Rupture of supra-aortic neck arteries due to lesions caused by tracheal tubes.

Haemorrhages from the common carotid or innominate artery caused by tracheal tubes are rare but dramatic complications of head and neck surgery. Patients with metal tracheostomy cannulas or patients in the phase of wound healing after radical tumour resections of the pharynx and larynx run a greater risk. Following extensive tumour resections and radiation of the upper respiratory and digestive tracts in 680 patients, acute bleeding of the larger neck arteries occurred in 18 patients. In four of these cases, fistulae developed in the innominate or common carotid artery. These ruptures were caused by both plastic and metal tracheal tubes.

Early auditory evoked potentials (EAEPs) in the rabbit. Normative data and effects of lesions in the cerebello-pontine angle.

Early auditory evoked potentials (EAEPs) were measured in rabbits. After establishing normative data an operative method was developed to expose the cerebello-pontine angle. Compression of the VIIIth nerve and a reduction of blood supply to the cochlea via the labyrinthine artery altered the EAEP systematically. In all cases of an VIIIth nerve lesion wave 1 continued to be elicited whereas waves 2-5 disappeared. Circulatory disturbances had an effect upon wave 1. The results indicate that the generator of wave 1 is the unit cochlea/acoustic nerve and of waves 2-5 the auditory pathway within the brain stem. The resultant data help to understand wave alterations in patients with acoustic neuromas.

[Preoperative intravenous and intra-arterial cytostatic therapy (VMBP) in carcinoma of the head and neck].

Two groups of patients with carcinomas of the oropharynx and the oral cavity were selected for praeoperative cytostatic therapy. Vincristine, Methotrexate, Bleomycin and Cisplatin were administered intravenously and intra-arterially (VMBP). The remission rates of the tumours (complete and partial remission) were, for intra-arterial application, 81%, and for intravenous application, 66.7%, indicating a possible superiority of intra-arterial therapy. In 10 intra-arterial (47.6%) and in 8 intravenous cases (38.1%), a complete clinical remission was accomplished. This was confirmed by the histological examination of the tumours, where no tumour cells could be recognized.

[Ambulatory combination therapy with cisplatin, bleomycin (DDP-BLM) in recurrence and metastasis of malignant tumors of the head and neck].

16 patients with recurrent or metastatic carcinomas of the upper respiratory and digestive tract underwent a total of 55 courses of chemotherapy with Cisplatin and Bleomycin. The cytostatic treatment was performed under outpatient conditions. 0.75 mg./kg. Cisplatin was administered at intervals of 5-8 weeks. In addition, 15 mg. Bleomycin was administered weekly. Hydratation and diuresis were forced by 1000 ml. 0,9% NaCl solution and 1000 ml. levulose 5% with potassium substitution and 250 ml. mannitol 10% after an initial injection of 2 ml. Furosemid. The treatment was generally well tolerated. A nausea lasting 1-3 days was observed after Cisplatin application. Remission of tumours, stopping of tumour progression and a reduction of pain was achieved. The "performance status" was maintained. In 6 cases there was no effect.

Ambulatory Care↗

[Early auditory evoked potentials (EAEP) following cochlear nerve lesions in the cerebellopontile angle. An animal experiment study].

After exposure of the cerebello-pontine angle, various lesions were applied to the cochlear nerve in 20 rabbits. Changes of the EAEP were registered at the same time. Lesions proximal to the internal auditory meatus hardly exercised any influence on wave I. Waves II-V disappeared immediately after the trauma, Partial or complete recovery was observed, depending on the extension of the nerve lesion. Dissection or crushing of the cochlear nerve resulted in permanent disappearance of the waves II-V. Wave I often showed a slight reduction of amplitude, combined with a broadening of its duration and increase in latency. Lesions of minor intensity produced a transient disappearance of waves II-V. Recovery of these waves occurred within the following 2-3 days.

[The remission of extensive cancers of the mouth and oropharynx with intra-arterial polychemotherapy with vincristine, methotrexate, bleomycin, cisplatin (VMBP)].

Initial intra-arterial chemotherapy was performed during a therapy course in 21 patients with squamous cell carcinoma of the oral cavity and oropharynx, including chemotherapy, operation, and radiation. A combination of Vincristine, Methotrexate, Bleomycin, and Cisplatin (VMBP) was administered. The aim of this initial therapy was to reduce the tumour mass. 10 patients showed complete clinical remission. In 5 of these no malignant cells could be found on histological examination of the primary site of the tumour.

[Experiences with intratumor application of bleomycin-oil suspension (BOS) in cancers of the mouth, oropharynx and maxillary sinus].

Bleomycin can be administered intravenously, intramuscularly and also locally into the tumor. Bleomycin in oil suspension (BOS) remains longer in the tumor-region with higher levels. In 6 patients with squamous cell carcinomas of the mouth, oropharynx and maxillary sinus, the intratumorous BOS-therapy was performed. The maximum dosage given was 180 mg. The results included: Progression (1), no-response (2), minor response (2), partial remission (1).

[Computed tomography and ultrasound: comparison with intraoperative findings of large metastases in the neck (author's transl)].

The evaluation of soft tissue lesions in cases with extensive metastases, in the neck is of great importance. The therapeutic concept will depend upon the size distribution and infiltration of the metastases into the structures of the lateral neck. The limiting factors include the great vessels of the neck, the deep neck muscles, the base of the skull, the mediastinum and the proximal parts of trachea and oesophagus. The timing of therapy (operation, radiation and cytostatic therapy) can be planned according to these factors. Computed tomography and ultrasound provide the information about the soft tissues of the neck. Spread, size and site of infiltration can be fairly estimated as shown in 25 patients which have been operated on.

[Rupture, emergency ligature and elective ligature of the carotid artery].

680 patients underwent surgery with radical neck dissection and tumour resection because of malignant diseases of the upper respiratory and digestive tract. Further therapy was pre- and/or postoperative radiation and in some cases preoperative cytostatic therapy. Carotid "blow out" occurred in 18 patients (2,6%) because of fistula formation, bad wound healing and tumour recurrence. Emergency ligation of the common carotid artery or internal carotid artery caused no neurological casualties in 70% of the patients. 4 patients underwent elective carotid ligation. Here, too, no neurological signs were seen. In these patients sufficient collateral perfusion was proved by angiography with balloonocclusion under simultaneous EEG-registration and measurement of the brain perfusion with 133 xenon.

[Intraarterial cytostatic therapy of head and neck: operative and cytostatic complications and side effects (author's transl)].

In the past 30 months 51 patients with squamous cell carcinomas in head and neck were treated by intraarterial application of cytostatic drugs. A catheter was placed in the external carotid artery below of the vessel supplying the tumor. For placement of the catheter the temporal and the superior thyroid artery were used. The right position of the catheter tip is important for achieving a high concentration of the cytostatic drugs in the tumor region. The wrong positioning of the catheter tip in the carotid bifurcation leads to perfusion or embolization in the internal carotid system. In one case we noted a persistent hemiplegia in three other cases temporary hemiplegia, due to perfusion of the brain with highly concentrated cytostatics. Postoperative bleeding of the arterial stump was noted in 3 cases. Local side effects of the cytostatics were mucositis, loss of hair and hyperpigmentation. The systemic side effects correlated with those after intravenous injection.

[Reactive immunologic changes during intraarterial methotrexate therapy in patients with carcinoma of the oropharynx (author's transl)].

The immunity deficiency as colateral effect of intra arterial therapy with methotrexate in oropharynx carcinoma was measured by lymphocyte stimulation tests. Ten patients taken in two groups were treated with different doses of methotrexate and citrovorum factor. In determined stages of the cellular cycle an extensive reactive trauma to the lymphocytes was detected. This is not the case in the usual measurement of the number of lymphocytes, leucocytes and thrombocytes. This method seems to be appropriate in order to establish the doses, period of time and frequency for application of the intra arterial perfusion with methotrexate.

[Variations in middle-ear producen during a mask-ventilation (author's transl)].

In 30 patients ageing from 19-60 years dueing forced mask-ventilation, the pressures present in the epipharynx and the middle ear were measured. In 12 patients results raising pressure of the middle ear which has a corresponding level in the epipharynx accompanied by a time delay. It was possible to measure peaks in pressure of 100 cm H2O which however, by increased forced ventilation could produce higher results. Disturbances of the movable structures of the middle ear, due to increased forced ventilation appears probable.

[Changes caused by earmoulds in shape of the formation of the external auditory meatus (author's transl)].

The lateral part of the external auditory meatus is widened by hearing aids. This was found when comparing angles, diameters and volumes of the external auditory meatus in two groups of children. One group of 56 children had bilateral hearing aids, the other group of 48 children of the same age had healthy normal ears and did not use hearing aids. The widening process is increased with the length of time that a hearing aid is used.
