[Evaluation of multislice dynamic MR imaging of the whole liver by inversion recovery snap shot FLASH method].

We acquired dynamic images over the whole liver by inversion recovery snap shot FLASH method after a bolus intravenous injection of Gadolinium-DTPA. Each nodule of hepatoma in the liver showed early enhancement and gradually turned to show low intensity. In two cases of hepatoma, small intrahepatic metastases, which were not detected by US, CT and spin-echo image of MRI, were suspected as high intensity nodules on early phase. Also recurrent areas after TAE were enhanced on early phase. This method is practical for improving the detection of lesions and is useful for evaluating the recurrence after TAE.

An immunoenzymometric assay for a CA125-like antigen, CA602.

An immunoenzymometric assay (IEMA) for a new CA125-like antigen, CA602, was developed. Five monoclonal antibodies raised against a human ovarian carcinoma cell line could detect their respective antigens in the sera of ovarian carcinoma patients. The antigen levels detected in serum by the various antibodies correlated significantly to each other, and to CA125 levels. The results of epitope analyses and combined IEMAs suggested that the epitopes recognized by these antibodies are not same, but exist on the same antigen which bears the CA125 epitope. A sensitive IEMA was developed with 602-1 and 602-6 antibodies which showed high reactivity to the CA125-like antigen. The antigen defined by these two antibodies was designated as CA602, and serum CA602 levels correlated well with CA125 levels. The CA602 antigen is a CA125-like antigen. Furthermore, the serum CA602 levels did not correlate to CA54/61 levels. The combined assays of CA602 and CA54/61 may increase the detection of ovarian carcinoma.

The intracoronary administration of urokinase following direct PTCA for acute myocardial infarction reduces early restenosis.

Early restenosis after successful percutaneous transluminal coronary angioplasty (PTCA) without antecedent thrombolytic therapy in patients with acute myocardial infarction (AMI) was assessed by performing in-hospital cardiac catheterization in 62 (88%) of 70 consecutive patients. Specific attention was focused on the effectiveness of the intracoronary administration of urokinase in cases with angiographic residual thrombus after successful direct PTCA. The following two treatment regimens were used: PTCA alone (43 patients) and PTCA followed by the intracoronary infusion of urokinase (27 patients). The rate of early restenosis was higher after successful direct PTCA alone (28%) than after direct PTCA followed by intracoronary urokinase (5%). Bleeding complications were no different between the two groups. These findings suggest that intracoronary urokinase can be effective in reducing early restenosis in patients with angiographic residual thrombus after successful direct PTCA. Therefore early restenosis may be related to residual intracoronary thrombus.

Percutaneous transluminal coronary angioplasty for patients with unstable angina pectoris.

Percutaneous transluminal coronary angioplasty (PTCA) was successful in 91% of 76 patients with unstable angina pectoris refractory to pharmacological treatment. However, the rate of acute occlusion and reocclusion was rather high (95). Restenosis developed in 56.5% of successful cases after initial PTCA, and 29 patients underwent 2nd, and nine 3rd PTCA. Most refractory unstable angina can be controlled by PTCA, which may require repeating in some patients.

[A case of retroperitoneal Castleman's disease associated with bladder tumor and a review of 59 cases in Japan].

We present a case of retroperitoneal Castleman's disease associated with bladder tumor. The patient was a 62-year-old man, who underwent partial cystectomy under a diagnosis of bladder tumor in 1989. Subsequently, recurrent bladder tumor was detected and he consulted the outpatient clinic at our University Hospital in November, 1990, at which time computerized tomography (CT) revealed a retroperitoneal tumor. From February, 1991 four courses of combined chemotherapy (methotrexate, vinblastine, pirarubicin, cisplatin) were administered for the tumor. The tumor reduction rate after the chemotherapy was 60% on CT, and retroperitoneal lymph node dissection and left nephroureterectomy were performed in July, 1991. Histological examination revealed Castleman's disease, hyaline-vascular type. Fifty-nine reported cases of retroperitoneal Castleman's disease in Japan are reviewed.

Electrical resistance and current leakage of appliances for transurethral resection.

Urethral stricture following transurethral resection of the prostate (TUR-P) is closely related to the electrical resistance and current leakage of appliances. Electrical resistance and leakage were therefore investigated in 7 old bipolar loops, 3 new bipolar loops (1, 2 and 3) used for TUR-P, 2 new bipolar loops (4 and 5) for TUR-bladder tumor (TUR-Bt), and 4 new monopolar loops (6 through 9) for TUR-P. Two of the 7 old bipolar loops were found to be positive and 5 negative for current leakage. The 2 positive loops had resistances of 4.4 and 17 ohm . Before use, the electrical resistance of the 5 new bipolar loops (1 to 5) ranged from 0.5 to 0.6 ohm. The resistance of loop 1 was less than 5 ohm after the sixth use, and that of loop 3 was 2.8 ohm after the fifth use. By contrast, loop 2 exhibited resistance of 115 ohm after the third use. The average operation time for loops 1, 2 and 3 were 66, 64 and 62 minutes. Loop 4 showed resistance of 14 ohm after the third use, and loop 5 1 ohm even after the sixth use. Furthermore, none of the 5 loops showed current leakage. On the other hand, all 4 monopolar loops (6 through 9) exhibited current leakage after the first use, and showed relatively high electrical resistance. These findings demonstrate the necessity of timely replacement of bipolar loops and the exchange of monopolar loops after the first use.

Cervical mucus and prediction of the time of ovulation.

12 normal ovulatory women were studied during 17 menstrual cycles. The first day on which the women had increasing quantities of 0.1 ml or more clear cervical mucus (IQCCM) was closely related to the time of ovulation as monitored by basal body temperature and radioimmunoassay of serum-luteinizing hormone, follicle-stimulating hormone, estradiol, and progesterone. The results show that the time of ovulation can be predicted clinically without specialized tests by observing the day of onset of IQCCM.

Anovulation and oviductal hyperplasia in rats treated with clomiphene citrate 5 days after birth.

Five day old female rats were administered either 500 micrograms clomiphene citrate or propylene glycol. Vaginal opening occurred significantly earlier in the clomiphene treated group as compared with the control group. Clomiphene treated rats showed permanent sterility associated with anovulatory vaginal cycles in which the stage of vaginal cornification predominated. Furthermore, clomiphene treated rats exhibited a high incidence of pyosalpinx (or oviductal abscess). The possible mechanisms responsible for the anovulatory sterility and the pyosalpinx induced by neonatal clomiphene treatment are discussed.

Possible involvement of indirect mechanism in cardiovascular action of dobutamine in dogs.

Possible mechanisms of dobutamine effects on adrenoceptors were investigated in comparison with tyramine, dopamine and noradrenaline after depletion of noradrenaline stores by ephedrine in dogs. Dobutamine, like other amines, induced pressor and positive chronotropic effects. After repeated treatment with ephedrine (total 40 mg/kg), both effects of dobutamine were markedly depressed. However, after ephedrine, the effects of tyramine were eliminated while those of noradrenaline were slightly decreased. Pressor responses to dopamine were reversed to depressor and positive chronotropic responses were abolished. The results suggest that dobutamine differs from other catecholamines in its mode of action and that its cardiovascular actions may be mediated in part through an indirect action on adrenoceptors.

Distribution and characterization of the adrenoceptors in dog coronary arteries.

Effects of catecholamines were studied in vitro on different portions of the left coronary artery of the dog. Adrenaline, noradrenaline and isoprenaline produced dose-related contractions in the common portion of the coronary artery which were blocked by phenoxybenzamine. The proximal portions were relaxed by noradrenaline and isoprenaline, but contracted or relaxed by adrenaline following an initial transient contraction. The contraction was blocked by phenoxybenzamine and the relaxation was blocked by propranolol. Adrenaline, noradrenaline and isoprenaline caused only relaxations which were blocked by propranolol in the distal portion and in the small branches. In coronary, femoral and tracheal strips contracted previously by KCl with or without a phenoxybenzamine pretreatment, isoprenaline produced dose-dependent relaxations. The dose-response curves for isoprenaline were shifted to the right on treatment with practolol. The blocking effects of practolol were nearly ten times potent in the coronary artery in comparison with the femoral artery and the trachea. The results indicate that the common portion mainly possesses alpha-adrenoceptors, the proximal portion contains both alpha- and beta-adrenoceptors, and that beta-adrenoceptors dominate in the distal portion and small artery branches. It is also shown that the beta-adrenoceptors in the coronary vessels differ from the femoral and tracheal beta-adrenoceptors in pharmacological feature.

Changes in hypothalamic LH-RH content and blood levels of LH-RH, gonadotropin and estradiol during the preovulatory stage of rat estrous cycle.

In order to investigate the sequence of events concerning gonadotropin surge, serum LH, FSH and estradiol concentrations were measured during the rat estrous cycle as well as hypothalamic and blood levels of LH-RH in the preovulatory stage. Normally cyclic female Wistar rats kept on 12 hr light (from 22.00 hr to 10.00 hr) and 12 hr dark were killed at different times of day during each stage of the cycle. The hypothalamus was quickly dissected out, divided into 3 portions (the anterior, middle and posterior) and extracted in 90% methanol. Blood LH-RH was extracted by affinity chromatography prior to radioimmunoassay. The content of LH-RH in the anterior and middle hypothalamus started to decrease between 1.00 hr-3.00 hr, reached its nadir at 6.00 hr on proestrus and recovered to its previous values on estrus. Almost simultaneously blood LH-RH concentration showed an increase of 18.3 pg/ml-8.8 pg/ml between 1.00 hr-3.00 hr, and then fell to less than 1.0 pg/ml at 6.00 hr. On the other hand, serum estradiol level began to elevate on diestrus II followed by its peak at 6.00 hr on proestrus, while the peaks of serum LH and FSH were observed at 8.00 hr and 10.00 hr, respectively. These studies indicate that the elevation of serum estradiol was followed by the release of LH-RH from the hypothalamus and the LH-RH may be responsible for the preovulatory discharge of gonadotropin.
