[Prevention of deep-vein thrombosis in patients with gynaecological cancer undergoing radiotherapy. A comparison of calcium heparin and a semisynthetic heparin analogue (author's transl)].

Patients treated for gynaecological cancer by combined external irradiation and intravaginal radium application participated in two randomized, controlled, and prospective trials. In the first trial the incidence of deep vein thrombosis (DVT) diagnosed by the 125I-fibrinogen test was 43% in the control group. The injection of 7,500 i.u. of subcutaneous calcium heparin twice daily prevented DVT in the test group. An increase of soluble fibrin monomer complexes (SFMC) and fibrinogen in the plasma of patients with DVT suggested that the patients were hypercoagulable. No correlation between AT III values and DVT was observed. In the second trial, the effect of the same dose of heparin was compared with a twice daily s.c. injection of 5,000 U of a semi-synthetic heparin analogue (SSHA). The incidence of DVT was reduced to 15% by heparin; in the patients given SSHA the incidence was 25%, which was not significantly different from the heparin-treated group.

[Urodynamic follow-up investigation after radical abdominal hysterectomy and lymphadenectomy for invasive carcinoma of the uterine cervix (author's transl)].

80 patients without recurrence of carcinoma of the cervix were investigated. The patients had radical abdominal hysterectomies and lymphadenectomies according to Makkenrodt-Latzko-Meigs in the years 1970-1977 at the first university department of gynecology and obstetrics in Munich. The investigation was carried out in the urodynamic testing unit which is described. 47.5% of the patients were free of symptoms. 21% of the women had stress incontinence. 19% of the patients had neurogenic bladder symptoms. 44 of the 80 patients had post-operative radiotherapy. There was no significant difference in the urinary symptoms and signs of the group with and the group without post-operative radiotherapy. The symptoms of the patients from a standardized questionaire did not coincide with the urodynamic study results.

[Perinatal mortality in foreign workers (author's transl)].

From 1970 to 1972, there were 216 perinatal deaths among 5595 newborns at the I. Frauenklinik der Universität München. 54 of these deaths were children of foreign workers (so-called "Gastarbeiter"). The data have been processed on punch cards and analysed by a computer. The differences noted underwent significance testing by the CHI-Quadrat test. Only statistical significant results are published. The perinatal mortality in the above period shows no difference between foreign and German ward patients. There is, however, a significant lower perinatal mortality in private patients. We feel that this difference is due to a significant lower rate of prematures in the private patient group. The cocial status as well as higher interest and motivation in health resulting in better prenatal care are discussed as causal reasons for this fact.

Nemaline cardiomyopathy in a young adult: an ultraimmunohistochemical study and review of the literature.

Heart transplantation was performed in a 26-year-old man who suffered from severe dilatative cardiomyopathy. A nemaline myopathy characterized by the accumulation of Z-line material and the formation of rod-like structures had been diagnosed in the skeletal muscle. Routine light microscopy of the heart disclosed only nonspecific findings. On electron microscopy scattered cardiomyocytes showed formations of rod-like structures and a structural desintegration of contractile filaments near the intercalated disks. Immunocytochemistry at the light and electron microscopical level exhibited an accumulation of alpha-actinin, desmin, and occasionally vinculin in abnormal cardiomyocytes. The rods were specifically stained with alpha-actinin and were less immunoreactive for desmin. No mutations were revealed in the skeletal muscle alpha-actin gene. The results illustrate a complex derangement of the cytoskeletal apparatus in nemaline cardiomyopathy. Nemaline cardiomyopathy may be difficult to diagnose in routine diagnostic procedures. A close correlation between the severity of cardiac dysfunction and the morphological expression of the disease in the heart may not be found. Nemaline cardiomyopathy should be included in the differential diagnosis of dilatative cardiomyopathy and may be diagnosed with certainty by ultrastructural-immunhistochemical investigations.
