[Value of the studies of multimodal evoked potentials for evaluation of neurotoxic effects of combined exposure to lead, copper and zinc].

Central nervous system dysfunction was studied in workers with occupational exposure to Pb+Cu+Zn by measuring somatosensory evoked potentials of short and long latency, visual evoked potentials and brain-stem evoked auditory potentials. The tests were done in 40 workers with a history of exposure from 6 to 19 (mean 14.3) years in a copper foundry. Pb poisoning was not diagnosed in them, but 18 workers had some abnormalities of heme synthesis. Pb concentration in blood was from 11.4 to 62.8 (mean 35.7) mcg% in the whole group. Pb concentration in air in the foundry exceeded 1.1-175 times the MAC value, the concentration of CU exceeded it 7.25 times. Zn concentration was half the MAC value. No significant abnormalities were found in the parameters of multimodal evoked potentials. The obtained data suggest a protective effect of copper and zinc on heme , Pb blood level and conduction velocity of somatosensory, visual and auditory impulses in workers exposed to high Pb air concentration.

[Value of multimodal evoked potentials in the assessment of the neurotoxic effects of carbon disulphide].

Central nervous system dysfunction among CS2-exposed workers was studied by measuring short latency SSEPs and VEPs. The examinations were done in 40 men. Chronic CS2 intoxication was diagnosed in 21 of them. Latency of N19, N11, N20 of somatosensory evoked potentials and latency of P100 wave of visual evoked potentials were statistically significant longer in CS2 exposed group in comparison with controls. The amplitude of N20 SSEPs was also significantly higher. Increase of N20 amplitude (33 persons) and elongation of its latency (25 persons) were frequent abnormalities of SSEps. In individual assessment of VEPs, most frequent were: abnormal difference in interocular latency of P100 (26 persons) and elongation of P100 latency (25 persons). In the analysed SSEPs and VEPs parameters there was no significant difference between two clinical groups. The authors conclude that the CS2--neurotoxic effect leads to impairment of the peripheral and central part of the somatosensory pathway, as well as to central dysfunction of the visual pathway.

[Late sequelae of recurrent occupational lead poisoning--the status of the nervous system].

In 44 men with a history of repeated occupational lead poisoning multidisciplinary clinical and laboratory investigations were carried out. The number of lead poisoning episodes was from 2 to 7. Apart from peripheral hypalgesia on the extremities (in 6 cases) no other neurological abnormalities were found. The motor nerve conduction velocity in the median, ulnar, radial, peroneal and tibial nerves was slightly slower in 27 cases and considerably slower in one case. Slowed down conduction velocity was most often observed in 2 nerves (12 cases) and in one nerve (9 cases), less frequently in three (4 cases) and four (2 cases) nerves. In one case this slowing down was found in all five studied nerves. Conduction changes were found in 21 peroneal nerves, 15 ulnar nerves, 12 median nerves, 7 tibial nerves and 2 radial nerves. The authors think that for the evaluation of the condition of the peripheral neuron in cases of past lead poisoning it would be sufficient to determine the conduction velocity in the ulnar and peroneal nerves. Plumbism neuropathy have the character of subclinical lesions not impairing motor skill.

[Value of electroencephalographic studies for the evaluation of the neurotoxic effect of metallic mercury].

The results of 373 EEG investigations were analysed. They were obtained in a period of 4 years in 222 workers with occupational exposure to metallic mercury in the synthesis of acetaldehyde and salt electrolysis. Abnormal or borderline abnormal results were found in 13% of them. Results were compared in groups differing in age, duration of occupation, working post and Hg excretion with urine. Significantly greater frequency of increased Hg absorption was found in the group with abnormal records. The authors stressed the following morphological features of EEG in those exposed to Hg: tendency for seizure activity, loss of ability of following of rhythmic flashes, unchanged frequency of background activity despite increasing degree of other abnormalities, absence of low-voltage tracings, persistence of abnormalities in the records after loss of contact with mercury.

[Visual evoked potentials and the time of motor reaction to visual stimuli in chronic metallic mercury poisoning].

In a group of 10 patients with diagnosed occupational chronic poisoning with metallic mercury a correlation was sought between the parameters of the visual evoked potentials and the results of the psychological test evaluating the rapidity of motor response to the visual stimuli. The analysis suggest a cerebral dysfunction of the visual neurons and reduced rate of visual information transformation to the motor effector at the level of the cortical association centre.

[Chronic toxic encephalopathies caused by metallic mercury].

On the basis of neurological, psychological and electroencephalographic investigations the authors present an assessment of the state of the nervous system in 24 males observed in hospital for suspected chronic intoxication with metallic mercury. The most frequent symptoms were: headaches and dizziness (17 cases), recent memory disturbances (16) and irritability (15). Cerebellar signs were found in 13 cases, one eeg record was abnormal, in 4 cases the eeg was borderline normal. Psychological tests revealed in 21 cases evidence of organic brain damage with slowing down of the simple visual and auditory reactions (16), disturbances of recent memory and persistent attention (14), decreased intellectual efficiency (7) and visual perception (5). The authors call attention to the necessity of appropriate prevention of mercury poisoning in view of its selective neurotoxicity and irreversibility of the pathological changes. The importance of psychological testing for psychological testing for the correct assessment of the status of the nervous system in cases of metallic mercury poisoning is stressed.

[Cerebral bioelectric activity disorders in acute intoxication with hypoglycemic agents].

The authors present 10 acute suicidal poisonings with hypoglycaemic agents with particular reference to the course of disturbances of brain bioelectric activity. In most patients diffuse changes prevailed during the coma in the form of irregular, usually high-voltage slow activity. Normalization of EEG tracings was very slow, often it took several months, although the duration of hypoglycaemia was usually short (1--5 days). In patients with prolonged toxic coma profound structural lesions of the central nervous system developed with appearance of decortication (apallic syndrome). The authors think that the particular severity of central nervous system lesions in these intoxication and the persistence of changes point to the necessity of seeking more efficient measures of treating neuroglycopenia.
