[Diagnostic difficulties in cases of vasogenic lesions in the cerebellum].

The authors describe the clinical manifestations in 200 cases of cerebellar vascular lesions confirmed on autopsy. The clinical symptoms and signs included very little typical cerebellar signs, the most frequently observed changes were: gradually increasing coma, muscular hypotonia, sluggish reflexes, ocular signs. The diagnosis was made even more difficult by the presence of additional pyramidal signs. The causes of diagnostic errors emphasized by the authors are: 1) an erroneous conviction of doctors that vascular cerebellar lesions must be associated with cerebellar signs, 2) difficulty in carrying out whole neurological examination since gait and posture cannot be observed in these bedridden patients, 3) frequently cerebellar focal lesions are associated with focal lesions in other brain structures (brainstem and cerebral hemispheres) with consequent superimposition of various symptom complexes, 4) presence of profound coma which not infrequently blurs many signs. Nevertheless, the authors could have established the clinical diagnosis of vascular cerebellar lesions in one-third of these cases.

Estimation of the dynamic carotid flow in patients with normal radionuclide cerebral perfusion.

Radioisotope angioscintigraphy following intravenous injection bolus of 99mTc-pertechnetate was performed in 49 patients with cerebrovascular disturbances. The region of interest placement on the sequential analog images of both middle cerebral and internal carotid arteries and time-activity curves were displayed on the video monitor. In 35 cases of this group we have noticed symmetrical cerebral perfusion and a significant difference between right and left internal carotid flow, and in 33 cases of which dynamic studies were made stenosis or occlusion of the extracranial part of the carotid internal artery was revealed by the X-ray angiography.

[Diagnostic difficulties in brain strokes in the light of personal observations].

The authors report a clinico-neuropathological analysis of 350 case records and autopsy protocols of patients dying from stroke in the years 1962-1966 at the Department of Neurology of the Grochów Hospital in Warsaw. Full agreement of clinical diagnosis and autopsy diagnosis was found in 314 cases while in 36 cases (about 10%) the clinical diagnosis was not correct. The authors believe that the diagnostic difficulties encountered by clinicians are due frequently to a complex pathological mechanism of vasogenic lesions in the brain and frequency of encephalomalatic foci found in the same patient beside haemorrhagic lesions. In the present material 116 (33%) such cases have been found.

[Angio-ultrasonography--a method of examining the extracranial arteries].

The authors describe a method of investigations of the extracranial cerebral arteries--angioultrasonography based on the Doppler effect. The study was done with a Polish apparatus designed and produced by the Experimental Laboratory of the Institute of Basic Technical Problems, Polish Academy of Sciences. The study confirmed the usefulness of this method in the diagnosis of arterial diseases involving the extracranial arteries supplying the brain. A high agreement was demonstrated between the results of angioultrasonography and classical angiography. Owing to the use of a grey scale it was possible to localize accurately the site of stenosis or occlusion of the studied artery.

[Assessment of regional cerebral blood flow (rCBF) and vascular changes in angiographic studies in cases of cerebral ischemia].

In 22 patients the regional cerebral blood flow (rCBF) was measured after intra-arterial injection of xenon 133Xe. In all patients cerebral angiography was performed after that and in 4 patients aortic arch angiography was done as well. In 2 cases of subclavian artery stenosis with normal state of the branches of the internal carotid artery the rCBF was not decreased. On the other hand, a significant decrease of rCBF was found in cases of transient cerebral ischaemic episodes with angiographically demonstrated bilateral stenosis of the afferent arteries and coexistence of stenosis of the afrerent arteries supplying the brain and intracranial arteries. On the basis of an analysis of rCBF angiograms the authors suggest the conclusion that a joint evaluation of the clinical manifestations, results of rCBF measurements and angiograms increases the accuracy of assessment of the condition of the cerebral circulation in patients qualified for patency-restoring operations.

[Importance of dynamics of scintigraphic changes in the brain for prognosis in stroke].

The results of scintigraphic investigations of the brain were analysed in patients with ischaemic strokes. In all cases scintigraphic investigation was done twice at least. Particular attention is called to the dynamic development of changes in the size of pathological foci in relation to the degree of improvement in the neurological condition of the patients over one year of follow up. Among patients with considerable neurological improvement control investigations carried out after 4 to 8 weeks of the disease demonstrated return of normal scintigraphic findings or a considerable reduction in the size of pathological foci. On the other hand, in patients without neurological improvement pathological scintigraphic changes persisted much longer, they were present in the investigations performed after 4 to 8 weeks, and even after 3--4 months. The presented observations point out that scintigraphic investigations of the brain can have a prognostic significance for the prognosis in brain strokes. These investigations must be done, however, at least twice between the 2nd and 3rd weeks and between the 4th and 8th weeks of the disease. Normalization of scintigraphic findings or a considerable reduction in the size of the pathological changes in the second investigation may suggest good prognosis.

[Evaluation of sulocton action in patients with atherosclerotic brain ischemia].

Sulocton effect was evaluated in 33 patients with symptoms of brain ischaemia during atherosclerosis. Thirty patients received the drug orally for 2 months in doses of 100 mg thrice daily. Three patients discontinued the treatment earlier (two of them discontinued it because of side effects). After two months of treatment a significant improvement was observed in such disturbances important for wellbeing and social contacts as: anxiety and fear, mood depression, disequilibrium of emotion, motivation and initiative. In many patients headaches, dizziness and gait disturbances disappeared or diminished. Sulocton was useful in the treatment of patients with cerebral atherosclerosis.

[Scintigraphic studies in stroke].

The authors report the results of scintigraphic investigations in 70 patients treated for brain strokes. The diagnostic and prognostic values of these investigations were estimated. It was observed that ischaemic foci were present most frequently in the area of vascularization of the middle cerebral artery. The intensity of neurological signs was correlated with the size of the pathological focus in the cerebral scintiscan. A close relationship was observed between improvement in the neurological status of the patients and regression of pathological changes in control scintigrams.

[Indications and contraindications in the surgical treatment of stenosis and occlusion of the extracranial cerebral arteries].

On the basis of a survey of the pertinent literature and own experiences the authors discuss briefly the indications and contraindications to surgical treatment of stenosis or occlusion of the extracranial arteries supplying the brain. They stress that reconstructive operations on the extracranial cerebral arteries are primarily of prophylactic importance preventing stroke development, they should be carried out in transient brain ischaemia and imminent stroke, severe completed strokes are not treated in this way. Stenosis of the internal carotid artery lends itself in the first place to this reconstructive operation which should be done possibly early when the artery is not yet completely occluded. The best time for the operation is 3-4 weeks after development of brain symptoms after abating of the acute stage of the disease.

[The clinical picture of stenosis and occlusion of the extracranial cerebral arteries].

The authors report a clinical analysis of 200 cases of various forms of stenosis and occlusion of the extracranial arteries supplying the brain. The cases were collected in a period of 15 years. Follow-up investigations were conducted in 100 conservatively treated patients and 100 surgically treated ones. Syndromes of cerebral, ophthalmic, trophic and vascular signs are described, their incidence is discussed and the most salient features are emphasized.

[Usefulness of Doppler ultrasound method in the diagnosis of cerebrovascular disorders].

The authors carried out ultrasonographic investigations in 136 patients with disturbances of the cerebral circulation and compared them with the results of clinical observations. In a high proportion of cases an agreement was found between the results of ultrasonographic investigations and clinical observations. Besides that, the authors call attention to the importance of these Doppler ultrasonographic investigations in the so called silent cases without clinical signs, and in patients with progressing atherosclerotic changes, particularly in the qualification of these patients for surgical procedures.

[Results of examination of cerebral arteries by the Doppler ultrasound method as compared to angiographic results].

The result of angiographic and ultrasonographic investigations were compared in 87 patients with cerebral circulation disturbances. A high degree of agreement (79%) was demonstrated between the results of Doppler ultrasonography and angiography. The authors stress the importance of ultrasonographic investigations in the differential diagnosis between stenosis and occlusion of these arteries and call attention to the possibility of repeated investigations enabling the progression of atherosclerotic lesions to be observed.
