[Fractures of the proximal radius in the growth stage].

From 1970 to 1985 in the Orthopedic University Clinic (Oskar-Helene-Heim Berlin) 116 children with fractures of the proximal end of the radius received medical treatment. The frequency of the remaining complications like restriction of movement, differences in elbow axis angle (carrying angle), disturbances of growth or nerve lesions are described. The cause of those complications and the facilities for treatment are discussed in regard to controversy recommendations for therapy in literature. Special reference is made to problems of dislocation ad latus and late operative treatment.

Adolescent

[Soft tissue diagnosis of the shoulder joint using sonography, computerized tomography and magnetic resonance tomography (MRT)].

Injuries and degenerative alterations of the shoulder predominantly affect the soft tissue. To investigate this area we use a multitude of diagnostic examinations: apart from clinical and functional diagnosis also the image reproductive methods of arthrography, ultrasound and computer tomography in addition to the classical X-ray. To ascertain pathological changes to shoulder soft tissue the simplest and quickest, least dangerous and most straightforward examination is the sonograph as it includes dynamic movement procedures. Warning: a high degree of experience and anatomical knowledge is a must! Computer- and especially nuclear spin tomography--being static examining forms a comprehensive picture of every anatomic structure and its pathology--may well represent a valuable addition to shoulder diagnostics. Nevertheless, as yet the clinical and material requirements are still enormous.

[Neurinoma of the L3 spinal nerve root as a cause of stress-induced lumbo-ischialgias].

Degenerative diseases of the lumbar vertebral discs or the vertebral joints are the main causes of low back pain and sciatica following overstraining during sports activities. However, they are rarely seen in sportsmen engaged in competitive sports because these persons are mostly young and well-trained. Other neurological diseases are even rarer as possible causes of sciatica. That is why we report on a young athletic who is a short-distance hurdler (110m hurdle race) and has been suffering from a neurinoma of the spinal nerve root L3 with sciatica and pain in the knee as initial symptoms. Attention is drawn to the importance of neurological examinations as part of medical examinations conducted by sports physicians for the purpose of obtaining an overall guiding picture of the subjects' health status.

Adult

[Apophyseal rupture of the pelvis and lower extremity caused by sports].

Apophyses are secondary ossification centers and serve as the insertion or origin of tendons. They are under a dynamic but not static load. During sexual maturity, they represent a weak point in the locomotor system. Apophyseal injuries can apparently be attributed to an incongruity between load tolerance and increased strain on the seam of the apophysis. The majority of these injuries occur during sports and involve men more frequently than women. The injury is often primarily misinterpreted as a pulled-muscle. The treatment method is dependent on the localization of the injury and the extent of the fragment dislocation.

Adolescent

[Experimental experience with autologous nerve-transplantation in rabbits (author's transl)].

Autologous nerve transplantations of the tibial nerve were carried out in 15 rabbits. In an average period of 19 weeks a return of function could be observed in 13 of the animals. Neuro-physiological, histological and scanning-electron microscopic examinations were done. A comparison was performed with results after simple nerve suture, done under equal circumstances. Nerve suture leads to a relative better regeneration. To explain this result the formation of a scar in the distal suture was discussed.

[Comparative measurements of PWC170 with work steps of different load increase and duration (author's transl)].

The "Physical Working Capacity" (PWC170) which has been shown to be an ergometric method for determination of the physical performance is due to measurement of heart rate during physical work under steady-state conditions. However the practical application of those methods of multiple work steps of six minutes is limited due to the time-consuming procedure. Therefore, we measured in 30 test-persons PWX170 with work-steps of 50 Watt/6 min. in comparison with 25 Watt/2 min. The difference between these two test-series of 3.7% was not significant. For daily determination of PWC170 the more time-saving procedure with work steps of 25 Watt/2 min. is suggested.

[The speed of nerve-conduction after micro-surgical suture of the tibial nerve of the rabbit (author's transl)].

Isolated division of the tibial nerve in the thigh preserving the peroneal and sural nerves does not cause loss of normal position of the animal, in particular of sitting. Nor does it lead to pressure sores. The animals are fitter then after division of the sciatic nerve. Microsurgical suture of the isolated tibial nerve in the thigh of the rabbit with 1 or 2 simple sutures with 10 x 0 thread show that an average recovery can be achieved in over 80 per cent. As few sutures as possible should be used. Otherwise there with be scarring and no return of function.
