A selective inhibitor of thromboxane biosynthesis enhances immediate and inhibits late cutaneous allergic reactions in man.

Thromboxanes may be involved in late cutaneous reactions (LCRs). A new selective inhibitor of thromboxane biosynthesis was studied for its effect on immediate skin test reactions (WFRs) and LCRs in nine adult nonatopic volunteers (open study) and eight adult atopic patients (double-blind study vs. placebo). WFRs and LCRs were elicited by intradermal injections of 25 micrograms of compound 48/80 and 200 IU of rabbit anti-human IgE antibodies, and the patients' receiving injections of 0.32, 1.6, 8, 40, and 200 IU of anti-IgE antibodies. Dazoxiben (100 mg orally three times once every 4 hr) increased the diameter of wheals (p less than 0.005 in volunteers; p less than 0.05 in six patients receiving no additional drug treatment), favored their dispersion (lymphangitis/pseudopodia), and caused systemic reactions (headache, chills) in at least two out of eight patients. Edema formation during LCRs was markedly depressed in 13 out of 16 persons. Flare diameter and erythemata during LCRs were not altered. These data suggest a role of thromboxanes and their natural antagonists (e.g., prostacyclin) in the regulation of allergic inflammatory processes.

[Maritogenic insemination - a contribution to the treatment of childless couples. Report on clinical experiences].

We treated 34 couples who urgently wanted to have children by means of artificial maritogenic insemination. Indications for this were combined asthenospermia and oligozoospermia (in 27 cases), therapy-refactory impotentia coeundi of the husband (in 3 cases) and sterility of uncertain origin (4 couples). We achieved 7 pregnancies. Abortions did not occur. 9 additional pregnancies were registered in 8 couples following treatment. In special cases artificial maritogenic insemination availed to recover from marital sterility. Pregnancies obtained after unsuccessful maritogenic insemination are discussed.

[Comparison of intra-uterine devices, Copper-T-200 and Dana-Super-Fix-Cuprum (author's transl)].

The two above IUDs were compared by application to 411 patients, with the Copper-T 200 being used on 239 and the Dana-Super-Fix-Cuprum on 172. Gravidity occurred to 2.9 or 5.8 per cent of all probands. The Pearl indices were 2.6 or 4.2. Spontaneous expulsion was observed in 2.5 or 3.5 per cent of all cases. Devices had to be extracted from 9.6 or 11.6 per cent of the patients. Adnexitis was diagnosed in 0.4 or 1.7 per cent of the cases. Continuity values were 85.0 per cent for the Copper-T-200 and 79.1 per cent for the Dana-Super-Fix-Cuprum. No substantial difference in effectiveness was found to exist between the two above copper-based intra-uterine devices.

[Management of functional sterility using clomiphene. A clinical experience report].

121 patients are reported who underwent Clomiphen treatment for functional sterility in the Frauenklinik des Bezirkskrankenhauses Karl-Marx-Stadt from 1. 1. 1974 to 31. 12. 1977. Monophasic cycles (66 patients), corpus luteum insufficiency (41 patients) and secondary amenorrhoea (14 patients) were the indications for the Clomiphen therapy. 54 pregnancies (44,6%) were the result; in 18,5% gravidity was terminated by abortion or initiated interruption (1,9%). The clinical data are stated.

[Management of functional sterility using clomiphene and cyclophenyl at the Karl-Marx-Stadt District Hospital Gynecologic Clinic].

We report on 39 barren patients suffering from anovulatory cycle disturbances or corpus luteum inadequacies. Them were given treatment in the Gynecological Hospital of Karl-Marx-Stadt County at Karl-Marx-Stadt in the past few years and were administered Clomiphendihydrogencitrate or Cyclophenyl to provoke ovulation. Before and during therapy, the total of estrogenes and pregnanediol were determined in the 24h-urine every two days. The clinical data and the results of the hormone investigations were analysed, and an attempt was made to derive a forecast from the estrogene and pregnanediol secretion so as to be able to form an opinion on the positive or negative result of the treatment.

[Precanceroses of the cervix uteri--contribution to diagnosis within the scope of pregnancy interruption].

The report deals with the early diagnosis of the carcinoma of the cervix under the aspect of the induced abortion between 1972 and 1974. In 5313 cases of induced abortion, we diagnosed 13 carcinoma of the cervix of the group 0 (0,26%). The medical treatment was carried out by means of conization and hysterectomy, respectively. The significance of the searching for the carcinoma during the pregnancy is mentioned.

Urine protein analysis by gel electrophoresis and laser densitometry after chemotherapy in pediatric cancer patients.

A common side effect of chemotherapy is reversible or nonreversible nephrotoxicity. SDS polyacrylamide gel electrophoresis combined with laser densitometry was evaluated as a suitable method to analyze pathologic urine proteins. A total of 52 pediatric patients were followed during and 63 patients were followed after therapy. During therapy renal damage was recorded in 43% of the leukemia patients, in 56% of nephroblastoma patients, and 75% of patients with other tumors. Three or more months after therapy pathologic patterns were seen in 25% of acute lymphoblastic leukemia patients, in 35% of patients with nephroblastoma, and in 62% of other patients. Patients with persistent complete tubular proteinuria and mixed glomerular/tubular proteinuria were found to have a high risk for irreversible renal damage and should be controlled periodically. This method permits a rapid and reliable analysis of urine proteins and is suitable for follow-up tests of renal function during and after chemotherapy.

Determination of extra renal clearance and generation rate of beta2-microglobulin in hemodialysis patients using a kinetic model.

Beta-2-microglobulin (beta2-m) has been proposed as a marker of middle molecules to assess one aspect of the efficacy of dialysis. Until now, few data have been published about extra renal (metabolic) clearance and generation rates of beta2-microglobulin, which are necessary for calculation of total clearance and mass removal of beta2-m in hemodialysis patients. We have developed a simple method to derive extra renal clearance and generation rates of beta2-m by measuring the pre and post dialysis blood concentrations of beta2-m using kinetic modeling. Ten stable hemodialysis patients were included in this study. Pre and post dialysis concentrations of beta2-m were measured during dialysis with low flux dialyzers (F6 HPS) and after 10 days switching to high flux dialyzers (F60S or Superflux). With a validated two pool model, the generation rate of beta2-m can be determined if extra renal clearance is known. Assuming the generation rate of beta2-m to be constant in each patient, the computer reiterated the calculation of extra renal clearance until the calculated generation rate was equal for both the low flux and the high flux dialyzer. Extra renal clearance was found to be between 1.97 and 4.11 ml/min (average, 3.2 ml/min). Generation rate was found in a rather narrow range between 1.63 and 2.56 mg/kg per day (average, 2.09 mg/kg per day). There was no correlation between extra renal clearance and generation rates. With this simple method, extra renal clearance and generation rates of beta2-m can be determined using data by switching hemodialysis patients from impermeable to permeable membranes.
