Community support for needle exchange programs and pharmacy sale of syringes: a household survey in Baltimore, Maryland.

In October 1995, community attitudes toward needle exchange programs were assessed in Baltimore, Maryland. Household interviews were conducted with a random sample of residents living within six contiguous census tracts. Multivariate logistic regression was used to determine which factors were independently associated with acceptance of needle exchange programs. Of 274 eligible respondents contacted, 138 (50%) completed the interview. Respondents were statistically similar to the 1990 census population by income but were more likely to be female, black, between the ages of 35 and 44 years, and to have attended college or be a college graduate. Although 72% of respondents thought needle exchange programs would attract injection drug users to the neighborhood, 65% favored needle exchange, and 47% favored selling needles in a pharmacy without a prescription. Factors independently associated with acceptance of needle exchange programs included the perceptions that needle exchange programs decrease the number of discarded needles on the street, that needle exchange programs do not encourage a person's injection drug use, and that needle exchange programs decrease HIV incidence. Despite concern about attracting injection drug users to the neighborhood, support for needle exchange programs was high.

[Venous insufficiency following lack of a healthy superficial femoral vein and popliteal vein].

Using the superficial femoral yein in ultimate arterial reconstructive surgery we have not found distinct signs of venous insufficiency concerning the clinical status, the strain gauge plethysmography and the measurement of limb circumference. For special indications the use of the superficial femoral vein presents a valuable addition to our armamentarium of arterial substitutes.

[Mastication rehabilitation via dysgnathia surgery and simultaneous implantation].

Within the last 2 years 26 endosseous implants have been used in combination with orthognathic surgery in the maxilla and mandible. The results show, that endosseous implants can be used simultaneously with orthognathic surgery. This helps to treat dysfunctions often seen in partially edentulous patients that undergo orthognathic surgery and at the same time reduce the risk of relapse by achieving stable occlusion.

[Caval interruption operations for prevention of lung embolism].

Interruption of the inferior vena cava by an umbrella-filter, clip or Green-field-filter has been performed in more than 400 patients during the past 14 years. Overall hospital mortality was 1% by clip application and 7% by Mobin-Uddin-umbrella. The indication for this method is limited to those patients suffering from recurrent pulmonary embolism, despite correct anticoagulation, and if thrombectomy is insufficient or contraindicated. Inferior vena cava patency was objectively assessed in 121 patients by cavogramm. Caval patency of 95% in those with Greenfield-filter and clip was significancy greater than the 15% patients seen with Mobin-Uddin devices. Venous status was objectively assessed in 45 patients by intracaval measuring the pressure in front of and behind the stenosis.

[Azathioprine and its side effects].

The clinical signs and symptoms of a patient induced us to consider critically the so far known side effects of azathioprine. The literature concerning 643 patients treated with azathioprine reports on reactions as changes in blood count, gastro-intestinal complaints, intrahepatic cholestasis, acute pancreatitis, and leukemia. Irreversible side effects occurred in four cases.

Results of a ten year period of reconstructive surgery for renovascular disease.

During a 10-year period 242 patients underwent 259 renal artery reconstruction for correction of renovascular hypertension or preservation of renal function. Bilateral stenosis was found in 26 patients (10.7%). Twenty-five of the 259 renal arteries were totally occluded. Stenosis was atheroslcerotic in 147 patients, fibrodysplastic in 88 and of other origin in 7 patients. The operative procedures included thrombendarterectomy with or without patch plasty in 105 patients, resection and end-to-end-anastomosis in 68, reimplantation in 34, vein graft interposition in 32, and patch graft angioplasty in 14 patients.--Overall hospital mortality was 4.1%. Whereas there were no deaths in fibrodysplastic disease, it was 6.8% in atherosclerosis. The late mortality of 185 patients followed was 9.2%, stroke and myocardial infarction being the most common causes of death. Postoperatively, blood pressure was normal in 36% of patients with atherosclerosis and in 70% of those with fibrodysplastic disease. Blood pressure improved in 53% of the patients with fibrodysplastic disease. These relationships did not change significantly 2, 4, and 6 years after reconstruction. The results indicate the favorable prognosis of renal artery reconstruction in fibrodysplastic stenosis and the limitations of surgery in atherosclerosis in a non-selected group of patients. Although the introduction of percutaneous intraluminal dilatation widens the therapeutic possibilities for renovascular hypertension, exact indications and intensive after-care are still mandatory.
