A new born girl perinatally suffered from a generalized blistering disease of the skin. By means of histological, in particular ultrastructural methods the diagnosis of Epidermolysis bullosa herpetiformis Dowling Meara could be confirmed in the first weeks of life. The Dowling Meara type is one of the epidermal variants of this group of inherited blistering diseases with a relative benign course. The early diagnosis is helpful in the prognostic assessment of the disease and in the medical and genetic advice for the parents.
For several years cyclosporin A has been used as an immunosuppressive agent in transplantation medicine and to avoid graft-versus-host disease. Some case reports concerning beneficial effects on different dermatoses have aroused speculation about the application of this drug in dermatology. The mode of action, a summary of the dermatologic case reports, and a discussion of the side effects of cyclosporin A are presented in this article.
Using the electron microscopic technique of negative staining, viruses of the herpes group were detected in leukoplakic lesions on the tongue of a bisexual, HTLV-III-/LAV-infected male. Oral "hairy" leukoplakia was diagnosed, which was the first clinical symptom of on HTLV-III/LAV infection in this patient.
Ecthyma contagiosum is caused by a member of the parapoxvirus group, the so-called orf virus. The typical feature of the disease is described, and some of its peculiarities are illustrated by means of a case report.
Fibroblast are responsible for the synthesis of the structural proteins of the connective tissue. A further property of these cells, their migratory ability, could be analyzed in the last years. A special form of migration is chemotaxis, which can be quantitatively measured in a modified Boyden chamber in-vitro. Using this method chemoattractive substances could be characterized, which are able to stimulate fibroblasts and tumorcells to chemotactic migration. Furthermore it could be proved, that benign and transformed cell lines react in a different manner towards these chemoattractive substances. The in-vitro results allow some hypotheses about both fibroblast migration in wound healing or chronic inflammation, and the mechanisms of tumor cell evasion in the tumor surrounding tissue or the metastasizing process in other organs.
Using human umbilical cord endothelial cell cultures and a modified 3HTdR uptake technique, endothelial cell cytotoxic activity (ECA) has been demonstrated in sera of 95/130 patients with progressive systemic sclerosis (PSS), 14/20 patients with Raynaud syndrome (RS), 52/153 rheumatoid arthritis (RA), and 47/113 systemic lupus erythematosus (SLE) sera. ECA could be enriched by gel filtration from PSS sera in a molecular weight range of 5 k daltons. ECA was partially associated with serum proteins, mainly in the albumin containing fraction, albeit at a lower level of activity. In PSS, no relationship of ECA to the type of skin involvement was observed. ECA appears to be a low molecular weight mediator of, as yet, unknown origin.
Skin cancer is the most common human carcinoma. There has been a dramatic increase of the incidence over the last decades. This trend is very likely caused by the increased UV-light exposition of the population. Whereas basalioma and spinalioma as the most frequent types of skin cancer have a low mortality, the bad prognosis of malignant melanoma is the reason for the 100% increase of the mortality rate over the last ten years. Cancer prophylaxis means the early detection of precancerous or suspect lesions. Popular information on the risk and a more generous indication for excision are the best instruments to lower the mortality of skin cancer.

Chancroid (ulcus molle) was treated in 7 male patients with enoxacin (2 X 400 mg/die). Microbiological and clinical examinations proved complete healing in all cases after 7 to 12 days. No side effects were mentioned. Enoxacin seems to be an excellent alternative treatment for this sexually transmitted disease.
Diffuse fasciitis with eosinophilia (DFE) is a well described syndrome, among the connective tissue disorders. However, DFE is not commonly accepted as an own entity, because of its rare evolvement in scleroderma and the often indistinguishable histological pattern of both diseases. Here we report on the association of DFE and morphea in two patients and an additional lichen sclerosus in one of them. This points to a close relationship of DFE and other connective tissue diseases.
Two elderly individuals suffering from acute recurrent dermatitis of light-exposed skin between spring and autumn were shown to be allergic to feverfew (Tanacetum parthenium) of the Compositae family. Patch tests revealed not only strong reactions to the plant and its constituent parthenolide but also a number of cross-reactions to related species. The source of this contact allergy was dried airborne plant particles that attached themselves to the skin during long periods outdoors. Differentiation between patterns of airborne contact dermatitis and persistent light reaction is possible by several distinct observations, i.e., in patients with airborne contact dermatitis: a) Patch tests with Compositae plant extracts and constituents will be positive. b) There will be no skin lesions in winter. c) Skin regions that are usually not impaired during light exposure will also be involved: the upper eyelids, the retroauricular region, the shady area beneath the chin.
On the basis of medical history and clinical findings of two affected soldiers, we discuss Strandberg syndrome with regard to clinical symptomatology, pathogenesis, hereditary behaviour, differential diagnosis and prognosis. Electron optical as well as histological studies and HLA-type are presented.
Several pharmaceutics on the market contain polyvinylpyrrolidone (PVP). As the high-polymeric parts of the PVP molecules cannot be eliminated after parenteral or subcutaneous application, they accumulate in the organism. PVP residues in the dermis cause a considerable variability of skin changes, ranging from deposits nearly without reactions up to swelling, papules, granulomas and pseudotumors.
SV40 transformants of human embryo fibroblasts showed an enhanced chemotactic response to fibronectin and conditioned medium of fibroblasts in comparison to the non-transformed cells. An even higher chemotactic response was characteristic for a malignant human fibrosarcoma cell line, whereas cells derived from low-grade malignant dermatofibrosarcoma showed a normal response in the Boyden chamber assay. The high stimulation of chemotaxis is paralleled by a reduced synthesis and an altered deposition of fibronectin in the pericellular matrix of these cells. Both high chemotactic response and lack of deposition of fibronectin may play a role in tissue invasion and formation of metastasis by malignant tumors.
For comparative studies on the migratory potential we screened fibroblast strains derived from mucopolysaccharidosis (MPS) patients regarding their differential response to chemotactic stimuli and analysed their production of extracellular matrix components. Indirect immunofluorescence staining of MPS-fibroblasts showed the same distribution of type I and type III collagen and of fibronectin as in controls. Biochemical quantification of type I and type III collagen demonstrated an unaltered ratio of these collagen types, although the total amount of newly synthesized collagens was slightly reduced in fibroblasts from MPS patients. Whereas the synthesis of major extracellular matrix components was close to normal, the response of the MPS cells to chemotactic stimuli was greatly affected. Chemotactic migration was improved when fibroblasts were pretreated with medium conditioned by normal fibroblasts, although they never reached normal levels.
In vitro fibroblast chemotaxis was studied in the presence of synthetic leukotriene B4 (LTB4) and of supernatants from ionophore-stimulated lymphocytes, monocytes, and basophils (LMB-S). LTB4 and LMB-S induced a dose-related, directed migration of human and rat embryonic fibroblasts. By checkerboard analysis, this response was chemotactic and not chemokinetic. Optimal migration toward LTB4 occurred at 10(-8) M concentrations, while higher concentrations were inhibitory. The LMB-S contained two types of fibroblast chemotactic factors, one that corresponded to LTB4 and one additional, heat labile, non-dialyzable substance of greater than 10 kilodaltons. LTB4 and LMB-S were inactive against transformed human connective tissue cells (HT 1080, McCoy) while these cells moved well in response to conditioned medium derived from confluent fibroblast monolayer cultures. These results suggest that LTB4 is a specific chemoattractant for fibroblasts and that it acts in concert with other chemotactic factors derived from inflammatory leukocytes to regulate the influx of fibroblasts to tissue sites.
Vitamin A and several other retinoids were added to fibroblast cultures in order to study possible alterations in biochemical properties and cellular responsiveness. The proliferation of cells was inhibited as the concentration of retinoids increased from 10(-9) to 10(-5) mol/l. Synthesis of non-collagenous proteins and production of both type I and type III collagen were decreased. The onset of type III collagen synthesis by tendon fibroblasts in culture was delayed. Furthermore, the chemotactic response of fibroblasts to fibroblast-conditioned medium was markedly reduced in the presence of retinoids (10(-6) to 10(-12) mol/l).
