Development of a biopolymeric keratoprosthetic material. Evaluation in vitro and in vivo.

Based on the results of in vitro and in vivo experiments, we have determined that the optimal material for the central transparent portion of a perforating keratoprosthesis is a polyvinyl alcohol copolymer hydrogel. The material supports the maintenance and growth of corneal epithelium in vitro as shown by population doublings and transmission electron microscopy. Discs preseeded with epithelial cells were cultured in vitro and transplanted into rabbit corneas. The proliferation of these cells in vivo was demonstrated using 3H-thymidine. Other experiments showed that the preseeded cells not only migrated from the central disc onto the peripheral rim of the host cornea but also that host peripheral epithelial cells migrated onto the anterior surface of the disc. The experiments described in this paper demonstrate that corneal epithelial cells preseeded onto hydrogel discs and transplanted into rabbit corneas remain adherent and are capable of proliferating.

Computerized tomography in the diagnosis of abdominal aortic aneurysms.

Computerized tomography allows good definition of the retroperitoneal space, especially the abdominal aorta. During a 15 month interval, 30 patients with pulsatile, abdominal masses were evaluated by computerized tomography to determine the presence of an aortic aneurysm, 4 centimeters or greater in size. Twenty-one of the 30 patients were found to have an aneurysm. In patients with a normal aorta, information concerning another disease process was often obtained. Results of computerized tomography were then compared with the other diagnostic techniques available and, in certain instances, with the size of the aneurysm at operation. Computerized tomography is the most accurate means of evaluating abdominal aortic aneurysms. The ability to detect a retroperitoneal space hemorrhage and to assess the pathologic condition of other abdominal viscera are two additional advantages.

Use of collagen-hydroxyethylmethacrylate hydrogels for cell growth.

Collagen-hydroxyethylmethacrylate hydrogels were prepared by polymerizing monomeric hydroxyethylmethacrylate in the presence of various concentrations of soluble native collagen. The resulting transparent hydrogels were evaluated as substrata for growth of IMR-90 human embryonic lung fibroblasts. Without collagen no significant growth occurred, whereas a dose-response curve expressing maximal cell growth against collagen concentration could be constructed quite readily by the use of appropriate hydrogels. The method allows for quantification of the collagen contribution to cell growth and, in a more general sense, provides the foundation for a relatively easy procedure to probe mechanisms of cell adhesion and cell differentiation.

Exaggerated phosphaturic response to volume expansion in patients with essential hypertension.

1. Tubular handling of sodium in hypertensive patients has been evaluated with urinary phosphate excretion used as a marker for proximal tubular reabsorptive capacity. 2. Nine hypertensive patients and nine normal control subjects were studied during sustained water diuresis and the intravenous infusion of isotonic sodium chloride solution to produce volume expansion. 3. In the hypertensive patients there was exaggerated phosphaturia, natriuresis and enhanced distal delivery of sodium. Sodium reabsorption in the diluting segment was normal. 4. The enhanced distal delivery and augmented phosphaturia suggest that a decreased reabsorption of sodium in the proximal tubule is the most likely explanation for the exaggerated natriuretic response to volume expansion in hypertensive patients.
