Diacetyl-splenopentin (BCH 069) did not affect the concentrations of various hormones in men.

We investigated the effect of BCH 069 (similar structure as thymopentin) on concentrations of hormones of the hypophysis, adrenals, gonads and thyroid glands in men after a therapy period of 4 or 6 weeks. No significant effects of BCH 069 three times weekly 50 mg s.c. for 4 weeks or i.v. for 6 weeks on serum concentrations of prolactin, LH, TSH, cortisol, testosterone, progesterone, estradiol and ETR were observed.

[Severe oligozoospermia as a sequela to partial obstruction of the seminiferous tubules].

Arguments for the presence of a partial obstruction as cause of severe oligozoospermia may result from following findings. 1. The frequency of normal FSH values in oligozoospermia (44.9%) do not differ significantly from cases with azoospermia (50%). 2. Including a quantitative analysis of testicular morphology 5 out of 63 patients (7.9%) suffering from severe oligozoospermia show a normal spermatogenesis. 3. The frequency of this constellation in our material including severe oligozoospermia, normal testicular volume, normal FSH value, and normal spermatogenesis agree with few findings in the literature. The number of etiological factors of partial obstruction require also invasive diagnostical procedures like vas deferens radiography and surgical exploration.

[Prolactin stimulation using the metoclopramide test in females taking oral contraceptives].

In 31 patients taking oral contraceptives (o.c.) for a period between 1 year to more than 3 years, basal serum prolactin levels and metoclopramide induced prolactin values were determined 30 and 60 minutes following an i.v. injection of 10 mg of metoclopramide. The basal prolactin levels were elevated in 7 women to more than 1,000 mU l. The 3 groups of patients taking o.c. with different estrogen doses showed higher drug induced increase of their prolactin levels than the controls. These differences were statistically significant between groups I and III and the control group. No differences could be found between the challenged values of the users groups. The prolactin increase challenged with 200 micrograms TRH in 5 women under o.c. was considerably smaller than that observed in the metoclopramide groups, but exceeded the TRH induced levels found in the controls. The significance of these findings is discussed with special reference to the promotion of prolactinomas.

[Testicular function in obstructive azoospermia].

The normal histological findings of the testicles with simultaneous azoospermia according to the former opinion renders greatly probable an obstruction of the seminal ducts. The reversed case of an ascertained obstruction, however, must not necessarily contain also normal histological structures or normal hormone tests. In the obstruction syndrome in 6 out of 23 cases we find changes in the diameter of the tubule, reductions of the spermatogenesis and changes at the interstice. Imprint-cytologically an increase of pathological spermatides at the cost of normal cell developments is shown. According to this the FSH-values are slightly but statistically significantly increased. LH, PRL and testosterone are found within the normal. For the diagnostic delimitation of the obstruction syndrome from a production azoospermia according to these findings the performance of a biopsy of the testicles with histological evaluation and the determination of the FSH-value. The relatively discrete tubular changes alone cannot be used for the explanation of the discrepancy between anatomical and functional results of the operative reconstruction of the seminal ducts.

[House-dust allergy in bronchial asthma].

1. House dust allergy and house dust mite allergy are with 18.4 and 14.1% the most frequent causes of the atopic asthma also under our conditions. 2. In about 60% the house dust allergy is conditioned by a sensitization by constituents of mites (excretions). 3. The responsible antibodies may be established by means of the radio-allergo-sorbent-test. In about 50% results were found corresponding with the intracutaneous test. 4. In the restricted possibilities of the allergen carency the hyposensitization plays an important role. In critical choice of the patients in 50-60% a clinical improvement is to be achieved.

[Radiocardiographic assessment of the effect of digoxin on heart conduction insufficiency].

On 29 patients with heart insufficiency radiocardiographical investigations in rest and under stress were carried out before and after treatment with digoxin. Here it was shown that in existing functional insufficiency the determination of the stroke volume and of the minute volume in rest after digitalisation did not result in a remarkable change, but in obstructive insufficiency an improvement of the pump function of the heart began already under conditions of rest. Only in stress also in patients with functional insufficiency a significant increase of the heart-time volume and the stroke volume after application of glycoside was proved. An exacter information about the effect of glycoside was given by the temporary volume function particularly under stress.

Influence of hormonal contraceptives on the pituitary response to LH/FSH-releasing hormone.

In the pre-ovulatory phase the absolute and relative LH increase was much greater than during the luteal phase and less pronounced in the early follicular phase of the normal cycle. FSH release was affected only during the pre-ovulatory period, where a retarded, 3- or 4-fold increase compared to basal levels was recorded. In the women taking oral contraceptives of the conventional type the first LH-RH test showed gonadotropin responses similar to those obtained during the luteal phase of the controls. The second test brought a significantly lower LH response, suggesting an increasing exogenous steroid inhibition at the pituitary level in the course of the therapeutic cycle. This inhibition seems to be reversed during the monthly tablet-free interval. A particularly small and retarded gonadotropin response was observed in patients taking Deposiston. These results are discussed as to their clinical significance.

["Hypothyroid" goitre in puberty treated with a combined T3/T4 preparation (author's transl)].

After administration of combined T3/T4 preparations (ratio of 4 : 1) to juveniles with goitre, size II-III (WHO), and hypothyroid T4 or PBI levels, the T3 value reached hyperthyroid levels 7-8 hours afterwards. The increased T3 level with normal T4 blood levels can lead to iatrogenic T3 hyperthyroidism. Serial tests demonstrated that at increasing doses a raised T3/T4 ratio, which is observed with low T4 values, can return to normal but with increasing doses this ratio, caused by T3 rise, can again become elevated. For this reason one should reconsider treatment with T3/T4 preparations and, if necessary, replace it by monotherapy with L-thyroxine.

[TRH refractory TSH values, triiodothyronine levels and T3/T4 quotients in hypothyroid children treated with T3/T4 preparations (author's transl)].

TRH-tests, triiodothyronine (T3), TBI, and thyroxine (T4) determinations were carried out in 23 children with primary hypothyroidism treated with T3/T4-preparations. T3/T4 quotients were calculated. 7 out of 10 patients with TRH refractory TSH values and normal T4 levels had T3 values above normal range. In 6 of them pathologically elevated T3/T4 quotients could be observed. A TSH refractory TSH suppression was only found in patients with superior normal T4 values. After a reduction of the replacement doses the T3 values were normal. The findings show--that not only the T4 values should be controlled under treatment with thyroid preparations but also the T3 and TSH serum levels. The replacement therapy of hypothyroidism with T3/T4 preparations in proportion 1:4 should be revised.

[Performance of uteroplacental perfusion in contrast to hormone-analytic investigations in intrauterine retardation and under the influence of intravenous long-term application of heparin (author's transl)].

The digital estimation of utero-placental circulation by means of nuclear-medical examinations with help of 113 m In-Cl and regarding to the "region of interest" gives no depositions in cases with an intrauterine retardation. This concerns the activity as well in the placenta as in the uterus too. Exceptions represents intrauterine retardations on the base of heaviness EPH-gestosis and placenta previa. Herewith the rise of activity corresponds with the thickness in the III. trimester of pregnancy. A treatment about several weeks with Hepain under bed conditions leads to no proofed influence of the uteroplacental circulation, on the contrary to a rise of estriol and HPL-values, meaning a improvement of placental performance. The estimation of the utero-placental circulation by means of nuclear-medical examinations is situated to the problem of fetal condition by EPH-gestosis, placenta previa and other.
