Insulin-stimulated glucose uptake and fasting blood glucose.

Changes in insulin-stimulated glucose metabolism were studied in young and aged subjects, subjects with impaired glucose tolerance, and patients with NIDDM by means of the glucose clamp technique. The diabetic group includes obese and non-obese patients treated without insulin and non-obese patients treated with insulin. The glucose disposal rate (GDR) was decreased in aged subjects (5.8 +/- 0.4 mg/kg/min) compared with young controls (7.4 +/- 0.3 mg/kg/min). In patients with IGT, it was further decreased to 3.6 +/- 0.5 mg/kg/min, which was comparable to the rate in NIDDM without insulin treatment (3.3 +/- 0.4 mg/kg/min). There were no differences in the GDR between obese (3.0 +/- 0.3 mg/kg/min) and non-obese (3.4 +/- 0.6 mg/kg/min) diabetic patients. In insulin-treated diabetic patients, GDR ranged widely, but the mean value was partially normalized (5.2 +/- 0.9 mg/kg/min). In the diabetic group, no correlation was observed between fasting blood glucose and GDR. These results suggest that in the course of developing NIDDM, a decrease in insulin-stimulated glucose uptake precedes a rise in fasting blood glucose. Thus, as previously reported for Caucasian NIDDM patients, resistance to insulin-stimulated glucose uptake may be one of the basic defects in Japanese patients with NIDDM. The degree of glycemia, however, is not directly related to the magnitude of the defect in insulin action.

Long-term results of immunochemotherapy for advanced ovarian cancer.

Fifty-one patients with stage III and IV ovarian adenocarcinoma underwent initial cytoreductive surgery at Kurume University Hospital between January 1982 and October 1985. They were assigned postoperatively by randomized trials to immunochemotherapy or chemotherapy alone. Of all 51 patients, long-term results of immunochemotherapy for 43 patients were evaluated versus radicality of initial cytoreductive surgery. As a result, the group of patients treated with immunochemotherapy tended to have a better prognosis than the group of patients treated with chemotherapy alone, whereas no statistical difference was observed between the two groups. Moreover, no significant difference was observed in the monitoring of OKT 4/8 ratio between the two groups. However, immunochemotherapy produced a favorable prognosis in the patients with residual disease of greater than 2 cm in diameter at initial surgery. In conclusion, these data suggested that immunochemotherapy may have some impact on survival of patients with ovarian adenocarcinoma.

Rapid downregulation of transcription mediated by the cAMP response element of c-fos promoter does not require new protein synthesis.

Induction of the c-fos gene is well known to be transient, due to autorepression of its transcription. While the dyad symmetry element (DSE) in the c-fos promoter has been shown to mediate transient c-fos transcription in response to serum, it is yet unknown whether the cAMP response elements (CREs) in the promoter mediate transient transcription in response to cAMP. We observed that forskolin induced transient expression of the c-fos gene in the presence of a calcium ionophore A23187 in NIH3T3 fibroblasts. We investigated transcriptional abilities of one of the CREs (-60 CRE) as well as the DSE. Both of the elements mediated the synergistic effects of forskolin and A23187 on transcription. Transcription driven by the -60 CRE was downregulated at around 2 hrs. after stimulation of forskolin plus A23187 with similar kinetics to that by the DSE. The downregulation with the -60 CRE was less sensitive to an inhibitor of protein synthesis comparing with that by the DSE. These results suggest that function of the -60 CRE activated by forskolin plus A23187 is downregulated by activation of pre-existing factors.

[A glomus tumor of the trachea--a case report].

We present a 61-year-old man with a glomus tumor of the trachea. To our knowledge, this is the 6th reported case in the literature. The patient was asymptomatic, and bronchoscopic examination revealed a highly vascular polypoid mass about 6.0 cm above the bifurcation of the trachea. We undertook a sleeve resection of the trachea with end to end anastomosis through a right posterolateral thoracotomy. The patient's postoperative course was uneventful. The tumor was 1.2 cm in diameter and was composed of glomus cells, which resembled to modified smooth muscle cells, ultrastructurally.

[The development and the applications of clinical laboratory system by using of fuzzy set theory].

For clinical laboratory data to be used effectively in wide areas of clinical medicine, it must be stored in proper database systems. Moreover, the system must be installed in the hospital information system for easy access by the hospital staff in their routine activities. From this standpoint we have developed a clinical test relational database system at Hiroshima University Hospital, which provides the functions of easy on-line retrieval and statistical analysis. We have also improved the functions to incorporate fuzzy query processing based on the theory of fuzzy sets. This system enables us to use natural linguistic representation and leads to a medical knowledge-base.

Clinical Laboratory Information Systems

[A case of pulmonary arteriovenous fistula accompanied with marked polycythaemia].

A 67-year-old male was admitted with cyanosis, digital clubbing and exertional dyspnea. Laboratory data revealed severe polycythaemia with 26 mg/dl hemoglobin, red blood cell; 866 x 10(-4)/mm3, hematocrit 72.8% and PaO2 44.6 mmHg. Selective pulmonary angiography demonstrated a large arteriovenous fistula involving the right middle lobe. After venesection of 1,200 ml of blood, the middle lobectomy was performed safely. In a case of pulmonary arteriovenous fistula with such severe polycythaemia, preoperative venesection is useful to decrease perioperative complications.

[Two cases of cardiac injuries due to nonpenetrating trauma of the chest].

Successful repair of nonpenetrating traumatic cardiac injury is uncommon. Two cases of nonpenetrating traumatic cardiac injuries from accident are reported. Case 1. Seventy eight year old man with shock was admitted to Iida City Hospital four hours after getting a bruise by a wood block on the anterior chest wall. Soon after his arrival ultrasonic cardiography was performed because of a doubt of cardiac trauma. And it revealed cardiac tamponade. Then the emergency operation was performed. After pericardial effusion was drained, rupture on the out-flow tract of the right ventricle was found and was successfully repaired without the aid of cardiopulmonary bypass. Case 2. Sixty five year old man with left heart failure was admitted to Iida City Hospital four months after getting a bruise on left chest and back from falling. Chest and back pain was reduced three weeks after the accident, however, shortness of breath was getting worse. Ultrasonic cardiography and aortography showed aortic regurgitation. Under cardiopulmonary bypass aortic valve replacement was performed. His aortic regurgitation was caused by perforation of non-coronary cuspid due to accident. Ultrasonic cardiography is useful to make a diagnosis of nonpenetrating traumatic cardiac injury. It is necessary to operate as soon as possible in cardiac rupture, and to select valve replacement for aortic regurgitation.

Staurosporine facilitates recovery from the basal forebrain-lesion-induced impairment of learning and deficit of cholinergic neuron in rats.

Alzheimer's disease is characterized by the loss of cholinergic neurons in the nucleus basalis of Meynert and by a primary loss of memory function. Since staurosporine has been reported to induce differentiation in human neuroblastoma cells in vitro, we studied the effects of staurosporine on the amnesia induced by basal forebrain-lesion in rats. Staurosporine (0.05 and 0.1 mg/kg intraperitoneal) attenuated the impaired performance of water maze and passive avoidance tasks, even though the drug administration began 2 weeks after the lesion. Moreover, staurosporine (0.1 mg/kg) partially reversed the decrease of choline acetyltransferase activity in the fronto-parietal cortex induced by basal forebrain-lesion. These results suggest that staurosporine attenuates impairment of learning through reversal of damage to cholinergic neurons induced by basal forebrain-lesion. This evidence indicates that neurotrophic factor-like substances may be used in novel therapeutic approaches to Alzheimer's disease.

Proclotting enzyme from horseshoe crab hemocytes. cDNA cloning, disulfide locations, and subcellular localization.

Proclotting enzyme is an intracellular serine protease zymogen closely associated with an endotoxin-sensitive hemolymph coagulation system in limulus. Its active form, clotting enzyme, catalyzes conversion of coagulogen to insoluble coagulin gel. We present here the cDNA and amino acid sequences, disulfide locations, and subcellular localization of proclotting enzyme. The isolated cDNA for proclotting enzyme consists of 1,501 base pairs. The open reading frame of 1,125 base pairs encodes a sequence comprising 29 amino acid residues of prepro-sequence and 346 residues of the mature protein with a molecular mass of 38,194 Da. Three potential glycosylation sites for N-linked carbohydrate chains were confirmed to be glycosylated. Moreover, the zymogen contains six O-linked carbohydrate chains in the amino-terminal light chain generated after activation. The cleavage site that accompanies activation catalyzed by trypsin-like active factor B, proved to be an Arg-Ile bond. The resulting carboxyl-terminal heavy chain is composed of a typical serine protease domain, with a sequence similar to that of human coagulation factor XIa (34.5%) or factor Xa (34.1%). The light chain has a unique disulfide-knotted domain which shows no significant homology with any other known proteins. Thus, this proclotting enzyme has a mammalian serine protease domain and a structural domain not heretofore identified in coagulation and complement factors. Immunohistochemical studies showed that the proclotting enzyme is localized in large granules of hemocytes.

Flow cytometric analysis of nuclear DNA content in patients with recurrent epithelial ovarian cancer.

Numerous reports have recently indicated that the DNA content of various malignant tumors can be of great value in predicting biological behavior and prognosis of the tumors. This study was undertaken to determine the nuclear DNA content in the primary and recurrent ovarian carcinoma of the same 20 consecutive patients by flow cytometry, and the results on clinical outcome was examined. The tissue samples of the recurrent tumors were obtained at second-look laparotomy. Of the primary tumors, 12 were diploid, 4 were pure aneuploid, and 4 were "mosaic", while of the recurrent tumors, 17 were diploid and 3 showed pure aneuploid. No significant difference of the DNA index at recurrence was observed. DNA ploidy was preserved in 12 out of 20 patients at recurrence. The time to recurrence after the initial treatment showed no significant difference versus DNA ploidy and change of ploidy. The sites and status of recurrence differed by DNA ploidy. At recurrence, the patients with DNA diploidy had a tendency to survive longer than those with DNA aneuploidy. The determination of DNA ploidy at recurrence may be useful as a variable parameter in predicting the survival of patients with ovarian carcinomas.

[CT and sonographic diagnosis of retained surgical sponge].

Seven cases of pathologically proven retained surgical sponge were reviewed and classified into four types, depending on CT and US findings. Type I: CT shows a mass with tiny gas bubbles, and sonogram shows an echogenic area with strong posterior shadow. Type II: CT shows an irregular high density mass with no gas bubble, and sonogram shows an echogenic area with strong posterior shadow. Type III: CT shows a low density mass with irregular internal high density areas, and sonogram shows a cystic mass with zigzag internal components. Type IV: CT shows an elliptic high density mass with low density area internally. Judging from the experiment and operative findings, these patterns were decided by the number and status of the gauze, volume of the exudate and hematoma, and status of the granulation. The author concludes that these characteristic CT and US findings, together with a history of surgery, permit the correct diagnosis of retained surgical sponge.

[Platinum-based combination chemotherapy for malignant mixed mesodermal tumor of the ovary: a case report].

Malignant mixed mesodermal tumor (MMMT) of the ovary is a rare neoplasm with a dismal prognosis and therapeutic modalities remain an enigma. The present paper describes our experience with an impressive response utilizing a cis-platinum based combination chemotherapy in a 56-year-old female with this disease and a review of the literature. The patient was treated with PAIE therapy following a cytoreductive surgery. Two further courses of a single-agent chemotherapy of cis-platinum were given. A PR was obtained after completion of the third course and tumor markers showed a prompt fall with a tumor response. Nevertheless, no further therapy was performed because of her refusal. She expired 5 months after diagnosis because of tumor progression.

Antineoplastic Combined Chemotherapy Protocols

The structure of (xylose)2glucose-O-serine 53 found in the first epidermal growth factor-like domain of bovine blood clotting factor IX.

We reported the presence of a new trisaccharide composed of two xylose and reducing terminal glucose residues linked to serine residues of bovine blood clotting factors VII and IX (Hase, S., Kawabata, S., Nishimura, H., Takeya, H., Sueyoshi, T., Miyata, T., Iwanaga, S., Takao, T., Shimonishi, Y., and Ikenaka, T. (1988) J. Biochem. (Tokyo) 104, 867-868). The present paper describes the detailed structural analysis of the trisaccharide. Glycopeptides were prepared from bovine factor IX by digestion with Pronase followed by purification by column chromatography. The trisaccharide was released from the protein by the beta-elimination reaction with hydrazine, and the reducing end of the sugar chain was tagged with 2-aminopyridine. The fluorescent pyridylamino derivative of the trisaccharide was purified by gel filtration and reversed-phase high performance liquid chromatography. The glycopeptides and pyridylamino-trisaccharide thus obtained were subjected to methylation study, 500-MHz 1H nuclear magnetic resonance spectroscopy, and periodate oxidation. Glucose and xylose belong to the D series by high performance liquid chromatography on a chiral column. From the results, the structure of the trisaccharide is proposed as: D-Xyl p alpha 1-3-D-Xyl p alpha 1-3-D-Glcp beta 1-O-Ser-53.

A new trisaccharide sugar chain linked to a serine residue in the first EGF-like domain of clotting factors VII and IX and protein Z.

Recently, we determined the complete amino acid sequence of bovine factor VII (Takeya, H. et al. (1988) J. Biol. Chem. 263, 14868-14877). In the course of the studies, we found an unknown serine derivative at position 52 in the first epidermal growth factor-like domain of factor VII. A pentapeptide isolated from the S-aminoethylated factor VII contained Ser-52, which could not be identified with a gas-phase sequencer. The same results were also obtained for a pentapeptide containing Ser-53 of factor IX and protein Z. Component sugar analysis revealed that the peptide contained 1 mol of glucose and 2 mol of xylose. This sugar component was also confirmed by high-resolution fast atom bombardment mass spectrometric analysis of the pentapeptide. The trisaccharide was released from the peptides by means of beta-elimination reaction and its reducing end was identified as pyridylamino-glucose. These results indicate the existence of a (Xyl2)Glc-Ser structure in factors VII, IX and protein Z. Similar results were obtained for human factors VII, IX and protein Z. This is the first report of a (Xyl2)-Glc-Ser structure in glycoproteins to our knowledge. The presence of the unique trisaccharide structure in factors VII, IX and protein Z leads us to anticipate its biological role in the tissue factor pathway.
