[The course of pregnancy in congenital thyroid gland aplasia. Case report with special reference to maternal hypothyroidism].

A report is given on a 28 years old women with congenital aplasia of the thyroid gland. She was substituted with thyroxine (300 micrograms per day). Her first pregnancy was complicated by gestational hypertension and pre-eclampsia. Delivery was by forceps. During the first trimester of her second pregnancy, bleedings occurred. The thyroid-stimulating hormone level (TSH-level) was increased (18.3 microU/ml). The patient did not show clinical signs of manifested hypothyroidism. The thyroxine dosis was increased. Bleedings disappeared. Labour was terminated and induced. Labour intra partum was hypoactive. The delivery was again by forceps. The newborn did not show any signals of hypothyroidism. Dysfunction of thyroid gland is associated with reduced fertility. Hypothyroidism in pregnancy is associated with an adverse outcome in fetal health as well as an increase in obstetric complications. Thyroid hormones play a vital role in fetal development and maturation of brain. Women with a hypothyroidism have a lower rate of pregnancy and a higher rate of spontaneous miscarriages compared to a normal population. Recognition and treatment of thyroid disorders in reproductive age occur before conception. Iodoprophylaxis is necessary for prevention of congenital hypothyroidism (cretinism). Iodoprophylaxis is necessary to prevent endemic goiter in pregnancy. Euthyroid goiter is an indication for a combined treatment with jodid and levothyroxine. Treatment should be individualized. Assessment of efficacy of treatment is based on measurement of TSH- and free thyroid hormone (fT4)-levels.

In vitro phosphorylation in isolated horizontal cells of the fish retina: effects of the state of light adaptation.

Horizontal cells, which are second-order neurons of the vertebrate retina, exhibit synaptic plasticity governed by light and dark adaptation. We have investigated the alterations in the protein phosphorylation patterns of isolated carp (Cyprinus carpio) horizontal cells in relation to their state of light adaptation by using an in vitro phosphorylation assay and compared the resulting data with protein synthesis patterns of the whole retina. Phosphoproteins and [35S]methionine-labelled proteins were analysed by one- and two-dimensional gel electrophoresis followed by autoradiography. The state of light adaptation significantly affected the in vitro phosphorylation of horizontal cell proteins with molecular weights of 68, 56/58, 47, 28 and 15 kDa, but had no effect on the protein synthesis of retinal proteins. In the light the most prominent increase of 32P incorporation was observed in the 47 kDa protein. The biochemical properties of this protein closely resembled those of the growth-associated GAP-48, found in the fish retina. In addition, the phosphorylation of horizontal cell homogenates in the presence of protein kinase activators such as cyclic AMP, calcium, calmodulin and phospholipids revealed that horizontal cells of the fish retina contain cyclic AMP-, calcium/calmodulin- and calcium/phospholipid-dependent protein kinase activity resulting in the phosphorylation of several horizontal cell proteins, including the phosphoproteins which were affected by the state of light adaptation.

[Tocolysis with ethanol--effects on the ethanol content of the blood in the mother and neonate].

In 10 pregnant women infusions with fructose and ethanol have been performed during labour until delivery. Ethanol concentration in blood have been estimated during delivery and in the post partum period. A decrease of labour activity could be found already with ethanol blood level below 0.5 mg/g. Ethanol elimination in newborn was delayed about 30 per cent in comparison with their mothers. We couldn't observe any risk neither fetal nor maternal by ethanol infusion.

[Adaptive behavior of mature newborn infants following obstetric pain control with peridural fentanyl].

Obstetric pain relief using fentanyl epidurally is a safe and efficient method. In this study the effects of this obstetric analgesia on the adaptation pattern of 26 eutrophic newborns in the first hour of live have been estimated with cardiorespirograph in comparison with a normal groups. We did also determinations of acid-base-balance in arterial blood of umbilical artery and in hyperemic capillary blood of newborns. There were significant differences in basal frequency and the duration till the beginning of resting phase in the cardiorespirogramm. The differences to normal newborns may be correlated to the slight increase of partial pressure of carbon dioxide in the newborns following obstetrical analgesia. During the first hour of life newborns can good compensate. There is no sedation of newborns by fentanyl.

[Results of ultrasound brain studies in clinically normal newborn infants].

Ultrasound investigations demonstrating intracranial hemorrhages (periventricular intraventricular hemorrhage-PVH/IHV) were performed in 401 term newborns. Without consideration the mode of delivery incidence of intracranial hemorrhage was 4.7%. There were only subependymal hemorrhages grade 1 and 2. In case of vaginal-operative delivered children frequency of hemorrhage in forceps delivered babies was really lower than in vacuum extracted newborns.

[Fluid distribution and catheter position in the peridural space in postoperative and cancer pain therapy. A radiological study].

A radiological study was made in 33 patients of the position of the epidural catheter and the spread of the contrast medium Metrizamide (Amipaque) when used in volumes of 10, 15 and 20 ml. The puncture at the interspinal space L2/L3 and L3/L4 allows the safe inclusion of eight peridural segments in all cases, whereas puncture of the upper regions of the spine results in a less predictable effect with spread of the injected fluid to higher spinal segments. A larger volume of contrast medium of 15 ml allows the safe inclusion of a larger number of spinal segments. Abnormal presentation, areas of fluid loss and the distribution of the analgesics or local anaesthetics injected may be detected early, which allows correction or adaptation of decisions concerning the fluid volume or the concentration of analgesics such as morphine.

[BMT 9141--new foetal monitor (author's transl)].

The BMT 9141, a foetal monitor, is described in this paper. It has succeeded the BMT 504, an earlier model which has worked well. It has been designed for alternative external or combined internal foetal monitoring. An automatic labour evaluation unit has been added as a new element. The monitor has stood its clinical test with good success.

Reverse transcriptase polymerase chain reaction for detecting Ewing's sarcoma in archival fine needle aspiration biopsies.

OBJECTIVE: Previous studies on pediatric soft tissue sarcomas have demonstrated a chromosomal 11;22 (q24;q12) translocation in Ewing's sarcoma (ES) and peripheral primitive neuroectodermal tumors that appears to be a unique oncogenic marker. To investigate the usefulness of reverse transcriptase polymerase chain reaction (RT-PCR) as an ancillary method for cytodiagnosis, we tested archival aspirates derived from ES patients to establish whether any beta-actin RNA expression or tumor-specific EWS/FLI-1 gene translocation had occurred. STUDY DESIGN: Sixteen skeletal specimens aspirated 10-15 years earlier from patients with cytologic and histologic diagnoses of ES were prepared for PCR. The amplification products were sequenced. RESULTS: Amplifiable RNA was detected in smears from 12 patients by beta-actin RT-PCR. Seven aspirates from beta-actin-positive patients showed ES-specific genomic translocation (58%). Sequence analysis of the resulting PCR fragments revealed EWS/FLI-1 fusion transcriptions of varying length. CONCLUSION: When RNA was retrievable, RT-PCR applied to routinely stained aspirate smears was a highly specific method in differential diagnosis of ES.

The integrated concentration of cortisone is reduced in obese children.

We evaluated the possibility that there is enhanced conversion of cortisol (F) to cortisone (E) in obese children. IC-E was measured from 15 lean children aged 12.7 +/- 2.2 years, body mass index Z-score (BMI-SD) = -0.35 +/- 0.82, IC-F = 197 +/- 70 nM/l and 9 obese children aged 12.3 +/- 3.2 years, BMI-SD = + 4.7 +/- 2.1, IC-F = 149 +/- 53 nM/l. IC-E was higher in lean children 76 +/- 25 nM/l compared to obese 60 +/- 11 nM/l (p < 0.04). There was no difference in the ratio of IC-E/IC-F between lean 0.40 +/- 0.10 and obese subjects 0.42 +/- 0.09 (p < 0.06). IC-E was directly correlated with IC-F: IC-E = 0.25 x IC-F + 26 (n = 24, r2 = 0.57, p < 0.0001). In a multiple regression model (overall r2 = 0.32, p < 0.02), IC-E was related to BMI-SD inversely (p < 0.0054) and influenced as well by interaction of BMI-SD with sex (p < 0.043), IC-E being lower in boys with increasing body mass. In childhood, obesity is associated with decreased plasma IC-E and IC-F levels, the ratio of IC-E/IC-F is independent of body mass. Reduced IC-E levels in obese children are most likely due to the impact of body mass on IC-F.

Image cytophotometric DNA analysis of cystadenolymphomas.

Warthin's tumour of the parotid gland is generally a benign lesion, consisting of an epithelial and a lymphoid component with unclear histogenetic relationship, 17 cases of Warthin's tumour were investigated for ploidy related parameters by the image analysis system CAS200. All cases investigated were euploid. A diploid DNA pattern (Auer type I) was detected in 7/17 (41%) cases both in the epithelial and lymphoid component of the same tumour. In addition to a diploid cell fraction a tetraploid cell component was found in 7/17 (41%) cases, coincidentally, in the epithelial and lymphoid component (Auer type II). Euploidy in all investigated cases is in agreement with the generally benign nature of Warthin's tumours. The similarity of the DNA pattern (Auer type) in the genetically unrelated epithelial and lymphoid components in most cases (84%) is suggested to be related to a similar functional state of these two components and is discussed in relation to the histogenesis of cystadenolymphomas.
