Inhibition of clastogenicity of benzo[a]pyrene and of its trans-7,8-dihydrodiol in mice in vivo by fruits, vegetables, and flavonoids.

In the in vivo mouse bone marrow micronucleus assay, homogenates of spinach, artichoke, peaches, and blue grapes as well as commercial concentrates of these vegetables and fruits reduced induction of micronuclei by benzo[a]pyrene (BaP) by 43-50%. Concentrates of strawberries (31% reduction) and of cauliflower (20% reduction) were less potent. Inhibition of genotoxicity by spinach and peaches was not caused by any delay in maturation of micronucleated erythrocytes as shown by experiments with sampling times of 24, 48, and 72 h after dosing of BaP. Pre-treatment of the mice with spinach 48, 24, and 12h before application of BaP resulted in a 44% reduction of micronuclei while peaches generated only a marginal effect. A post-treatment procedure administering spinach or peaches 6h after dosing of BaP did not indicate any protective effects. When trans-7,8-dihydroxy-7,8-dihydrobenzo[a]pyrene (BaP-7,8-OH) was applied for induction of micronuclei spinach and peaches reduced the number of micronuclei by 55 and 48%, respectively. Pre-treatment of mice with spinach 96, 72, and 60 h before sacrifice caused a decline of hepatic 7-ethoxyresorufin-O-dealkylase (EROD) and of 7-pentoxyresorufin-O-dealkylase (PROD) activities by factors of 2.2 and 1.4, respectively. However, statistical significance was not reached. On the other hand, peaches had no influence on hepatic EROD or PROD activities. The flavonoids quercetin and its glucoside isoquercitrin, administered orally in doses of 0.03 mmol/kg body weight simultaneously with intraperitoneally given BaP, reduced the number of micronuclei in polychromatic erythrocytes of the bone marrow of mice by 73 and 33%. Ten-fold higher concentrations, however, reversed the effects with a particular strong increase observed with isoquercitrin (+109%; quercetin: +16%).

[Prevention of thrombosis in surgery: low molecular weight heparin/standard heparin].

In a randomized study the efficiency of the low molecular weight heparin (LMWH) Fragmin and unfractionated heparin Calciparin in preventing thromboembolic disease was examined. Ninety patients undergoing a herniotomy participated in the study. The LMWH also showed good efficiency in a laboratory test of anti-factor Xa-activity. There were no complications.

[Comments on the induction of anesthesia in arthroscopy of the knee joint].

Anaesthesia was applied to 1,036 patients for diagnostic or therapeutic knee-joint arthroscopy, between 1983 and 1985. Different indications for general and regional anaesthesia, setbacks of spinal anaesthesia, and advantages likely to result from combined use of peridural anaesthesia and peridural analgesia are discussed in some detail.

[Crohn's disease and pregnancy].

The interrelationship of pregnancy with Crohn's disease is as nebulous as aetiology and pathogenesis of this inflammatory bowel disease. In the years 1964-1981 we have operated upon 342 patients with Crohn's disease. 47 women suffering from this disease became pregnant. Whereas pregnancy appears to have little influence on the course of Crohn's disease, the incidence of miscarriages appears to rise with increasing activity and extent of bowel involvement. Only in case of exacerbation or complications is medical or even surgical therapy indicated in the course of pregnancy. Abortion does not appear to influence the course of Crohn's disease favourably.

[Morphometrical investigations on bowel interpositions after total gastrectomy (author's transl)].

Different types of interposition were performed on beagle dogs as stomach substitutes after total gastrectomy: (1) interposition of jejunal segment, (2) interposition of ileocecal region, (3) interposition of colon segment, and (4) jejunal segment according to Roux-en-Y configuration. The interpositions were evaluated morphometrically at defined sites 3 months after the operation. One of the most evident changes found in the interposed intestinal segment was strong hypertrophy of the circular muscular layer. In interposed colon segment the hypertrophy was less than in jejunal ones. The volume burden the newly formed stomach exposed to seems to be the pathogenetic factor responsible for this change. A change in villous morphology was shown with a reduction of the surface of the intestinal mucosa. Simultaneous enlargement of crypts presented a picture similar to the partial mucosal atrophy of the hyperregenerative type. In contrast, the mucosa of the colon interpositions was regular and showed no evident structural change. The alterations of the small intestinal mucosa seem to be induced mainly by differences in the intraluminal milieu. Besides the volume burden insufficient chyme digestion may be responsible for local malnutrition of the mucosa, which results in a morphological change. The hypertrophy of the muscular layer can be interpreted as adaptation to the reservoir function and the degeneration of small intestinal mucosa, as caused by trophic interference.

[Histological and enzyme histochemical studies of intestinal interposition after total gastrectomy].

The formation of a stomach substitute with various intestinal interpolations (jejunum, ileocecum, colon) and the esophagojejunostomy by a Roux-anastomosis are carried through after total gastrectomy in animal experiments. The structural rebuilding proceedings in the intestinal interpolations were histologically and enzyme-histochemically examined after an experimental time of 3 months. A dilatation of the intestinal wall in connection with muscular wall hypertrophy appears in all operating method. In that way the reservoir function is guaranteed in the various stomach substitutes. The mucosa of the jejunal interpolation shows a hyperregeneratoric atrophy. A small enlargement of the surface originates in the colon interpolation from a villiferous conversion of the proximal parts of the crypts. The advance of the activity of the brush border enzymes in the ileocecum and the colon interpolation refers to an attempt of the mucosa on a change of the conditions of resorption. The essential pathogenetic factor of all rebuilding events appears in the intralumic environmental alteration in the intestinal interpolations.

[The clinical significance of the oxytocin challenge test (author's transl)].

In order to test the function of fetoplacental unit 398 oxytocin challenge tests were performed on 280 patients who had exceeded the calculated date of delivery or where factors of risk were involved. 8% of the tests performed showed a positive result, 1% was suspect and 91% were negative. The figure of 20% of false-positive test results was relatively high compared to only 1.6% in the case of false-negative test results. In cases where the calculated date of delivery has been exceeded, pregnant women who showed a negative test result are kept under observation by us as out-patients, whilst patients whose oxytocin challenge test was positive are kept under close observation in hospital.

[Clinical feature, treatment and prognosis of carcinoid tumors (author's transl)].

14 cases of carcinoid tumors over a period of 16 years are reported. There are 7 carcinoids of the vermiforme appendix, 3 of the small bowel, 1 of the rectum, and 2 carcinoids of the bronchus. No carcinoid syndrom was observed. Three patients with carcinoid tumors, localized in the small intestine died immediately post operationem. All others are still alife and followed up between 6 month and 13 years, are free of symptoms or rezidives. Poor prognosis of the carcinoids of the small intestine seems to depend on late diagnosis, complications already occured at the moment of operation, and advanced age of these patients. Different malignance is not considered to be an important factor of prognosis.

[Septic shock following abdominal operations (author's transl)].

Septic or toxic shock is a life threatening complication after abdominal operations. The etiologic analysis of our 102 patients showed the following conditions: 1. diseases, which have already preoperatively a high incidence of septic complications, 2. sepsis developing after primary aseptic diseases, 3. septis without any etiologic connection to the primary disease or operation. An initial standarised intensive therapy must start before any irreversible organ damage may occur. First aim of all surgical measures is the eradication of the source of infection. Early relaparotomy is the only possibility for correction of intraoperativ technical defects. Only by longstanding combination of intensive personal and technical support prognosis of septic shock after abdominal operations can be improved.

[Anal and perianal complications of Crohn's disease (author's transl)].

Anal and perianal lesions in Crohn's disease such as fistulae, fissures, abscesses and proliferative ulcerous proctitis are often misinterpreted and consequently unsatisfactorily treated. They always indicate a florid intestinal attack or a relapse after previous intestinal resection. Out of 153 patients with Crohn's disease in the last 11 years we have had 59 cases (= 38.6%) with a history of anal changes or such changes were demonstrable on admission to hospital. Only after subtle diagnosis in which the nature and extent of these complications and the location of the intestinal disease focus are determined, the most promising therapy, after all possibilities of conservative treatment have been exhausted, is intestinal resection. Local surgical measures which would often endanger continence, are reserved for the individual case.

[Ulcerative colitis - indications for surgery and methods of treatment (author's transl)].

Morphology, clinical signs and symptoms as well as surgical therapy of ulcerative colitis are discussed comparing own findings with results reported in the literature. Case reports are given of 48 patients with special emphasis on indications for surgical treatment, methods of surgery and complications. Timing of surgery and medical pretreatment do play a decisive role. The treatment of choice today is proctocolectomy with terminal ileostomy in one session; more serious complications as well as transition into a carcinoma can thus be avoided. In some acute cases a fistula-operation may be indicated for short-term relief.

[Recidivism of Crohn's diseases. Indication and therapeutic problems of reintervention].

The recurrence of Crohn's disease often presents problems in the indication for surgery. Reoperation is indicated in all recurrent complicated cases and in postoperative complication not specific for Crohn's disease. However relaparotomy should be performed only after conservative measures have proved unavoiding. In the Department of Surgery at the University Hospital in Mainz 153 patients with Crohn's diseases were treated over an 11-year-period. Of 310 patients operated for Crohn's disease 153 reoperations were performed in 73 patients. The aim of reoperation should be the prevention of further complications. In view of the individually differing courses of the disease no general guidelines can be laid down for the indications for laparotomy.

[Surgical and medical treatment of Crohn's disease (author's transl)].

Regional enteritis does present therapeutical problems because of the unpredictable course of this disease. 162 patients were operated upon in the Surgical Department of the University of Mainz Medical School between 1964 and 1976. Medical and surgical treatment of these patients has been rather different due to the heterogenity of the patient group. Curative therapy of regional enteritis does not exist. Medical treatment is aimed at converting the active form of the disease into an inactive, 'burnt out' form. One therapeutic regimen widely used is the combination of salazosulfapyridin and corticosteroids, another regimen is the combination of azathioprine and prednisolone. Surgery is indicated when complications arise and after medical treatment has failed. The optimal approach is resection of the afflicted part of the intestine. Even after surgery prognosis is doubtful and depends upon the activity of the disease in each individual case.

[Biological effect of bariumsulfate as contrast material in bone cement. An animal study on rabbit femora (author's transl)].

The investigations presented in this paper concern the question of toxicity of Bariumsulfate as an ingredient of bone-cement. In 6 rabbits the medullary cavity of the right femur was filled with bone-cement which contained Bariumsulfate as X-ray contrast ingredient and the left one with BaSO4-free bone-cement. 4 1/2 months post operationem the femura were investigated by histological and microradiographic methods. The status of both sides was compared. On the BaSO4-containing side regularly necrotic lamellar bone structures in the zones of osteogenesis and single disseminated necroses are found which are not caused by the primary lesion occuring during the implantation procedure. Additionally the zone of primary lesions which is found on both sides was more extensive at the BaSO4-containing side.
