[Rotavirus-associated gastroenteritis in adults].

The human rotavirus in adult diarrhea cases was evaluated in 109 cases during 12 months, 1996. Rotavirus antigen was identified, using the ELISA technique, in stool samples in 23 cases (21%). We arranged the positive cases as rotavirus-associated gastroenteritis in adults and analyzed them. Fourteen out of them (74%) occurred from June through September. Fourteen cases (74%) needed to be hospitalized. Colonoscopy revealed some inflammatory changes in 9 out of 16 cases (47%). Most of symptom was watery diarrhea, but bloody diarrhea was observed in 6 cases (32%). It was suggested we should introduce a concept of rotavirus-associated gastroenteritis in adults.

Nucleosides and nucleotides. 131. Synthesis and properties of oligonucleotides containing 5-formyl-2'-deoxyuridine.

Thymidine was converted into 5-formyl-2'-deoxyuridine (1), which was incorporated into oligonucleotides, 5'd(GGAGA1CTCC)3' (I-1) and 5'd(GCTGC1GCGAAAGCTG)3' (II-1). To avoid side-reactions and degradation, protection of the formyl group of 1 using a newly developed protecting group, N,N-di-(3,5-dichlorophenyl)ethylenediamine, was necessary. Compound 1 was unstable under the conditions employed for enzymatic complete digestion of oligonucleotides, so that a peak corresponding to 1 was not detected clearly by HPLC analysis of a nucleoside mixture obtained by complete hydrolysis of I-1. Therefore, the oligonucleotide I-1 was treated with cyanomethylene-triphenylphosphorane to give an oligonucleotide containing (E) and (Z)-5-(2-cyanovinyl)-2'- deoxyuridine, which was then hydrolyzed, and the newly generated nucleosides were detected by HPLC analysis. The Tm of the self-complementary oligonucleotide I-1 (40 degrees C) was higher than that of the parent oligonucleotide, 5'd(GGAGATCTCC)3', (31 degrees C) in a buffer containing 0.01 M sodium phosphate (pH 7.0) and 0.1 M NaCl. DNA replication study on a template-primer system [primer, 5'd(32P-CAGCTTTCGC)3'; template, 3'd(GTCGAAAGCGXCGTCG)5' (X = 1 or T)] showed that dATP was incorporated into the DNA strand at a site opposite to 1 by Klenow DNA polymerase, but with a reduced rate. The formyl group of 1 in the oligonucleotides reacted with amines to give Schiff base derivatives.

Site-specific integration of the actinophage R4 genome into the chromosome of Streptomyces parvulus upon lysogenization.

The lysogenization of Streptomyces parvulus by actinophage R4 occurs by site-specific integration of the phage genome into the chromosome. The DNA fragments containing the attachment sites on the host chromosome, the phage genome, and the two junctions created by insertion of the phage genome were cloned and sequenced. The attachment sites were found to share a common core of 12 bp. This common core sequence was not detected in chromosomal DNAs of S. coelicolor and S. lividans.

[A phase II study of SM-5887 for advanced gastric cancer].

A phase II clinical trial of SM-5887, a new totally synthesized anthracycline derivative, was carried out in 13 patients with inoperable or recurrent gastric cancer. No patient had been given anthracycline previously. SM-5887 was administered by I.V. bolus with a dose of 100 mg/m2 every three weeks. Twelve of 13 cases were eligible and evaluable for the response. Of the 12 evaluated cases, 6 showed no change (NC), including one minor response (MR). The remaining 6 cases showed progressive disease (PD). Adverse effects were relatively mild in most cases and included anemia, leukocytopenia, thrombocytopenia, nausea/vomiting, phlebitis, hair loss and fever. Among them, leukocytopenia was observed most frequently.

[Ifosfamide-induced urinary bladder lesion and its inhibition by mesna].

This study examined the characteristics of urinary bladder lesions (cystitis) induced by ifosfamide (IF), an antitumor agent, and their inhibition by mesna in rats. Single i.v. doses of 50 mg/kg or more of IF induced cystitis characterized by increased bladder weight with mucosal hemorrhage and edema within 3 hr, with a plateau being reached 1 to 3 days after the injection. This change occurred earlier than myelotoxicity. Injection of IF to ureter-ligated rats did not induce the cystitis. Also, when IF was infused to one of a pair of rats in which the ureters had been crossed, the cystitis occurred only in the non-injected member of the pair whose bladder received urine flow from the IF-injected rat. Cystitis did not occur with direct injection of 10 mg of IF into the urinary bladder, but did with injection of the metabolite, 14 micrograms of acrolein. These findings show that IF-induced cystitis is not a systemic effect like myelotoxicity or an antitumor effect but a local effect. The cystitis could be completely inhibited by mesna, and this effect, which was dose-dependent, occurred from one-tenth of the IF dose. The effect was strongest when mesna was administrated within 1 hr of the IF injection. There was little difference in the effect between single and divided doses. The combined administration of IF with mesna did not lower the antitumor effect of IF.

[Clinical evaluation of cefmenoxime in internal medicine, with special reference to infection associated with hematological disorders].

Clinical evaluation of cefmenoxime (CMX, Bestcall) was performed against infections associated with hematological, respiratory tract and other disorders. Clinical effectiveness of CMX against severe infections with hematological disorders including sepsis, pneumonia, pyelitis and so on was 74.4% for good responses and against the respiratory tract infections, 96.2% for good responses was obtained. Neither objective or subjective side effects nor extreme abnormalities in laboratory tests were observed in these patients. It can be concluded, therefore, that CMX is one of the most useful drugs against infectious diseases associated with hematological disorders, respiratory tract and other disorders.

The influence of histologic type on survival in early extranodal non-Hodgkin's lymphoma in head and neck.

Since the Rappaport classification provides little information for the prognosis of extranodal non-Hodgkin's lymphoma (NHL), the Kiel classification was used for 71 patients with head and neck extranodal NHL, stages IE and IIE. The ratios of low grade malignancy ( LGM ) and high grade malignancy ( HGM ) were about 60 and 40%, respectively. The lesions comprised 37 non-thyroid lymphomas and 34 thyroid lymphomas. 8 cases of thyroid NHL did not fit into a rigid classification scheme, and were separatively analyzed. In the whole group of patients and the subset of 37 patients with non-thyroid NHL, the 5-year survival rates for LGM were significantly better than for HGM . LGM showed a more favorable prognosis than HGM in 26 patients with thyroid NHL, but the difference was not statistically significant. 8 cases of thyroid NHL showed diffuse proliferation of centrocytes or centroblasts in which various numbers of nodules consisting of large centroblasts and centrocytes with high mitotic activity were scattered. The cases behaved as HGM . From these findings, we suggest that the Kiel classification in extranodal NHL is helpful.

Preliminary chemical and X-ray studies on the interactions of E. coli DNA with putrescine.

The interactions of putrescine, the major diamine in E. coli, with E. coli DNA as a model of phage DNA were studied by melting temperature analysis, equilibrium dialysis and X-ray diffraction with the aid of molecular model building. The chemical analysis of the DNA-putrescine complex shows that the molar binding ratio of putrescine to DNA (phosphate) is nearly 1 to 2. The equilibrium (or reversible) binding of putrescine to DNA was suggested by the fact that the melting temperature increased according to the concentration of added putrescine, and its elevation was not saturated even at the molar ratio of 6 to 1. The equilibrium dialysis experiments indicate that the association constant for the complex is a little smaller than, but in the same order (10(3) liter/mol) as, that of DNA-spermine complex. The binding of putrescine stabilizes the B-form of DNA fiber, which is well preserved even at 66% relative humidity. The distance between the neighboring DNA helices in the wet fiber increased with the increasing degree of hydration, as in the case of native DNA. Unlike spermine, putrescine does not form precipitate upon mixing with DNA in the concentration range for UV measurement, suggesting that the cross-bridge formed by putrescine is intra-double helical. The equilibrium binding of putrescine to DNA, seems to be important for the life cycle of lambda-phage.

Childhood non-Hodgkin's lymphoma developing from recurrent neck mass.

Non-Hodgkin's lymphoma (NHL) in an 8-year-old boy developing from recurrent neck mass during 15-months history is presented. Two biopsies carried out during this period were interpreted as reactive change. The third biopsy was diagnosed as NHL. He died of the systemic disease 19 months after the first admission. Clinical, histological and immunocytochemical investigations for the three biopsy materials revealed that the first and the second biopsies were reactive, and the third biopsy was neoplastic, B cell lymphoma. Known etiological factors causing lymphoreticular disorders are reviewed by the literatures, and absence of these factors in the present case is described.

Malignant histiocytosis with slow clinical course.

Previously we reported that malignant histiocytosis presenting as lethal midline granuloma (MH-LMG) showed rapidly progressive course (mean survival 14 months). Because of the same biopsy findings of polymorphic cellular infiltrates (PCI), we considered that MH-LMG and midline malignant reticulosis (MMR) are similar disease. In this paper, we studied 5 cases with MH-LMG showing slow clinical course with more than 5 year survivals, and a similarity found in MH-LMG and MMR in which disseminated and localized disease are present, is discussed. Among clinical data, no abnormality in hematologic findings in MH-LMG with slow clinical course was the only different point from that with progressive course. Biopsy findings of PCI in which atypical histiocytes with positive reactions for lysozyme by immunoperoxidase procedures were present, were identical with those in MH-LMG with progressive course. Autopsy findings justified a diagnosis of MH. MH with slow clinical course seems to be identical with MMR showing localized lesion. Therefore a term MMR should be replaced by a term MH-LMG in the classification of LMG.

[Clinical evaluation of cefotiam in internal medicine].

Cefotiam (CTM) was administered to 52 patients with infectious disease associated with respiratory system, hematological malignancy, urinary system and other system. Good clinical responses were obtained in 38 out of 52 cases (73.1%). Neither objective and subjective side effects nor extreme abnormalities of laboratory tests were observed in these patients. It can be, therefore, concluded that CTM is 1 of the most useful drugs for infectious diseases in respiratory system, hematological malignancy, urinary system and other system.

[Chronic toxicity study of latamoxef in beagle dog (author's transl)].

Chronic toxicity study of latamoxef (LMOX, 6059-S) by intravenous administration at dose levels of 100, 200 and 400 mg/kg daily for 6 months was carried out in adult Beagle dogs. All dogs of the 6059-S treated groups survived throughout the experimental period without showing any toxic signs other than occasional vomiting. Slight decrease of RBC, Ht, Hb and platelet, and increase of reticulocytes were noted in the 400 mg/kg group. Blood chemistry revealed decreased activity in GPT in the 6059-S treated groups, and increased contents of total protein and lipids in the 400 mg/kg group. These clinical changes were slight and transient in most instances. Liver and kidneys weights increased slightly without accompanying any pathological alterations. Inflammatory changes, probably due to mechanical irritation, were found in the subcutis and around the vein at the injection sites in all groups including the mannitol control group. From these results it is concluded that the maximum nontoxic dose in dogs is in the range of 200 to 400 mg/kg when the 6059-S was intravenously administered daily for 6 months.

Preliminary chemical and X-ray studies on the interactions of E. coli DNA with putrescine.

The interactions of E.coli DNA with putrescine in aqueous solution and fiber were studied by melting temperature analysis, equilibrium dialysis and X-ray diffraction. The equilibrium binding of putrescine to DNA is shown, of which association constant is in the order of 10(3) 1/mol. The binding of putrescine stabilizes the B-form of DNA fiber and the crossbridge of putrescine in the intra-double helix is suggested by precipitate formation experiments of various polyamines with DNA.

Steroid-induced concentric membrane whorls in dog liver.

When daily doses of 10 mg/kg of the androgenic steroids fluoxymesterone, methyltestosterone, testosterone propionate, oxymetholone and mepitiostane were administered to adult male and female beagle dogs for 6 months, concentric membrane whorls were produced in the hepatocytes of all groups. The whorls frequently had a central core mainly composed of lipids or mitochondria and the membranes of the whorls, consisting of paired membranes, continued to the smooth or granular endoplasmic reticulum at the periphery of the structures.
