Immunocytochemical analysis of the tumour suppressor protein (p53) in feline neoplasia.

Detectable p53 immunostaining in neoplasia generally correlates with the presence of a mutation in the coding region of the p53 gene and may provide insights into the pathogenic mechanisms underlying tumourigenesis. p53 immunoreactivity was examined in 77 feline tumours, selected as a representative sample of 486 specimens submitted for diagnosis and analyzed to estimate the relative frequencies of feline neoplasias. Immunocytochemical staining demonstrated nuclear immunopositivity in 46% of the squamous cell carcinomas (SCCs), 50% of the osteosarcomas, 33% of the mammary carcinomas, 16% of the adenocarcinomas and 14% of the haemangiosarcomas. In contrast, none of the malignant lymphomas or fibrosarcomas examined showed p53 immunoreactivity. These data support a role for p53 aberrations in the pathogenesis of certain feline tumours.

Risk of dysphagia after transesophageal echocardiography during cardiac operations.

BACKGROUND: Dysphagia can be a significant complication following cardiac operations. This study evaluates its incidence and relationship to intraoperative transesophageal echocardiography (TEE) for specific indications versus known factors such as stroke or prolonged intubation. METHODS: Records of 838 consecutive cardiac surgical patients were reviewed, and categorized into those who received TEE for specific indications versus those who did not (nonTEE). Dysphagia was recorded when symptoms were confirmed by barium cineradiography. Multiple logistic regression identified significant factors causing dysphagia. RESULTS: TEE was significantly related to the development of postoperative dysphagia by multiple logistic regression (p < 0.001). After controlling for other significant factors (stroke, left ventricular ejection fraction, intubation time, duration of operation), the odds of dysphagia for TEE patients was 7.8 times greater than for nonTEE patients. CONCLUSIONS: TEE may be an independent risk factor for dysphagia following cardiac operations.

A multidisciplinary process to improve the efficiency of cardiac operating rooms.

OBJECTIVE: To alter the approach to cardiac operating room services in an attempt to remain competitive in a cost-driven market. DESIGN: Study of processes and strategies for tracking and decreasing the times required for the multiple components of the operating room period. SETTING: Cardiac operating rooms in a tertiary care, university-affiliated hospital. PARTICIPANTS: All patients undergoing primary coronary artery bypass grafting during December 1996 (baseline) and the following year (1997). INTERVENTIONS: After participation in cost-containment meetings, site visits, and development of a working group, data collection was begun detailing the times of the various components of the operating room period. Changes of process were made to reduce operating room times. Most of these changes involved multitasking care: multiple people performing various tasks at the same time. All measured operating room intervals were decreased. There was no difference in morbidity and mortality over this time period. CONCLUSION: Development of a working group, with support from the hospital administration, can significantly decrease the time of tasks in a cardiac surgery operating room without adversely affecting morbidity and mortality.
