[The medical responsibility for health and environment of future generations. The ecological dilemma in the meeting between medicine, environmental health services and protection].

Medical doctors are challenged to consider their responsibility for the health and environment of future generations. The fields of health, medicine, development and the environment overlap in various ways. This article describes ecosystem health and biohistory as new models of medical interest. The ecological dilemma, the biomedical model and postbiomedical thinking are also discussed. The municipal medical officer should regard the whole municipality, with its ecosystems, communities and organizations, as areas of medical concern. He or she should find a credible working style which opens up for interdisciplinary discussions and actions.

[The challenge of ecology to medicine].

The science of ecology is an important tool for reflecting upon the interaction between organisms and the environment. However, this science cannot be applied directly to human systems, since human interaction is very complex and is often based on symbolic meanings. An interdisciplinary approach to human ecology is therefore necessary. At a deeper level, ecological thinking might be integrated into a vision of the unity of reality, which includes both subjective and objective experiences, and where human values are just as important as facts. Ecological thinking challenges medicine to take a multidimensional view of man and to regard man, in common with life in general, as a self-organizing system. Value-based ecological thinking should be just as relevant in situations as diverse as our relationship to the individual patient and his community, and our own relationship to the industrial society and our common global environment.

Cancer incidence among workers exposed to radon and thoron daughters at a niobium mine.

The aim of this study was to investigate the incidence of cancer among 318 male employees of a niobium mining company which was only operated between 1951 and 1965. Many of the workers, especially underground miners, were exposed to the daughters of radon and thoron and also to thorium. The accumulated doses to the workers from short-lived radon and thoron daughters in the mine atmosphere were assessed to be relatively low; up to 300 working-level months. During the follow-up period 1953-1981, 24 new cases of cancer were observed compared to an expected number of 22.8. Twelve cases of lung cancer had occurred versus 3.0 expected. Among the 77 miners, 9 cases of lung cancer were observed against 0.8 expected. Associations between the occurrence of lung cancer and exposure to alpha radiation and smoking were found. For the radon and thoron daughter exposure, about 50 excess cases per million person-years at risk per working-level month were observed.
