Pancreatic supplementation in end-stage renal disease.

The common occurrence of a wasting syndrome in patients with end-stage renal disease (ESRD) on haemodialysis indicates the need for nutritional management. Pancreatic hypofunction has been documented in these patients, and is suggested as a contributory factor to the wasting of ESRD. The patient presented illustrates the presence of pancreatic disease, and details his management. The advantages of pancreatic supplementation in improving nutritional status in ESRD are considered.

Pancreatic exocrine hypofunction in the wasting syndrome of end-stage renal disease.

Pancreatic exocrine insufficiency in patients with end-stage renal disease on hemodialysis is not well documented despite the prevalence of a wasting syndrome, which may indicate pancreatic involvement. The present study intended to evaluate pancreatic exocrine function in this group. Eight patients were studied, none of whom had a history suggestive of pancreatic exocrine involvement. Routine 72-h stool collection was performed, and estimation for fecal fat showed steatorrhea in four patients. The secretin-pancreozymin test (direct stimulatory method) showed a statistically significant decrease in amylase and lipase levels in the duodenal aspirate, and increased basal and stimulated serum amylase (p less than 0.05). Values for volume, bicarbonate, and trypsin in the aspirate were normal. The role of pancreatic hypofunction in the etiology of the wasting syndrome of end-stage renal disease is therefore considered, as well as the advantages of pancreatic supplementation in improving nutritional status.

Acute hepatitis following enflurane anaesthesia.

A patient developed fever and acute hepatitis shortly after enflurane anaesthesia. Other causes of postoperative hepatitis were excluded. Cross-sensitization with halothane may have occurred, and the enflurane hepatitis may have been aggravated by halothane hepatitis.

Effects of fasting on the composition of gallbladder bile.

Gallbladder bile was aspirated from the duodenum in nine healthy women after three separate periods of fasting after a standardised meal. Mean cholesterol saturation index was significantly greater after a 15 hour fast (1.35 +/- 0.08) than after a 10 hour fast (1.05 +/- 0.10). However, after 20 hours of fasting, mean cholesterol saturation index had fallen to 1.13 +/- 0.10. Total biliary lipid concentration was unaffected. These findings suggest that fasting has a series of effects on gallbladder bile and that bile sampling should be carried out at at standardised period of time after the last meal.

Whipple's disease. A case report.

A case of Whipple's disease in which there were some unusual features, although the results of histological investigation were characteristic of the diseases, is reported. This is followed by a brief review of the literature concerning this rare condition.

Calcification of the left atrium.

Three cases of left atrial wall calcification complicating long-standing mitral valve disease are presented. Its possible pathogenesis and influence on further treatment are discussed.
