Differential hepatotoxicity of two oral retinoids (etretinate and isotretinoin) in a patient with palmoplantar psoriasis.

A 64-year-old woman developed biopsy-proven hepatitis during oral treatment of severe pustular psoriasis of palms and soles with an aromatic retinoid, etretinate. The elevations in hepatic enzyme levels reappeared when etretinate was reinstituted 18 months later. Analysis of serum and subcutis showed normal therapeutic concentrations of the drug. Isotretinoin therapy, although apparently devoid of hepatotoxicity, was clinically only marginally effective. Evidence compiled from the literature suggests that etretinate-hepatitis is a drug-specific reaction.

Cellular retinol-binding protein. Quantitation and distribution.

The distribution of cellular retinol-binding protein was localized in three tissues involved in the transport of vitamin A using the peroxidase-antiperoxidase technique. In addition, a sensitive radioimmunoassay was developed to quantitate cellular retinol-binding protein in various tissues. In liver, the protein was found in the hepatocytes and in the perisinusoidal fat-storing cells. The columnar epithelial cells of the jejunum and the cells of the proximal tubuli also exhibit high concentrations of cellular retinol-binding protein. Estimations of the content of the protein in various tissues from normal and retinol-deficient rats showed that the retinol status did not affect the tissue levels or the subcellular distribution. The appearance of high amounts of this protein in cells involved in the dietary uptake, storage, mobilization, and resorption of vitamin A suggest that one function of the cellular retinol-binding protein is to act as a vehicle in the intracellular transport of retinol through these organs.

Nasal cancer and occupational exposures. Preliminary report of a joint Nordic case-referent study.

Nasal and sinus paranasal cancers have been associated with several occupational exposures, for example, dust from hardwood, nickel and unspecific agents occurring in the boot and shoe industry. A joint Danish-Finnish-Swedish case-referent investigation was initiated in 1977 to study further the connection between nasal and sinus paranasal cancers and various occupational exposures. All new cases of these cancers were collected from the national cancer registers (Finland & Sweden) or from hospitals (Denmark). Those still alive who agreed to the interview (N = 167) were individually matched for age and sex with patients with colonic or rectal cancer. A detailed telephone interview was performed according to a standardized procedure. Both the cases and referents thought that their condition was the one under study. The exposures were coded blindly by an experienced industrial hygienist. The results showed associations between nasal or sinus paranasal cancer and exposure to hardwood or mixed wood dust (discordant pairs 14/2); softwood dust alone (13/4); chromium 16/6); nickel (12/5, not significant); welding, flamecutting, and soldering (17/16); and lacquers and paints (12/0). Hardwood dust exposure showed a connection with adenocarcinoma. Softwood dust exposure alone was associated with epidermoid and anaplastic carcinomas. No associations were found for a number of exposures, including agricultural chemicals, textile dust, asbestos, quartz dust, organic solvents, and leather work. Possible exposure to formaldehyde was evenly distributed between the cases and referents.

Abdominal aortic aneurysm with perianeurysmal fibrosis. A clinical entity.

Nine operatively treated patients with abdominal aortic aneurysm and associated perianeurysmal fibrosis are reported. The fibrotic reaction closely corresponded to the extent of the aneurysmal disease. In four patients only the periaortic tissue was involved, whereas in the other five there was also involvement of the tissue surrounding the inferior vena cava, the duodenum and one or both ureters. A tentative preoperative diagnosis of perianeurysmal fibrosis was made in three patients and in one of them this was confirmed at computer tomography. In view of the similarity in clinical and microscopic appearance it is suggested that "perianeurysmal fibrosis", "inflammatory aneurysms" and retroperitoneal fibrosis" associated with abdominal aortic aneurysms might represent the same disease or different stages of it.

Effect of trisodium phosphonoformate in genital infection of female guinea pigs with herpes simplex virus type 2.

A genital herpesvirus type 2 infection in guinea pigs has been used to evaluate the inhibitory effect of phosphonoformate on the infection. An early topical treatment prevented the appearance of vesicles and histopathological changes and no virus could be recovered. When treatment, either topical or both systemic and topical, was delayed to 24 hours post infection no therapeutic effect was observed.

Esthesioneuroblastoma. Histological, histochemical and electron microscopie studies of a case.

A case of esthesioneuroblastoma was examined by histological, histochemical and electron microscopic techniques. The tumour cells showed an argyrophil reaction with the Grimelius technique and contained cytoplasmic secretory granules, but in contrast to previous reports were devoid of histochemically demonstrable biogenic amines. For routine diagnosis the argyrophil technique may be useful in differentiating this type of tumour from epidermoid carcinoma.

Impaired release of vitamin A from liver in primary biliary cirrhosis.

In 44 patients with primary biliary cirrhosis serum levels of vitamin A, retinol-binding protein and transthyretin (prealbumin) were found to be significantly lower than in 25 sex- and age-matched controls. Liver biopsies were available for chemical analyses in 28 of the patients. Their mean liver vitamin A concentration (2.8 +/- 2.0 mumoles per gm wet weight) did not differ significantly from that in 22 cases of sudden death which served as controls (2.0 +/- 1.5 mumoles per gm wet weight). Immunohistochemical investigation showed a normal distribution of serum retinol-binding protein in the patients' livers, whereas the staining pattern of cellular retinol-binding protein, believed to be involved in the intrahepatic transport of vitamin A, was abnormal. Thus, the number size and cellular retinol-binding protein staining intensity of fat-storing (Ito) cells were clearly higher in the patients as compared with controls. The results suggest that the low serum vitamin A levels in primary biliary cirrhosis are not a consequence of vitamin A deficiency but instead reflect a defective mobilization of vitamin A from the liver.

Metronidazol as a probable cause of severe liver injury.

Metronidazol, a commonly used antibiotic drug, has been very rarely associated with hepatotoxicity. In particular, no reports have appeared in the literature about cases of metronidazol-associated severe hepatotoxicity, leading to liver transplantation or death. We report on a case of acute fulminant liver failure in a young woman, who had, two years previously, developed jaundice after intake of metronidazol. During the current hospitalization, metronidazol treatment had been undertaken two weeks previously and also this time the patient developed severe hepatocellular injury and cholestasis. A viral etiology was ruled out as well as vascular, metabolic and malignant etiology. Although, the cause of the liver injury in this case is not proven, the relationship between this drug and two occasions of severe liver damage, suggests a positive challenge as well as rechallenge. An International algorithm was used for the assessment of the causality of a drug in this case of acute liver injury and a "probable" classification was obtained.

Blood group isoantigen and involucrin expression in cervical intraepithelial neoplasia.

258 patients with dysplastic cervical lesions reclassified as CIN 1-3 and AIM 1-3 and with up to ten years of follow-up were immunostained with monoclonal antibodies against blood group isoantigens and involucrin for expression in normal and dysplastic squamous epithelium. The results show a varied staining pattern with a majority of unchanged staining properties in normal and dysplastic epithelium, the same frequency of increase as decrease in dysplasia and no correlation to progression or recurrence, in variance with previous reports.
