[The clinical behaviour of by immunology unique courses of NANB hepatitis (author's transl)].

By means of both immunohistology and immunodiffusion a number of 7 by definition from NANB hepatitis suffering patients were found to belong to an unique antigen-antibody system. These patients responded to NANB infection with certain similarities: 1. The course of the disease exhibited a phasic pattern since increases in SGPT activity were observed at days 47 +/-- 10, 76 +/- 14, 117 +/- 18 and 157 +/- 7. 2. Increases of Gamma-GT were found to exceed values as observed in HAV and HBV infection. Thus, Gamma-GT increases are incriminated as further characteristic of NANB hepatitis.

[Alpha 1-antitrypsin deficiency and the liver (author's transl)].

Alpha 1-antitrypsin (alpha 1 AT) is a glycoprotein of hepatic origin which functions as a systemic protease inhibitor (Pi). Its production is controlled by two autosomal-codominantly transmitted alleles. Among the numerous genetic variants some alleles (predominantly PiZ) may induce alpha 1 AT-deficiency, facultatively associated with childhood liver disease. However, the pathogenesis of this congenital disorder, which may progress to complete cirrhosis remains obscure at present. In addition, no clear cut relationship has been proven between alpha 1 AT-deficiency and deranged liver architecture, observed in advanced aged adults. Possibly this may reflect a more accidental coincidence with the consequences of chronic viral hepatitis (Non A-Non B-type). Nevertheless, this hypothesis is hitherto unestablished as it holds for the supposed association between alpha 1 AT-deficiency and the occurrence of malignant hepatoma.

[Peliosis hepatis. A clinical status inventory].

Peliosis hepatis is a rare condition, recognizable by macroscopical view. It is characterized by multiple blood-filled cystic spaces in the liver parenchyma. According to 49 out of 152 more recent case reports (1951-1981/82) its spontaneous occurrence is frequently associated with malignant and toxic processes. However, in about 70% peliosis may be induced by the action of certain drugs, especially by 17 alpha-alkylated steroids. Usually peliosis is found by chance at autopsy or peritoneoscopy, as the clinical picture does not provide sufficient conclusive criteria to allow a definite diagnosis. The striking macroscopical appearance contrasts to the dispute on the as yet unestablished natural history. As most important complication spontaneous intraabdominal bleeding can occur in occasional cases. On the other hand, the patients predominantly die of their underlying diseases and not of a peliosis hepatis.

[Endotoxins and the pathogenesis of gastrointestinal diseases (author's transl)].

Endotoxins are constituents of the capsules of intestinal gram-negative Flora. Under pathological conditions with limited clarification function of the liver RES (alcoholic hepatitis, severe virus hepatitis, cholestasis, cirrhosis of the liver) they do reach the systemic circulation. If the colon is primarily diseased (ulcerative colitis, Crohn's disease) or secondarily damaged (edema of the colonic wall in acute pancreatitis) they pass through the wall into the peritoneal cavity and from there into the systemic circulation (endotoxemia). Attention to the endotoxemias can explain a number of systemic complications which often determine the clinical course of the diseases named. A basis is also created for seeking an effective form of therapy.

Chiba-needle percutaneous cholangiography - a method without risk to the patient?

This critical review of PTC with the skinny needle was prompted by an unexpected complication - an intrahepatic hematoma. A negative PTC does not exclude the presence of dilated bile ducts. In the case of an obstructed biliary tree, bile leakage occurs in approximately 5%. Even after the Chiba needle or other techniques have become established, peritoneoscopy is still of value in the diagnosis of cholestasis. Thus, this method is not without some risk and optimistic reports should be judged with a certain reserve.

Lactulose--a drug with antiendotoxin effect.

Lactulose (beta-galactosido-fructose) was found to have anti-endotoxin properties: 670 mg lactulose abolished the gelating activity of mg E. coli endotoxin on Limulus lysate in vitro. When lactulose was fed to rats (6.3 +/- 1.1 g/kg/day) over a period of 4 or 8 days before i.v. administration of 0.5 g/kg galactosamine, the liver damage that normally develops was prevented. Since galactosamine-induced necrosis of hepatocytes and inflammatory reaction of the liver are mediated by systemic endotoxemia of intestinal origin, an anti-endotoxin effect of lactulose was demonstrated in vivo. We suggest that lactulose might offer a therapeutic basis in clinical situations in which endotoxemia is of pathogenetic significance, such as certain gastrointestinal and liver diseases, shock states and gram-negative sepsis.

[Problems of hepatoportal encephalopathy after portacaval anastomosis in the rat (author's transl)].

Necrosis and injury of ganglia cells as well as alterations of the glia represent the morphological changes in portal-systemic encephalopathy. This is a frequent complication of a spontaneous or surgical porta-caval collateral circulation. Such changes can also be observed when porta-caval end-to-side anastomosis (PCA) was constructed in otherwise healthy rats. Increased incorporation rates of 3H-thymidine into the glia were indication for enhanced proliferating activity. Efforts done to maintain the pancreatic venous blood flow to the liver beside PCA resulted in significant diminuation of brain pathology and function. The latter measured by EEG. This happened although the grade of hyperammonemia was virtually identical in both settings. The interpretation is offered that access of the liver to hepatotrophic substances (i. e. insulin) from the pancreas prevents portal-systemic encephalopathy in PCA which is in agreement with data of the literature. The observation implies that in clinical action exclusively those surgical techniques should be used by which the venous blood supply from the pancreas to the liver is maintained.

[Endotoxinemia in acute pancreatitis (author's transl)].

In a clinical study the question was asked, whether endotoxinemia of intestinal origin accompanying acute pancreatitis would influence decisively the course of this disease. In 21 of 32 patients with acute pancreatitis endotoxinemia could be diagnosed by the Limulus test, in this group 9 patients died. In the group of patients without endotoxinemia however no patient died. Extrapancreatic complications as for instance coagulation defects, renal failure, and pulmonary changes were predominant in the patients with endotoxinemia. From these findings the conclusion is made, that endotoxinemia occurs frequently in acute pancreatitis and influences decisively the course of the disease.

[Fiberendoscopic injection therapy of bleeding gastrointestinal lesions (author's transl)].

In 28 patients with acute gastrointestinal bleeding emergency fiberendoscopy was combined with aethoxysclerole (1%) injection of the bleeding lesion with purpose to controll haemorrhage. In 61% of 31 proceudres done in patients with oesophageal varices (n = 19) haemorrhage was controlled, and in further 16% deminuation of bleeding intensity was noted. In the remaining cases (n = 7) the procedure was ineffective. Only patients with Child C liver cirrhosis having oesophageal varices stages III and IV finally died because of uncontrolled haemorrhage. In 9 patients with bleeding from other lesions (gastric erosions and ulcers, Mallory-Weiss-Syndrome, erosio simplex Dieulafoy) haemorrhage was controlled in 8 patients. The method is practicable and efficient, but does not determine better the final outcome of patients with livercirrhosis Child C having oesophageal varices stages III and IV. In other cases tube treatment was avoided. The operation lethality within the series was 1,5%.
