence of the characteristics on the formation of the arithmetical measure medium XI of the group with infarctions for the position of the stenosis times degree of stenosis is determined left-sided with 30.0% and right-sided with 42.3%, the heart mass is determined with 25.9% and the barriers of the ostium as well as the type of supply with 1.8%. For the judgment of the functional capacity of the developed model it is used in 126 intravitally coronarographed patients, in whom no myocardial infarction was to be proved. The false coordination is 4 cases (3.2%). Consequently, the developed model is evident not only for the initial material. It is usable post mortem and intravitally. Apart from the judgment of the severity of the stenosing coronary arteriosclerosis it serves intravitally for the estimation of the endangering by myocardial infarction. Criterion for the estimation of the degree of endangering is the distance from the point of separation TP = 558 (values of the arthmetical measure) and TP = 122 (total number).
