Oral treatment of ichthyosis with an aromatic retinoid.

An aromatic retinoid (Ro-10/9359) was used for oral treatment of five cases of ichthyosis (three lamellar, two X-linked. Complete clearing of the skin lesions was achieved in all five patients within 24.2 +/- 3.2 days (X-linked 21.75 +/- 6.5, lamellar 23 days). Histopathology showed reduction of the hyperkeratosis, and thickening of the granular layer. Clinical side effects were of mild intensity and included cheilitis, conjunctivitis and pruritus. All side effects were reversible upon reduction of the daily dosage. In three patients treatment was discontinued after clearing of lesions. Fresh lesions re-appeared 6 weeks later. One patient with X-linked ichthyosis developed two recurrences during maintenance treatment; one patient with lamellar ichthyosis was kept in complete remission for 9 weeks on a reduced daily dosage.

A rapid and simple fibrinogen determination.

A fast and simple fibrinogen determination is presented, based on the ammonium sulfate precipitation of fibrinogen. The nature of the precipitated material is investigated by means of immunoelectrophoresis. A comparison is made with a paper electrophoretic method, and a method based on the clotting properties of fibrinogen. The linearity, the within-run precision and normal values, obtained by this method are given. An advantage over other well-known methods is the use of a serum blank instead of a plasma blank without reagent. In this way all interfering substances are detected immediately and simple subtraction of blank readings from sample readings gives true fibrinogen values. The clinical application with respect to the acute phase of inflammation is discussed.

Recurrent Bell's palsy. Etiology, frequency, prognosis.

The etiology, frequency and prognosis of recurrent Bell's palsy were studied in patients with peripheral paresis of n.facialis of various etiology. Fourteen (11.9%) of 117 patients with Bell's palsy had a recurrent paresis of n.facialis. Nine of these were of homolateral and five contralateral type. Seven recurrent facial paralyses in idiopathic Bell's palsy (i.e. 10.4% of the patients with idiopathic facial palsy), 3 facial palsies of viral origin (i.e. 8.8% of the patients with viral facial palsy), one in association with diabetes mellitus, one during pregnancy, one combined with positive rheumatic serological tests, and one in a case of Melkersson-Rosenthal syndrome were found. The frequency and heterogenity of etiology of recurrent facial palsies suggest a predisposing factor or immune mechanisms. In eight patients there was within 6 to 8 weeks a good, in two patients a moderate and in four patients a poor recovery of function. The greater reduction of the compound action potential of the m.orbicularis oris in recurrent homolateral facial palsy in relation to patients with single manifestation point out the greater denervation and therefore the poorer prognosis of recurrent palsies. A prophylactic decompression to prevent a third attack of recurrent facial paresis is considered.

Action Potentials

[Computer documentation of allergological data on patients suffering from allergies of the ENT-tract (author's transl)].

After a brief discussion of the purpose of EDP in medicine an EDP system is described that serves the collection of data on patients of an allergy department. It was taken into account that in addition to data concerning case histories and the general and special status data on the different courses of therapies and test results can be collected. The system is specifically adapted to the problem of allergies of the respiratory tract, but may also be used--with minor additions or alterations--for internal or dermatological cases. The aim of this documentation is the systemization and at the same time the rationalization of the examination course, without, however, ruling out individual extensions. Such a system facilitates and accelerates not only the collection of data, it also allows a faster systematic disclosure of errors and makes a meaningful scientific evaluation possible.

Acute Disease

[T-Lymphocyte shifts in patients with melanoma and bronchogenic carcinoma (author's transl)].

Lymphocyte subpopulations were investigated in 32 patients with malignant melanoma, 25 patients with bronchogenic carcinoma and 59 control subjects. Whereas the determination of membrane immunoglobulin-bearing lymphocytes and of complement receptor-bearing lymphocytes gave comparable results in the tumour patient and control group, significant differences were found in the T-lymphocyte population using the sheep red blood cell (SRBC) assay. The so-called "active" Wybran rosette test, which characterizes a T-cell subpopulation with an especially avid receptor for SRBC, gave significantly lower results in patients with melanoma (25% control 35%, p less than 0.01) and bronchogenic carcinoma (21% control 35% p less than 0.01). Determination of the total T-cell population using the Jondal rosette assay gave significantly lower values in the patients with melanoma (52%, control 63%, p less than 0.01), but not in those with lung cancer (65%). Low rosette values were detected even at low histological invasion levels (classified according to Clark) in the patients with melanoma. No correlation was found between the invasion level and the percentage of rosette-forming cells. The significance of these findings and the value of the rosette assay in the assessment of the immunological reactivity of tumour patients is discussed.

Properties of the K562 cell line, derived from a patient with chronic myeloid leukemia.

The K562 cell line derived from a CML patient in blast crisis was examined for properties of B and T lymphocytes and cell lines. K562 lacks the B markers of immunoglobulins, Epstein-Barr virus (EBV) genome and associated nuclear antigen, and receptors for EBV. A low proportion of cells from rosettes with sheep erythrocytes, the frequency of which is considerably increased after neuraminidase treatment. Unlike B lines but like T lines, K562 cells are lysed rapidly by C'/Fc receptor-positive human blood leukocytes and do not stimulate MLC reactions. On the other hand, K562 lacks T antigen, high radiosensitivity and sensitivity to growth inhibition by thymidine. The cells do not contain N-APase, an enzyme found in all lines derived from lymphoid cells and in lymphoproliferative diseases. By scanning electron microscopy, K562 cells were seen to be rounded and relatively smooth, with small numbers of short microvilli resembling undifferentiated leukemic cells. A few cells had narrow ridge-like profiles and small ruffles similar to granulocytic leukemic cells. K562 is strongly positive for immunoglobuln Fc receptors and pinocytosis, but does not phagocytose or mediate antibody-dependent phagocytosis or cytolysis. Among histochemical stains, K562 is positive for esterase, lipid, and acid phosphatase. There seems to be no doubt that K562 is not a B cell line. While it has some T cell properties, these are not exclusive. Some of its characteristics indicate that it is probably not lymphoid. Due to its low level of differentiation, its nature cannot be stated with certainty. On the basis of the possible presence of the cellular marker of chronic myeloid leukemia, the Ph chromosome, it may be regarded as belonging to the granulocytic series of cells.

Alkaline Phosphatase

Comparative study of alkaline phosphatase activity in lymphocytes, mitogen-induced blasts, lymphoblastoid cell lines, acute myeloid leukemia, and chronic lymphatic leukemia cells.

Alkaline phosphatase [orthophosphoricmonoester phosphohydrolase (alkaline pH optimum), EC 3.1.3.1] purified from a Burkitt lymphoma cell line (Daudi) and Moloney-virus-induced murine leukemia (YAC) showed unique catalytic properties in substrate specificity and inhibition by cysteamine-S-phosphate. It migrated on polyacrylamide gel electrophoresis in a single activity band. Alkaline phosphatase with similar properties was found in several human lymphoblastoid cell lines, in chronic lymphatic leukemic cells, in organs of leukemic mice, and in sera of patients with certain lymphoproliferative disorders.

[The determination of the sum potential amplitude for the prognostication of facial paresis of viral etiology].

In 29 patients with facial pareses of viral etiology the sum potential amplitude of the M. orbicularis oris was determined in a comparison of the two sides for prognostication purposes. If the sum potential amplitude is reduced by less than 70 per cent a return of good functions can be expected within 6-8 weeks. If the reduction of the sum potential amplitude lies between 70 and 95 per cent, a reliable prognosis is not possible, whereas with values below 95 per cent the paresis does not improve at all or hardly improves within 6-8 weeks. An evaluation of the returned function, about 1 year after the beginning of the paresis, i.e. at a time when the regeneration is basically completed, shows that all cases with an originally poor function (judged 6-8 weeks after onset of paresis) had changed into cases with a moderate function, and all cases with an originally moderate function had turned into cases with a good function. In another study concerning patients with idiopathic facial paresis the same method yielded results comparable to those of the above-mentioned study, so that the conclusion can be drawn that in the prognostication of the peripheral facial paresis by means of determining the sum potential amplitude-regardless of whether the facial paresis is of viral or idiopathic etiology-the same yardsticks have to be applied.

Action Potentials

[Determination of the latency for the prognosis of Bell's palsy (author's transl)].

In a previous study the authors discussed the value of the determination of the compound action potential amplitude of the m. orbicularis oris following supramaximal stimuli of the facial nerve for the prognosis of facial paresis. In this study they investigate the suitability of the determination of the latency periods for an early prognosis. The latency periods in 20 normal subjects and 26 patients with idiopathic facial paresis were messured and continuously checked. The study reveals that a difference of more than 0.5 msec. between the latency periods of normal and paretic patients does not even permit the proof of peripheral lesions in patients with partial denervation or absent or insufficient recovery. Ocassionally, even shorter latency were observed in paretic patients, which are probably due to a local increase of the threshold for supramaximal stimuli around the stimulation electrode. The necessary increase of the stimulus intensity probably leads to a nervous excitation further distally. Contrary to the observations of other authors these studies indicate that the determination of the latency does not constitute a useful prognostic criterion for idiopathic facial paresis; for an early prognosis the measurement of latency is less reliable than the determination of the compound action potential amplitude.

Electric Stimulation

[Experiences gained in the treatment of oto- and rhinogenic meningitides, 1965-1975 (author's transl)].

The studies permit the conclusion that morbidity and mortality due to purulent oto- or rhinogenic meningitides have not essentially changed over the past eleven years despite antibiotics that are more effective and can be given in higher doses than so far. Resistent strains of bacteria, allergies and insufficient antibiotic therapy are probable causes for that fact. Under certain circumstances a sufficient and specific antibiotic therapy may delay surgical intervention or make it even unnecessary. Advanced age and additional diseases seem neither to influence operability nor to increase morbidity.
