An intraoral appliance for management of the protrusive premaxilla in bilateral cleft lip.

Management of the protruding premaxilla and prolabium in bilateral cleft lip and palate can be a confounding problem. This report introduces an intraoral traction appliance that has been successfully used to move the premaxilla and prolabium to a more favorable position for surgical repair of the bilateral cleft lip. The appliance consists of a palatal baseplate for anchorage and a latex rubber traction strip looped over the prolabium to retract the premaxillary segment. The results in five cases are presented and compared to two cases where no presurgical management was performed and lip repair had been delayed for medical reasons. Advantages and disadvantages of the appliance are discussed.

Instrumentation for solving abutment parallelism problems in fixed prosthodontics.

Casts from a number of preparations by dentists with a variety of fixed prosthodontic experience levels were surveyed, and the preparations were commonly found to contain undercuts, overtapered surfaces, and lack of a parallel path of insertion. A new instrumentation for preparing abutment teeth was examined, which eliminated the common inaccuracies caused by traditional techniques of using freely held handpieces and visually assessing the finished preparations. Three clinical preparation procedures were discussed. Each patient posed particularly demanding problems that required precise tooth preparation. Each completed prosthesis was readily inserted in the mouth due to the unobstructed uniform path of insertion. Keyways were not required in any of the prostheses.

Early prosthetic management of cleft palate. Articulation development prosthesis: a preliminary report.

This study evaluated the effects of early prosthetic intervention for achieving normal articulation development in cleft palate patients. Early surgery is the preferred method of treatment to permit normal articulation development. Prosthetic management provides an alternative treatment to promote more normal articulation development when early surgery is not planned. It can be concluded that a prosthesis must be placed prior to development of meaningful speech if improved articulation development is to be achieved. Close cooperation among surgeon, speech pathologist, and prosthodontist is necessary to ensure timely surgical deferral and prosthetic treatment.

A technique for obturating palatal fistulas.

Use of nontoxic self-adhesive material is described to obturate residual hard palate fistulas. This material may be used during perceptual and aerodynamic speech assessments to evaluate speech distortion and the relative contributions of velopharyngeal port and fistula to nasal air leakage during speech. It may also be used as an interim obturator prior to surgical repair of a fistula.

Multiple-abutment fixed partial dentures in maxillofacial prosthetics.

Ten patients were presented to illustrate multiple-abutment fixed partial dentures used in combination with auxiliary prosthetic attachments of a fixed or removable design to solve diverse problems encountered in the rehabilitation of congenital and acquired orofacial defects. While such fixed prostheses have high initial costs, their effectiveness and long-term cost-benefits suggest that, where appropriate, they represent optimum therapy that will prove most economical over the long term.

A simplified technique for fabricating a lightweight obturator.

Polyurethane foam was used as a core material in the fabrication of a lightweight obturator. This is a time-saving method of achieving a meaningful reduction in the total weight of a prosthesis while increasing strength and facilitating repair.
