Transient visual blurring, retro-orbital pain and repetitive involuntary movements in unilateral carotid artery occlusion.

An 83-year-old man presented with episodes of right sided retro-orbital pain, visual disturbance, involuntary jerks of his left arm and less frequently his left leg. The symptoms could be triggered by exercise, heat or cough. EEG recordings revealed no epileptic discharges. Duplex ultrasonography showed an occlusion of the right internal carotid artery. Blood flow velocity in the right middle cerebral artery was reduced and vasomotor reactivity to hypercapnia was absent. Reduction of his antihypertensive medication rendered the patient asymptomatic. The combination of transient visual blurring, retro-orbital pain and contralateral limb shaking can be an unusual manifestation of carotid occlusive disease. In such a case, the symptoms may be managed successfully by the elevation of blood pressure.

[Toxicologic evaluation of pyrethroids in indoor air: demonstrated with the example of cyfluthrin and permethrin].

Pyrethroids have varying activities depending on vehicle or route of administration (oral, dermal, inhalational). Specific features like the sensory irritation potential of the alpha-cyano-pyrethroids on the respiratory tract can only be quantified adequately by inhalation testing. Thus equitoxic dosages can vary between inhalative and oral application, especially for alpha-cyano-pyrethrolds. The no-effect values for chronic exposures derived for permethrin (type I pyrethroid) and cyfluthrin (type II pyrethroid) show clearly, that each pyrethroid has to be considered as an individual substance toxicologically, and that any extrapolation from the oral to the inhalative route should only be done after a thorough assessment of the specific toxicological profile. The study of simulated pest control measures on carpets pretreated with permethrin showed, that no significant enrichment of permethrin in total dust could be seen from a carpet additionally treated with pyrethroids. The missing correlation between absolute (mg pyrethroid/m3 air) and relative (mg pyrethroid/kg dust) concentrations in air-borne dust as well as the low degree of translocation of pyrethroids from carpets (only about 0.044% x m(-2) x h(-1) of the cyfluthrin applied to the carpet can be regarded as possibly respirable) prove, that analyses of pyrethroids in household sedimented dust ("vacuum cleaner bag analyses") without knowing the absolute surface concentration and respective air concentrations are of little value for risk assessment. The data allow the conclusion, that a scientific assessment of health risks is only possible based on absolute concentrations of pyrethroids in indoor air.

[Cryosurgery of condylomata acuminata gigantea--a contribution to therapy].

Two cases of condylomata acuminata gigantea treated with cryosurgery are described. A clinical and histologic differentiation from Buschke-Loewenstein's tumours, sex-specific differences, possible malignant transformations and differential diagnostic diseases are described and references are made to confusions with condyloma-like, primary carcinomas. Compared to conservative surgical treatment of condylomata acuminata gigantea, cryosurgery offers the advantage of rapid necrosis of the condylomata which are repelled without any further surgical intervention and heal without scars. Therefore we consider cryosurgery a more refined technique in treatment of condylomata acuminata gigantea. In extensive involvement, however, cryosurgical measures must be repeated.

The influence of psychotropic drugs and releasing hormones on anterior pituitary hormone secretion in healthy subjects and depressed patients.

Pharmacoendocrinological studies have shown that psychotropic drugs with different actions have different effects on anterior pituitary hormone secretion in man. Substances with different effects on the central nervous system are characterized by a different pharmacoendocrinological profile. Studies with various receptor blockers have shown varying influences on the DMI-induced growth hormone, prolactin, and ACTH/cortisol secretion. Growth hormone stimulation was shown to be mediated by alpha 2-receptors and inhibited by beta-receptors. Investigations in male and female endogenous depressive patients demonstrated a significantly blunted growth hormone response to DMI compared with age- and sex-matched healthy subjects. A comparative study in male endogenous depressive patients showed a significantly diminished growth hormone stimulation both after DMI and after growth hormone-releasing hormone compared to healthy male subjects. In further tests a simultaneous application of four releasing hormones (GHRH, CRH, GnRH, TRH) was used. These investigations showed a significantly lower GH stimulation in endogenous depressive patients compared with age- and sex-matched healthy subjects, but not in neurotic depressive or schizophrenic patients. Cortisol stimulation was similar in all groups of patients and healthy subjects. TSH stimulation was significantly lower in endogenous depressive and schizophrenic patients than in healthy subjects. Somatomedin-C concentrations were significantly elevated in endogenous depressed patients compared with healthy subjects. The blunted growth hormone response in endogenous depression could be explained by inhibitory influences such as increased somatomedin-C concentrations or a hyperactivity of central beta-adrenergic-receptors.

[Potentialities and limits of cryosurgery of vulvar cancer--12 years' results of therapy].

From 1971 to 1982 146 patients suffering from vulvar carcinoma have been cryosurgically treated at the Department of Gynecology and Obstetrics of the Medical Academy of Erfurt. There were primary inoperable, progressive, metastatic or recurrent cases after preliminary operative or radiation therapy. The 5-years surviving rate by cryotherapy, rarely combined with other methods of therapy (cytostatic therapy with Bleomycin, x-radiation, operative lymphonodectomy) was 38.5%. Our results show that cryosurgery extends the possibilities of conventional treatment of vulvar carcinoma both in palliative therapy of progressive cancer and in primary treatment, especially in patients, who cannot be charged by other complicated operations. According to every other cancer-therapy cryosurgery should be performed only in a gynaecological centre for cancer treatment.

[Cryotherapy of vulvar precancerous conditions--results of 12 years' treatment].

The recommendations for therapy of the facultative and obligate precancerous diseases of the vulva contain a lot of measurements extending from a local conservative treatment to the vulvectomy designated by a very different share of recurrences. With regard to the sure analgetic and controllable cell destroying effects of the coldness we cryosurgically treated 32 patients suffering from a kraurosis and 3 women with a histological secured Bowen's disease at the Department of Gynecology of the Medical Academy of Erfurt from 1971 to 1982. The present outcome proves this measurement being a very successful method but not always without recurrence. Therefore we consider cryosurgery as a real enrichment in conventional treatment of the precancerous diseases of the vulva but this measure can not be taken for the method of choice.

[Crisis intervention in the framework of an acute psychiatric general ward].

Basing on the description of the concept for treating patients admitted and treated on the lines of crisis intervention for acute psychogenic crises, in an open, unsegregated (i. e. for both sexes) general psychiatric ward for acute cases in a university hospital, the data collected from 90 patients in the course of a year in respect of age, sex, cause of crisis, suicidal tendencies, diagnosis and duration of stay and follow-up treatment were analysed and compared with those collected by other crisis intervention units. It was found that there was major agreement regarding most criteria between our unit and other units; however, neurosis or personality disorder were diagnosed much more frequently than a psychogenic reaction. Over and above this, the inpatient treatment period was longer. We believe that the differences are due to an understanding of the crisis concerned which classifies the patients not so much as healthy persons thrown off balance by a major upset, rather as patients with chronic psychic difficulties that decompensate through various external factors. In the first place, treatment of the basic disease intends to protect the patients against re-exacerbation of unpleasant conflicts that are of a piercing nature. The article also points out the pros and cons of a ward to which crisis patients and psychiatric emergency cases are admitted without previous selection and can be treated adequately without additional stress being placed on the patient by a transfer or a change of physician and therapist.
