[Changes in the psychosomatic status following operative augmentation and reduction of the female breast. Catamnestic study from the operative-gynecologic and psychosomatic viewpoints].

We performed preoperatively a psychiatric and psychosomatic exploration of women who came to us with a request for performing breast-corrective surgery. 18 months after the operation, these women were again examined both gynaecologically and psychosomatically. Subsequent to a postoperative course without complications and with primary wound healing, 88% of the surgically treated women considered the result to be in accordance with their expectations, and the size of their breast as adequate. Analysis of variance of the personality data reveals that patients with hypoplasia or hyperplasia of the breast do not differ from each other in respect of their personality characteristics. Preoperatively, the patients assessed themselves on the average very negatively with regard to social resonance, and proved to be very depressive. After plastic surgery of the breast, i.e. after anatomic correction, definite changes are seen in the mental field. A significant improvement is noticeable in the way the patients experience themselves, their body, and their self-value. The same also applies to the experience of their sexuality, to social relationships and social resonance, social potency, and partner relationship. All these were assessed by the patients as considerably improved (significant on the 1% level).

[Encounter with the so-called Munchausen syndrome in the psychiatric clinic].

The term "Münchhausen's syndrome" comprises a group of patients characterised by habitual presentation for hospital treatment, mostly by means of a dramatic situation of an apparent acute illness caused by an either simulated or artificially produced disorder. The authors discuss a case that had been presented to them, and turn their attention to the problems of pathology, psychodynamics and differential diagnosis connected with this particular patient. Diagnostic classification may differ widely, depending on the individual case. In this particular woman patient the authors tend to diagnose, more likely than not, a borderline personality disturbance as defined by the Diagnostic and Statistical Manual (DSM III) of the American Psychiatric Association, this diagnosis being made mainly on the grounds of psychodynamic deliberations.

Psychotherapeutic effects by audiovisual heteroconfrontation in a case of anorexia nervosa.

After a short overview about the use of audiovisual methods in psychiatry and psychotherapy we present a single case study of a young woman suffering from anorexia nervosa. In a typical way the patient could not accept the idea that she had an emotional disorder and did not accept being admitted to hospital or to come as an outpatient to psychotherapeutic treatment. We used the method of audiovisual heteroconfrontation, i.e., she was confronted with the videotape of another patient suffering from the same disorder. After four sessions of confrontation she was extremely occupied with herself, she suffered more from her condition and was motivated to undergo psychotherapy. Finally, the process of heteroconfrontation is discussed.

[Contribution to differential-diagnostic validity of MMPI to prognosis of efficiency of autogeneous training (author's transl)].

Due to the fact, that a group of patients were not able to achieve success in the autogeneous training in spite of usual selection after diagnostic criteria, we submitted them as well as a control group to the MMPI. We found significant differences in the Hd-, D-, Hy-, and Si-scales. Then we calculated the discriminance function and the cut-off score and found after MMPI-profile an 80% probability of predicting if a patient will succeed in autogeneous training or not.
