[Siderosis of the brain and spinal cord. Report of two cases].

Two cases of superficial siderosis of the brain and spinal cord with cochleovestibular and cerebellar symptoms are diagnosed on brain and spinal MRI scans. Low signal intensity lines are noted on the surface of the brainstem, cerebellum, spinal cord and within the interhemispheric and sylvian fissures. In one case, no brain or vascular malformation is identified; in the second case, two cavernous angiomas are noted on the MRI study. 3D CISS may visualize thickening of the cochleovestibular nerve.

Brain Diseases↗

[Brain MR imaging of chronic alcoholism].

Brain complications from chronic alcoholism (Wernicke encephalopathy, central pontine myelinolysis, Marchiafava-Bignami disease, Korsakoff's syndrome, hepatic encephalopathy, cerebellar atrophy, hemorrhagic and ischemic brain lesions) may be diagnosed by MR imaging.

[Intracerebral radiation-induced cavernous angiomas].

We present four cases of cerebral cavernous angioma that developed after radiatherapy for brain tumor in three cases and for cavernous angioma in one case. The time interval between irradiation and the detection of the cavernous angioma varied from three to nine years and the doses from 24 to 60 Grays. Brain hemorrhage appeared in two cases. Explanation for the formation of cavernous malformations is unclear but is probably related to proliferation and dilatation of the vascular endothelium with formation of capillary telangiectasis with evolution to cavernous angiomas. The pediatric brain appears particularly vulnerable to radiation injury. The risk of hemorrhage appears higher than with spontaneous cavernous malformations.

[Epidermoid cyst of the fourth ventricle: four case reports].

Patients with epidermoid cyst of the fourth ventricle usually present with headaches and/or disequilibrium. These cysts are characterized by a focal lesion that is nearly isodense to CSF at CT and nearly isointense to CSF on T1W and T2W MR images. MRI using FLAIR and diffusion weighted images as well as 3D CISS acquisitions is useful to better characterize the lesions and their relation with the vermis, foramen magnum and CP angle cisterns. DWI images are useful for postsurgical evaluation of residual tumor. Extension of the cyst into the CP angle cisterns usually precludes complete surgical resection.

[Value of MRI findings in Gayet-Wernicke encephalopathy].

Wernicke encephalopathy (Wernicke-Korsakoff encephalopathy) is related to thiamine deficiency. We report the MRI findings in four patients with visualization of bilateral and symmetrical hyperintense foci on T2W and FLAIR images involving the periaqueductal gray matter, the mamillary bodies and around the third ventricle. Diffusion weighted images obtained in two patients demonstrated mild hypersignal in the same areas. Contrast enhancement within the mamillary bodies was noted in one patient. Follow-up MRI obtained in three patients showed rapid regression of signal abnormalities without correlation with good clinical outcome.

Special issues in the psychopharmacological management of Hispanic Americans.

Despite the fact that Hispanics are becoming the largest minority in the United States, there is a paucity of studies of psychopharmacology in Hispanic Americans. However, drawing from many sources, it is possible to illustrate multiple issues pertinent to their treatment with psychotropic medications. This review addresses several of these issues, including the medical validity of the term Hispanic; some general characteristics of the Hispanic population and its use of medical and mental health services; clinical psychopharmacology in Hispanics (antidepressants and antipsychotics); available information on characteristics of the cytochrome system in Hispanics; some environmental factors that may affect drug response in Hispanics; and Hispanic attitudes regarding mental health and medication. The article ends with some recommendations for the physician treating Hispanic patients.

[Development of an alarm system for the prevention of falls in hospitalized patients].

The present study describes the analysis of an online patient assessment system developed to prevent inpatients falls. A chart review was performed in order to identify risk factors present in the Nursing Assessment tool. The identified variables were Functional status including walking, transferring and toilet, sex and patients self-care ability. Authors developed a system using these variables, that is available since February 1997. In a second phase of this study, authors aim to evaluate the effectiveness of this computerized intervention.

[Giant bronchopyocele. General and radiological characteristics. Apropos of a case].

A case of giant bronchocele is reported. This lesion is due to a mucus and pus filled dilatation of the bronchial tree proximal to a stenosis. The latter can be either congenital or acquired, or in the context of a diffuse bronchopathy, which can be responsible for a functional stenosis. Both plain radiography and computed tomography findings are diagnostic. The etiology of the abnormality however represents an important problem to be solved. Further, a thoractomy is often necessary, except in those case arising in the context of a diffuse bronchopathy, which usually resolve either spontaneously or following medical treatment.

K+ current stimulation by Cl- in the midgut epithelium of tobacco hornworm (Manduca sexta). II. Analysis of Ba(2+)-induced K+ channel conduction noise.

Chloride-stimulated K+ secretion by Manduca sexta midgut (5th-instar larvae) was measured as K(+)-carried short-circuit current of the tissue mounted in an Ussing chamber. "Microscopic" parameters, such as single-channel current and channel density for the rate-determining passive transport step across the basolateral goblet cell membrane (i.e. K+ channels), were estimated by means of current-fluctuation analysis of the K+ channel blockade by haemolymph-side Ba2+ ions. Ba2+ was equally effective with Cl- or gluconate (Glu-) as the principal ambient anion. The Ba(2+)-induced K+ channel conduction noise is reflected by a Lorentzian, or relaxation, noise component in the power spectrum of the K+ current fluctuations. A reduced Lorentzian plateau value, but an unchanged corner frequency, were observed when Cl- was replaced by Glu-. The results from the analysis of a "two-state" model of K+ channel block by Ba2+, with respect to the anion-replacement effects, suggest that the observed changes in K+ current and Lorentzian plateau value mirror a complex change of the underlying parameters: Cl- omission reduces single channel current but increases channel density so that the product of single channel current and channel density is smaller in Glu- than in Cl-. It seems likely that basolateral K+ channels (1) are subject to anionic gating ligands, and (2) depend on anions with respect to the rate of K+ transfer through an open K+ channel.

[Use of a computer in a clinical-toxicological laboratory with a fuzzy search].

If a clinician is faced with acute intoxications a rapid toxicological test is essential. The toxic substances are identified by a combined use of various systems of analysis such as gas chromatography, thin-layer chromatography or UV-spectroscopy. As the result of the analysis a data pattern is obtained which can be interpreted by comparing it with substance-related reference patterns. This paper describes a computer-program which allows more efficient reference comparisons and which gives a support of the decisions. The algorithms of the comparisions are based on fuzzy-theory.

[Results of nutrition diagnostic studies in patients with bronchial cancer].

A group of 71 patients with bronchial carcinomas are submitted to nutritional assessment in this trial. A 6-point valuation scan from Schmoz et al. for the nutritional assessment is used. Especially the results of protein assessment and the skin tests with 4 recall antigens are worked out. These results are compared with reference ranges. In the pathological values, both the protein assessment and the skin tests are classified in 3 interference degrees. In the following interpretation, the group of patients are subdivided in one group, which undergoes conservative therapy (26 patients) and a second one, which is treated operationally (45 patients). The results showed that more than half of the patients had disturbed nutrition indices, more than 2/3 showed disturbed protein status and more than 90% of the patients showed pathological results in the skin tests.

[The cito-laboratory information system CILIS].

The cito-laboratory of a big hospital must be able to work a higher number of cito-analyzes economically and with a high degree of security and to send back the results as fast as possible to the demander. For that reason the simple employment of efficient analytical methods or tools is not sufficient. The cito-laboratory-information-system CILIS is able to support all the steps of work, which are necessary to workout cyto-results and to send them back to the demander. It is also able to relieve the nurses as well as the MTLA from writing and recording. CILIS is based on the computer A 5120. It is usable in other comparable institutions and with certain restrictions to further development or enlargement. In our hospital the system has been working since november 1986 during the regular shift and the shift from 2 pm till 10 pm and partly on working days from 10 pm to 7 am and respectively at the weekend. Since that time it has been approved excellently.

The chloride-stimulated K(+)-secretion by insect midgut and its modification in the presence of osmotic gradients: a short-circuit current and noise-analysis study.

K(+)-secretion in the midgut of the larval moth, Manduca sexta, was studied by measuring the kinetics of the lumen-directed short-circuit current (Isc) and the conduction noise from basolateral K+ channel block by Ba2+. Hemolymph chloride as well as hypotonicity both stimulate this K+ current (IK). The kinetic nature of the stimulation is, however, different in each case. Analysis of blocker noise supports, to a large degree, the interpretation obtained from kinetics, namely: chloride ions do not act via changes in cell volume but influence ion turnover and channel number.

A protocol for status epilepticus in a long-term care facility using rectal diazepam (Diastat).

INTRODUCTION: A growing population of residents in long-term care facilities (LTCF) is likely to present with status epilepticus (SE), which requires rapid treatment to avoid permanent neuronal damage, associated morbidity, and mortality. The treatment of choice for SE is an intravenous benzodiazepine; however, this is not feasible in most LTCFs because of the training level of the personnel and the risk of respiratory depression. Transport to acute care facilities is often delayed and, in addition, many LTCF residents have "do not hospitalize" orders. As an alternative to an intravenous benzodiazepine, rectal delivery of diazepam has been shown to be effective in stopping prolonged seizures and is more easily accomplished in the LTCF setting. METHODS: The Badger Prairie Health Care Center (BPHCC), a 132-bed LTCF in Verona, WI, developed a protocol for SE that uses a rectal diazepam gel preparation (Diastat) when intravenous access is unavailable. The protocol provides for initiation of long-term anticonvulsant therapy as a follow-up to the acute treatment of SE. RESULTS: BPHCC staff successfully employed the protocol eight times from August 1999 to November 2000; this article describes the protocol and three representative cases.
