Olanzapine for the treatment of fibromyalgia symptoms.

Fibromyalgia is a chronic condition that is diagnosed primarily by the presence of generalized pain along with tenderness on palpation of certain body regions. Unfortunately, the pharmacological treatment of fibromyalgia remains problematic. Two patients are described who highlight the use of the atypical neuroleptic olanzapine for the control of symptoms related to fibromyalgia. Prior to the use of olanzapine, both patients had received a multitude of treatments, none of which greatly improved their ability to function in daily activities. With olanzapine, both patients reported a significant decrease in pain and marked improvement in daily functioning. In one case, the pain returned during a period of time when olanzapine was discontinued, an effect that was reversed when olanzapine was reintroduced. The paucity of serious side effects (i.e., extrapyramidal signs) with the atypical neuroleptic olanzapine strongly favors further exploration and use of this drug for the treatment of fibromyalgia symptoms.

A high-resolution HAPPY map of Dictyostelium discoideum chromosome 6.

We have made a high-resolution HAPPY map of chromosome 6 of Dictyostelium discoideum consisting of 300 sequence-tagged sites with an average spacing of 14 kb along the approximately 4-Mb chromosome. The majority of the marker sequences were derived from randomly chosen clones from four different chromosome 6-enriched plasmid libraries or from subclones of YACs previously mapped to chromosome 6. The map appears to span the entire chromosome, although marker density is greater in some regions than in others and is lowest within the telomeric region. Our map largely supports previous gene-based maps of this chromosome but reveals a number of errors in the physical map. In addition, we find that a high proportion of the plasmid sequences derived from gel-enriched chromosome 6 (and that form the basis of a chromosome-specific sequencing project) originates from other chromosomes.

PTCA of complex atherosclerotic lesion at site of LAD myocardial bridging.

An 86-year-old patient with a history of chronic angina pectoris and a remote cerebrovascular accident presented to the hospital with an acute myocardial infarction complicated by a concomitant nonhemorrhagic cerebrovascular accident. Postmyocardial infarction angina pectoris, refractory to maximal medical therapy, led to coronary angiography. This demonstrated the "culprit" lesion to be a complex and critical atherosclerotic lesion at the distal-most extent of a long area of left anterior descending (LAD) myocardial bridging. This "culprit" lesion was successfully treated by balloon angioplasty. This is the second report in the recent literature of balloon angioplasty revascularization used in this fashion. Similarities and differences between our case report and the prior report in the literature are compared and contrasted.

Aspiration of a mobile thromboembolus from saphenous vein graft.

An 81-year-old woman who was 4 years post 3-vessel coronary artery bypass graft (CABG) surgery suffered a non-Q-wave myocardial infarction. Angiography revealed the recent occlusion of a saphenous vein graft (SVG) to an obtuse marginal coronary artery. Five days following failure of intragraft urokinase and systemic heparinization to recanalize the SVG, balloon angioplasty was undertaken. A mobile thrombus was observed to migrate spontaneously in a retrograde manner in the SVG and was removed during PTCA by aspiration through the guiding catheter.
