[Lung embolism caused by vaseline following insertion of a venous catheter].

Pulmonary embolism usually is due to natural causes, most often to obstruction of pulmonary arteries by blood clots breaking loose from venous thrombi forming in the leg or pelvis. Clots of other cellular material such as tumor fragments or fatty tissue (this especially in case of severe soft tissue and skeletal trauma), but also foreign material can give rise to this- often times lethal-condition. This is a report of foreign-body pulmonary embolism with obstruction of mainly the right lower lobe pulmonary arteries due to white vaseline embolism, probably responsible for the patient's premature surmise. Clinical history, therapeutic measures and course of disease--including radiographic and laboratory findings are being presented. Autopsy revealed massive pulmonary embolism to the right lower lobe due to a whitish greasy substance, which was chemically identified to be white vaseline. Additionally, this zone of infarction also showed signs of severe pneumonia with abscess formation resembling that of necrotizing infarction and therefore felt to be related to the foreign body embolism. Histology showed evidence of pneumonia and also inflammatory changes of the pulmonary arteries in form of additional formation of endothelium with involvement of the internal elastic layer. Finally, several possible causes for this case of pulmonary vaseline embolism are being discussed. The exact mechanism remains unclear; but in view of the course of disease, some radiographic findings as well as those of autopsy and histology, it seems most likely that the vaseline embolism was connected to transjugular catheterization rather than to the two other mechanisms also described.

Dormant tumor cells in liver and brain. An autopsy study on metastasizing tumors.

In 96 of 100 autopsy cases with metastasizing tumor diseases single disseminated tumor cells were detected in liver or brain tissues. These tumor cells appear to be in a resting state. Most of the resting or dormant tumor cells (dtc) are distinguishable from tumor cells within metastasis (mtc) by morphological criteria such as cell size or density, shape and size of the nucleus. These observations suggest that disseminated cells from many primary tumors might pass through a resting (Go) state before they eventually initiate malignant cell growth and give rise to metastatic colonies. Most likely the dtc are not affected by systemic chemotherapy.

Autopsy

EA-rosette test in cervical cell suspensions.

Use of the EA-rosette test to detect the presence or absence of cells with an Fc receptor is proposed as a method of screening for cancerous cells and cells of carcinoma in situ in suspensions of cervical cell populations, The EA-rosette test was performed on cervical cell suspensions from 2,437 patients. In 54% the rosette test was negative; i.e., no EA rosettes were observed in the cervical cell suspensions. The EA-rosette test was positive in 46% of the cases, within which most cells with severe dysplastic and carcinomatous features were found. The predictive value for the negative EA-rosette test was 99.8%.

Carcinoma in Situ

Cell suspensions from the cervix may or may not contain EA-rosette-forming cells. In the case of non-rosette formation no further cytological examination is required. In case of rosette formation of moderate degree (10 cells per counting chamber) further cytological examination is necessary since in these cell populations atypical cells might be present. This procedure reduces the number of cytological examinations for about 50% and therefore entails considerable reduction of the load for cytological laboratories.

[Excessive hyperplasia of the exocrine pancreatic tissue and Wernicke's encephalopathy (author's transl)].

A 52 year old woman with a history of mild pancreatitis had a palpable mass in the upper abdomen. She died in an unexplained coma. The autopsy revealed an excessive hyperplasia of the exocrine pancreatic tissue resulting in the largest pancreas ever reported (254 gramm). This hyperplasia was accompanied by a chronic pancreatitis. In the central nervous system a Wernick's encephalopathy had developed; severe loss of neurons was stated in the thalamus; extensive degeneration was found in the upper vermis of the cerebellum. The discussion of this case includes the relationship between the diseases of the pancreas and neuropsychiatric and neuropathologic findings. It is assumed that the hyperplasia of the exocrine pancreatic tissue could only be an additional factor in the occurence of post alcoholic complications. This hyperplasia of the exocrine pancreatic tissue is considered to be a hamartom-like formation and it should therefore be distinguished from other pancreatic hyperplasia.

Diagnosis, Differential

[Lymphocytic and non-lymphocytic dysplasia, carcinoma in situ and infiltrating squamous cell carcinoma of the cervix (author's transl)].

The degree of lymphocytic infiltration in 490 cases of infiltrating squamous cell carcinoma, carcinoma in situ, and dysplasias of the cervix was studied. 204 cases did not show any lymphocytic infiltration at the site of dysplasias, carcinoma in situ, and infiltrating squamous cell carcinoma. The degree of lymphocytic infiltration was furthermore not related to the grade of dysplasia. Taking into account all cases it was found that 75% of carcinoma in situ and infiltrating squamous cell carcinoma remained without or with only sparse lymphocytic infiltrations.

[Morphology of histological subgroups of Hodgkin's disease (author's transl)].

The morphological criteria for the subclassification of Hodgkin's disease were studied in order to obtain a better appreciation of their diagnostic value. We could not separate a specific criterion which would alone permit the diagnosis of a subgroup of Hodgkin's disease. However, the combination of different morphological features including form and frequency of the diagnostic Reed-Sternberg-cells improves the reproducibility of subgrouping Hodgkin's disease. According to their diagnostic significance we consider important: for the LD diffuse infiltration of the lymphnode with few Reed-Sternberg-cells and discret inflammatory reaction; for the MC heavy inflammatory reaction, occasionally with necrosis, numerous diagnostic Reed-Sternberg-cells, occasionally focal involvement in the lymphnode and appearence of birefringent collagen fibers; for the NS the lacunar cells and nodular arrangement of birefringent collagen fibers; for the LP the pleomorphism of the Reed-Sternberg-cells, collagen fibers without birefringency. Based on morphological ressemblance of the nuclei we assume that the tumor cells represent malignant lymphocytes; we consider therefore Hodgkin's disease as a lymphocytic lymphoma.

Diagnosis, Differential

[Receptors for complement and Fc and T-lymphocyte markers on cells of malignant tumours of man (author's transl)].

32 malignant solid tumours of various histological types were tested for the presence of Fc receptors, complement receptors and T-lymphocyte markers in single-cell suspensions of fresh tissue. Indicator cells were normal SRBC (E), SRBC coated with rabbit antisheep lg (EA) and EA coated with complement (EAC'). Fluorescent microscopy on tumour cells was done with rabbit anti-human IgG-fraction labeled with FITC. Almost all tumours possessed Fc receptors and/or complement-receptors. The presence of T-lymphocyte marker was less frequently found. It is proposed to establish a "receptor classification" in addition to the histologic classification of the tumours.

[Occurrence of malignant tumors in high-aged patients (author's transl)].

The analysis of macroscopic findings in 2385 autopsies for frequency and distribution for malignant tumors had the following results: Augmented frequency with increasing age was stated for cancer of the stomach, colon, prostate and breast. There is a clear decrease in the frequency of cancer of the lung and of the uterus with increasing age. Severe tumorindependent disease was an almost constant finding in tumor autopsies. The overall frequency of malignant tumors decreases with increasing age.

Age Factors

[The occurence of multiple diseases in high-aged patients (author's transl)].

Analysis of 1385 autopsies of 80-102 year-old patients was performed to investigate the frequency of simultaneous occurence of the following functionally pertinent diseases in one organism (polypathia): Severe aortic, coronary and cerebral arteriosclerosis, chronic pulmonary emphysema, contracted kidneys, fractures of the femur and malignant tumors. 80% of all patients of both sexes suffered from 2-4 of the above diseases. Even in the highest age group, however, 5 and more of the above diseases were very rarely developed. Patients with none of the above diagnoses were almost never encountered. The "polypathia" was stated also in patients who died by suicide or accident.

Aged

[Pathological-anatomical findings in 70-102 years old Caucasians (author's transl)].

The macroscopic findings of 2385 autopsy protocolls of 70-102 year-old males and females were analysed for frequency and functional significance of diseases. 1. Severe coronary arteriosclerosis was found up to 60% of the cases, with increasing frequency in higher age groups. However decrease of acute myocardial necrosis was stated including the highest age group. 2. The mean values of heart weights decrease in the highest age groups. 3. While the frequency of arteriosclerosis in the cerebral arteries increases the frequency of ischemic lesions of the brain decreases. 4. Benign hyperplasia of the prostate is found in 85% of the cases. 5. 80% of the patients had severe substantial chronic pulmonary emphysema and acute tracheobronchitis. These pulmonary lesions are supposed to be the functionally most important disease in patients of old age.
