Carotid endarterectomy in the presence of contralateral carotid occlusion: the role of EEG and intraluminal shunting.

Patients undergoing carotid endarterectomy in the presence of occlusion of the contralateral carotid artery appear at greater risk for operative-related stroke or death. We had experience with 37 such patients in a five-year period. Routine intraluminal shunting without EEG monitoring was used in nine patients. Twenty-eight patients had continuous EEG monitoring during surgery. Of this group, 12 patients required intraluminal shunting based on intraoperative EEG criteria. In the early postoperative period, there was one death, and there were no instances of new, fixed neurological deficits. Life table analysis shows that 80% of the patients are neurologically stable in the five-year follow-up period. Electroencephalographic monitoring proved valuable in the detection of patients requiring intraluminal shunting, in the occasional recognition of poorly functioning shunts, and in the determination of the importance of alterations in blood pressure or cardiac rhythm on cerebral blood flow.

Glottic reconstruction after near total laryngectomy.

It is generally accepted that vertical partial laryngectomy can yield satisfactory cure rates in properly selected glottic carcinomas. Several authors have discussed different reconstructive techniques following extended vertical partial laryngectomy. These have included the use of stents, keels, various soft tissue "free" grafts, or muscle transplants. All of the above require prolonged tracheostomy and staged surgical procedures with the resultant upper airway sometimes being less than satisfactory.

Capillaria hepatica (Bancroft) in select rat populations of Hartford, Connecticut, with possible public health implications.

Capillaria hepatica was found in 82% of 86 Norway rats (Rattus norvegicus) trapped in Hartford, Connecticut between February and November 1975. Adults were parasitized more frequently than juveniles and infection rate did not differ between sexes. Rats demonstrated a low intensity of infection, with 9% having extensive liver involvement. A seasonal variation in infection rate may have been a manifestation of environmental factors, cannibalism, and predation.

Antibody-dependent cellular cytotoxicity by allosensitized human T cells.

Peripheral human T cells, isolated by sheep erythrocyte-rosette formation and density centrifugation, were highly cytotoxic to both Ab-coated autologous lymphocytes and antibody (Ab)-coated chicken erythrocytes when stimulated in mixed lymphocyte culture, but were not lytic when freshly purified, or when unstimulated in 6-day culture. Allosensitized T cells were shown to effect this activity by a specific effector-target cell interaction dependent on Ab, as indicated by: (a) induction of killing by Ab to target cells not lysed in the absence of Ab. (b) inhibition of Ab-dependent killing by aggregated Ig. The mechanism by which allosensitized T cells effect antibody-dependent cellular cytotoxicity is discussed.

Antibody-Dependent Cell Cytotoxicity

The sphenoid sinus, the neglected nasal sinus.

O. E. Van Alyea stated that "The sphenoid sinus is the most neglected of the nasal sinuses." It is neglected by disease because of its location, by the physician because of its subtle symptoms when diseased, and the surgeon because of its inaccessability. An accurate knowledge and understanding of the anatomy of the area help the physician diagnose and manage problems in this area. Eleven cases are used to illustrate the wide spectrum of pathologic conditions in and about the sphenoid sinus. Only two of the patients were asymptomatic. All of the others had deep-seated, retrobulbar headaches. Eight of 11 patients had cranial nerve involvement. The optic nerve was involved in six cases. The abducens nerve was involved in five of 11 cases and the maxillary nerve in four of 11 cases. Initial roentgenographic examination must include lateral and submental vertex views. The newer techniques of angiography, polytomography, and computerized tomography aid in determining the extent of the pathologic condition and the treatment approach.

Diagnosis of juvenile angiofibroma by computed tomography.

Computed tomography (CT) accurately localized juvenile angiofibromata in 3 patients. The expanded pterygopalatine fossa and canal were visualized by CT in all three cases. Because of the hemorrhagic tendency of these tumors, a noninvasive modality such as CT is especially valuable in planning therapy.

Adolescent

An extraordinary form of confabulation.

We describe five patients with spontaneous, persistent confabulation. While the basis of confabulation in general is not known, the evidence in this specific group of patients with spectacular, impulsive, and spontaneous confabulation suggests a marked deficit in frontal function, superimposed upon a basic defect in memory, as a possible mechanism.

Specificity and genetic restrictions of the guinea-pig immune response to dinitrophenyl-lysyl-alanyl octapeptides.

A series of 2,4-dinitrophenyl (DNP) octapeptides containing L-lysine and L-alanine were prepared to examine the specificity and genetic restrictions of both cellular and humoral immune responses in inbred guinea-pigs. Strains 2 and 13 guinea-pigs were therefore immunized with Lys4-Ala3-Lys(DNP), Lys3Ala4-Lys(DNP), Lys2-Ala5-Lys(DNP) and Lys-Ala6-Lys(DNP). Only Lys4-Ala3-Lys(DNP) was under Ir gene control and could induce both antibody and T-cell responses in strain 2 guinea-pigs. In contrast, Lys4-Ala3-Lys(DNP) injected in strain 13 guinea-pigs and the other DNP-octapeptides injected in strain 2 or 13 guinea-pigs elicited only antibody formation and no specific T-cell mediated response. Antibody formed in the absence of specific T-cell responsiveness in either strain 2 or 13 was hapten specific and lacked the capacity to discriminate the immunizing antigen from closely related DNP-peptides. Antibody produced by animals with specific T-cell responses, on the other hand, was exquisitely specific for the immunizing peptide and could discriminate it from closely related peptides.

Alanine

The preoperative diagnosis of hepatic metastases in cases of colorectal carcinoma.

Preoperative LDH, CPK, alkaline phosphatase and liver scan results were compared with the presence or absence of hepatic metastases at operation in 124 laparotomies in 123 patients with colorectal carcinomas during a three-year period. The overall accurcy rates for the four tests ranged from 74 to 84 per cent. The false-negative rates were acceptably low (9-13 per cent) for all but the CPK determination. The false-positive rates were unacceptably high for all four tests (38 to 60 per cent). A 'metastatic score' for reducing the high false-positive rate is described. In view of the high false-positive rates of preoperative screening tests, laparotomy is presently the most accurate method of diagnosing hepatic metastases.
