[Clinical and immunological studies on a patient with myasthenia gravis before and after repeated plasma exchange].

A 56-year-old patient with severe acquired myasthenia gravis refractory to conventional therapy underwent a series of plasma exchanges. Three single exchanges did not improve her status. A series of intensive plasma exchanges of 6 liters each on 4 consecutive days reduced her anti-acetylcholine receptor antibodies to one-tenth of the initial value. The reduction was accompanied by an impressive clinical improvement. This study confirms that antibodies can be removed by plasma exchange. It further shows that the clinical improvement appears to be closely related to the quantity of antibodies removed.

Autologous marrow reconstitutions in severe aplastic anaemia after ALG pretreatment and HL-A semi-incompatible bone marrow cell transfusion.

Three consecutive patients considered to have end-stage acquired aplastic anaemia were given 100-160 mg/kg antilymphocyte globulin (ALG) i.v. followed by an infusion of 2-3.8 x 10(8) nucleated marrow cells/kg i.v. from HL-A one haplotype-identical, MLC-positive family donors. All patients showed autologous marrow reconstitutions lasting now 2-3 1/2 years. No clear-cut evidence of marrow engraftment could be established and no graft-versus-host disease was seen. It is assumed that these patients had some normal pluripotent haemopoetic stem cells which proved to be able of endoreduplication and of going into cycle after ALG conditioning and allogeneic marrow transfusion.

Controlled prevention of post-operative thrombosis in urological diseases with depot heparin.

The frequency of venous thrombosis after urological operation was investigated by means of iodine fibrinogen in 65 patients chosen at random. 34 patients were treated with depot heparin (Calciparin). Out of 31 patients, 18 (58%) presented an increase of the fibrinogen radioactivity. Moreover, these 18 untreated patients presented clinical signs of thrombosis. On the other hand, the fibrinogen radioactivity was increased in 4 patients (12%) of the Calciparin group. Comparison of the results of the control group and the experimental group is quite significant. The advantage of the prevention of thrombosis is more obvious in longer lasting operations.
