Coronary arteriography using balloon occlusion of the aortic root in infants with transposition of the great arteries.

A simple technique of coronary arteriography using balloon occlusion of the aortic root is described. The technique has greatly helped in the selection of the arterial switch operation for infants with transposition of the great arteries (TGA) in whom accurate recognition of the anatomy of the coronary arteries is important and yet selective coronary arteriography is technically difficult. The technique has been used in 13 consecutive infants and has proved very helpful in 12, with no serious adverse effects.

[Cor triatriatum associated with coarctation of the aorta and VSD].

A case of cor triatriatum associated with coarctation of the aorta (CoA) and VSD is reported. A 4-month-old boy underwent subclavian flap aortoplasty and ligation of PDA with the diagnosis of CoA and VSD. At the age of 1 year and 2 months, echocardiography revealed an abnormal subdividing membrane in the left atrium. Under the definite diagnosis of cor triatriatum (Lucas & Schmidt type I B1) and VSD, complete removal of the abnormal septum, patch closure of ASD and VSD was performed with the aid of cardiopulmonary bypass, blood cardioplegia and topical cardiac cooling. Postoperative course was uneventful and the patient was discharged from the hospital on the 38th postoperative day. An association of cor triatriatum, VSD, and coarctation of the aorta is very rare, and to our knowledge, the case presented here seems to be the first report of successful total correction by a staged approach in the literature.

[Early and late results of surgical treatment of coarctation of the aorta in infants less than six months of life--including the propriety of pulmonary artery banding].

During an 8-year period, 53 infants less than six months of life underwent repair of coarctation of the aorta (CoA). Preoperatively 58 percent of patients received intensive circulatory and/or respiratory care. The diagnosis was established by echocardiography and counter current aortography via a radial artery in the majority of recent cases and the surgical relief of CoA was effected within 24 hours of the diagnosis. The last 51 patients were managed under the policy of simple repair of the CoA in patients with significant VSD (Group II), with the addition of pulmonary artery banding for patients with complex cardiac lesions with VSD (Group III) or for patients with incremental risk factors in Group II such as prematurity of body weight of less than 1500 gm, other organ anomalies requiring major operation in neonate, pulmonary complications or severe malnutrition. There was no intraoperative death, one (2%) postoperative and five (10%) late deaths among the lost 51 patients. Only one (2%) showed sign of recurrence among 46 patients followed for 16-112 (mean 47) months.(ABSTRACT TRUNCATED AT 250 WORDS)

[Functional hemisphere differences in auditory and tactile recognition in aged people].

The effects of aging on hemispheric specialization in verbal and visuo-spatial functions were examined by dichotic listening and tactile recognition tests. In the first experiment, thirty aged (65-91 year) intact subjects and university students were tested by a dichotic listening tape with pairs of two-syllable meaningful words. The results revealed an overall decline of scores in the aged subjects but there was no interaction between subject and the degree of right ear advantage. In the second experiment, subjects were asked to judged whether successively presented nonsense tactile stimuli to each hand were identical or not. Reaction times and accuracy rates of each hand of the aged subjects were inferior to those of the control subjects. However, no interaction was found between hand and subject group. Both experiments show that there is no evidence to suggest any selective deterioration or specialization of hemispheric function in the aged people, though overall performance level declines with age, which may be caused by short-term memory deficit.

Event-related potentials and reaction times as measures of hemispheric differences for physical and semantic Kanji matching.

Hemispheric differences associated with Physical Identity Matching and Semantic Classification of Japanese characters (Kanji) were examined using visual event-related potentials and behavioural measures. Event-related potentials to the Kanji during matching tasks revealed hemispheric differences in the later components. Right hemisphere responses were larger than those or the left. The interaction of Types of Matches by Hemispheres was not significant. This interaction however was significant with the behavioural measure. On Semantic Classification Matching reaction times for right-visual-field Kanji were faster than those of the left field while no visual field difference was found for Physical Identity Matching. The sensitivity of both measures as indicators of laterality is discussed from the point of view of levels of processing.

Surgical treatment of idiopathic hypertrophic subaortic stenosis in children.

Five children with idiopathic hypertrophic subaortic stenosis (IHSS) were operated upon during the past 7 years and all survived. Their ages ranged from 4 years and 5 months to 14 years (mean 8 years and 5 months). The operations performed were 1 right ventricular myectomy and 4 transaortic left ventricular myotomies. Whether surgery performed in childhood would change the natural history or not is still unknown, however, according to our results and many available reports, we have so far reached the following conclusions; 1. Transaortic left ventrlcular myotomy or myectomy is a safe and effective operation for localized muscular subaortic stenosis in children. 2. Because the disease is progressive, operation in childhood may be worth to be considered, when the resting or provocative pressure gradient is excessive. 3. For those who have a marked increase in left ventricular outflow pressure gradient by the provocation, operation would be considered even though the gradient at rest is less than 50 mmHg.

[Changes in the osmotic fragility of erythrocyte membrane in morphine- and phenobarbital-dependent rats (author's transl)].

This is apparently the first attempt to elucidate the relationship between drug dependence and the osmotic fragility of erythrocyte membrane. The osmotic fragility was measured using a coil planet centrifuge (CPC) system. Utilizing the drug-admixed food (DAF) method, rats were made drug-dependent. The osmotic fragility of morphine-dependent rats was significantly enhanced, compared with that of naive rats. By withdrawing or treating the rats with levallorphan, the osmotic fragility was enhanced more than in the morphine-dependent state. When the morphine-withdrawal rats were again given the morphine-admixed food, the osmotic fragility recovered to the morphine-dependent level. The osmotic fragility of phenobarbital-dependent rats was significantly decreased, compared with that of naive rats. On the contrary, in the phenobarbital-withdrawal rats, the osmotic fragility was significantly enhanced, compared with that of the phenobarbital-dependent rats. With re-treatment of phenobarbital-admixed food, the osmotic fragility was recovered to the levels seen in the phenobarbital-dependent rats. Abstinence signs including weight loss, decrease in food and water intake, adrenal hypertrophy etc., were observed during morphine or phenobarbital withdrawal. The effects of food or water deprivation and application of ACTH on the osmotic fragility were then studied and we found that the osmotic fragility was enhanced with these treatments. These results suggest that enhancement of osmotic fragility during withdrawal of these drugs is partly influenced by these treatments.
