[Therapy of progressive scleroderma with gamma-interferon].

In an open study conducted over 6 months, gamma interferon was administered to 12 patients suffering from progressive systemic sclerosis. The dosage used was three injections, each of 50 micrograms s.c. in the first 2 weeks, followed by 50 micrograms twice a week. All patients fulfilled the criteria required for classification as type II in the ADF system or the major criterion of the ARA (proximal sclerosis). Therapeutic effects were noted as a decrease in of skin thickness (10/12 = 83%), improvement in muscle and joint pains (5/8 = 63%) and reduction of the procollagen-type III peptide in serum by more than 20% (8/12 = 66%). No effects on fibrosis or dysfunction of internal organs were seen. Disease progression did not occur in any of the patients. Side-effects of gamma interferon were noted in 4 patients, in the form of slightly elevated body temperature and transient influenza-like symptoms, but these occurred only at the beginning of therapy and soon passed.

[Primary cutaneous leiomyosarcoma].

Primary leiomyosarcomas of the superficial soft tissues are very rare neoplasms. Because of the different biological behaviour, they should be divided into cutaneous and subcutaneous tumours. Differential diagnosis includes leiomyomas, skin metastases from extracutaneous leiomyosarcomas, and other spindle-cell tumours of the skin. Ultrastructural and immunohistochemical investigations are useful when light microscopic features do not allow an unequivocal diagnosis. Although local recurrences develop in almost half the patients with cutaneous leiomyosarcomas, the prognosis is predominantly good. In contrast, metastatic spread is more common in patients with subcutaneous leiomyosarcomas. Treatment for superficial leiomyosarcomas consists in wide excision. We report on a 76-year-old women with a local recurrence of a primary cutaneous leiomyosarcoma.

[Therapy of bullous pemphigoid with erythromycin].

Bullous pemphigoid is the most common autoimmune mediated bullous disease in men. Erythromycin combined with a low dose methylprednisolone was tested in eleven patients in a prospective study. A historical collective of the last 33 patients treated before this study was started served as the control group. The duration of hospitalization as an expression of therapeutic response, but also of lowered side effects dropped down from 43 to 33 days in the erythromycin treated group. The study further shows a responder rate of about two thirds of the patients, one third, however, required a high dose regimen of corticosteroids in order to ameliorate. From this could be concluded, that two different subtypes of bullous pemphigoid exist concerning their therapeutic response.

Radioligand binding characteristics of beta 2-adrenoceptors of cultured melanoma cells.

The existence of beta-adrenoceptors on intact cells of the human malignant melanoma cell line A-375 was investigated using the binding properties of the tritiated radioligand (-)-[3H]CGP-12177, a hydrophilic non-selective beta-adrenoceptor antagonist. Displacement experiments of the radioligand from its binding site were performed with antagonists and agonists to determine the beta-adrenoceptor subtype selectivity. The binding of (-)-[3H]CGP-12177 was saturable, of high affinity (KD = 0.025 nmol/l, n = 12) and was rapid and readily reversible. The maximal number of binding sites (Bmax) was 33.5 +/- 1.9 fmol/10(7) cells or 2018 +/- 114 receptors per cell. beta-adrenoceptor antagonists inhibited binding of the radioligand with monophasic displacement curves. IC50 values were (nmol/l): propranolol (non-selective) 2.82, alprenolol (non-selective) 2.0, ICI 118,551 (beta 2-selective) 3.5 and bisoprolol (beta 1-selective) 2200. Agonists inhibited binding in the order of potency of isoprenaline greater than adrenaline greater than noradrenaline. It is concluded that cells of the melanoma cell line A-375 contain a homogeneous population of beta 2-adrenoceptors.

[L-tryptophan-induced eosinophilia-myalgia syndrome with features of diffuse fasciitis with eosinophilia].

Two female patients developed localized scleroderma on the trunk and the thighs after oral ingestion of L-tryptophan for some years. Both patients reported acute progressive myalgia and weakness of the proximal parts of the extremities. On laboratory evaluation, the leucocyte count was approximately 20,000/mm3, with 38% blood eosinophils in one patient and 53% in the other. The ESR was slightly elevated; electrophoresis and muscle enzymes were normal. Skin and muscle biopsies revealed characteristic features of diffuse fasciitis with eosinophilia. High-dose glucocorticoid therapy resulted in a rapid normalization of the ESR and blood eosinophilia, whereas the scleroderma showed little improvement. The diffuse edema observed in one patient receded within a few days. A correlation between oral ingestion of L-tryptophan and the eosinophilia-myalgia syndrome has been reported recently, and the present case reports must be discussed in the light of this observation. Both patients developed a tryptophan-induced scleroderma-like illness resembling diffuse fasciitis with eosinophilia (Shulman's syndrome).

[Skin and genital infections caused by gram-negative bacteria].

We discuss the classification of gram-negative bacteria as well as the most common dermatological disorders and reactions due to these germs. In addition, we give some information both on the bacteriological identification and the therapeutical concepts in these dermatoses.

[Experiences with percutaneous anesthesia using a lidocaine-prilocaine cream (EMLA 5% cream)].

Using an percutaneous anesthetic Lidocaine ointment (EMLA cream 5%), we treated patients with mollusca contagiosa, condylomata acuminata, and verrucae plantaris by superficial curettage or electrocaustic surgery. The same presurgical procedure was applied in taking biopsies from the glans penis. The analgesia achieved was complete in all patients with mollusca contagiosa, condylomata acuminata, and penal biopsies. In those with verrucae plantaris, the anesthesia was incomplete but tolerable, also in children. We discuss the pharmacological effects of and further indications for this new percutaneous anesthetic ointment.

[Primary neuroendocrine Merkel cell carcinoma of the skin].

A 78-year-old woman noticed a nodule on the dorsal aspect of the left lower leg which caused no symptoms but gradually increased in size. The raised, brown-red nodule, about 1.5 cm in diameter, was widely excised because malignancy was suspected. Light and electronmicroscopy revealed changes typical of Merkel-cell carcinoma. Immunohistochemistry demonstrated neuron-specific enolase and cytokeratin. No metastases were found. Together with Langerhans cells and melanocytes, Merkel cells belong to the three non-keratinocyte cell types of the epidermis.

[Clofazimine--therapeutic alternative in necrobiosis lipoidica and granuloma anulare].

Twenty patients, ten suffering from disseminated granuloma anulare and ten from necrobiosis lipoidica, were treated with clofazimine 200 mg p.o. daily. Six patients in each group (60%) responded to this regimen, and three of the responders in each group achieved complete remission of the dermatosis. In eight patients (40%) no improvement at all was observed. All the patients treated had reddening of the skin, but this was reversible after the end of therapy, as were the other side-effects, i.e. diarrhoea and dryness of the skin, which were not experienced by all patients.

[Diagnosis of urethritis in general practice. Methods and possibilities].

The diagnosis of sexually transmitted diseases can be exactly defined nowadays, since the specific germs responsible for urethritis have been microbiologically characterized. The term "unspecific urethritis", therefore, should be replaced or completed by the identified specific germ. A short description is given of the various techniques (native procedures, staining, culture, and technical methods like Elisa or immunofluorescence), which can be performed in dermatologic practises. In addition, a survey of the urethritis germs isolated at the Hamburg Department of Dermatology is presented.

[Osteosis cutis multiplex, an organoid nevus?].

We report on a 71-year-old woman suffering from histologically proved osteosis cutis multiplex (Arzt's disease). The differential diagnosis includes multiple milia, closed comedones and hidradenomas. We discuss various etiopathologic concepts and a nevogenic origin in particular.

[Linear circumscribed scleroderma--case report. Classification and differentiation of circumscribed scleroderma].

In linear scleroderma, a rare form of circumscribed scleroderma, the lesions are arranged in a band-shaped linear distribution and both the superficial and the deeper layers of the skin are attached to the underlying structures. This disease must principally be differentiated from eosinophilic fasciitis (Shulman's syndrome). On the basis of the clinical and laboratory data, systemic scleroderma can be excluded in nearly all the cases. Nevertheless, patients with linear scleroderma might develop systemic scleroderma or other systemic diseases of the connective tissue even after years. As a consequence, thorough physical examination as well as laboratory evaluation is necessary over a long follow-up period. We introduce a new classification of circumscribed scleroderma.

[Etretinate therapy in solitary keratoacanthomas].

In an open clinical trial, 5 patients suffering from solitary centrofacial keratoacanthoma were treated with etretinate. The dosage was 1,5 mg/kg body-weight. All patients showed complete healing after 4-5 weeks of therapy. There were no recurrences within 3 months after therapy.
