Perioperative antibiotic prophylaxis in patients undergoing radical surgery for gynecologic cancer: single dose versus multiple dose administration.

A prospective, randomized study comparing single dose intravenous antibiotic prophylaxis with multiple-dose regimen in patients undergoing radical surgery for gynecologic malignancy was carried out. The majority of patients had cervical cancer. Thirty-seven patients in group A received one dose of 4 g piperacillin and 800 mg tinidazole 30 minutes before surgery. Twenty-nine patients in group B additionally received 4g piperacillin 8 hours and 4g piperacillin and 800 mg tinidazole 16 hours after the first dose. There were no significant differences between groups A and B with regard to median patient age, performance status, cardiologic and pulmonary status, or frequency of diabetes. There were also no significant differences between either group with regard to the parameters documented postoperatively, such as median hospital stay, leukocyte values, or febrile episodes. Forty-three per cent of patients in group A and 48% of patients in group B had no complications (p > 0.05). Twenty-three per cent of patients in group A and 31% of patients in group B developed infections (p > 0.05), 85% of which occurred in the urinary tract. The data suggest that a single dose of intravenous antibiotics is as effective as multiple dose administration in the perioperative prophylaxis in radical gynecologic surgery.

[Induced abortion--a critical life change event].

In the present survey the psychic and physical mastering of abortion is considered to be a subject of the research of a "critical life-event". Special interest was devoted to the consequences of the process of mastering before and after abortion on the mental and physical condition of the examined human females (n = 39). The diagnosis showed that especially before abortion in a time of a serious state of inner conflict, interference concerning the physical condition and psyche of women arise, while actually after abortion the mood is more euphoric. The inconvenience increased in dependence on experience of social welfare and support. Three months after abortion specific complaints such as pains in the lower abdomen, bleeding and sudden urge to cry occur. The administration of contraception after abortion is becoming significantly safer.

[Significance of vaginal and cervix flora with reference to ascending infections].

The role of the vaginal and cervical flora and the extent of alterations in the flora in ascending infections of the female genital tract were evaluated in a microbiological study of 86 patients with a clinical diagnosis of pelvic inflammatory disease (PID). The patients were classified as having acute salpingitis (n = 15), an acute inflammatory adnexal tumour (n = 21), subacute salpingitis (n = 38), or chronic salpingitis (n = 12). 100 symptom-free women formed a control group. The spectrum of microorganisms found in the two groups was similar. However, there were significant differences in the distribution of pathogenic and apathogenic organisms. Negative cultures and cultures with exclusively apathogenic organisms were found in 16% of the patients with PID and 43% of the healthy women. Growth of exclusively apathogenic aerobic organisms was seen in 3.5% of the patients with PID and 18% of the controls. Exclusively apathogenic anaerobic organisms were found in 7% of the patients with PID and 32% of the controls. Mycoplasmas were significantly more common in the PID group than in the controls (31% v. 10%). The rates of Chlamydia trachomatis did not differ significantly (13% v. 10%). Patients with pathogenic organisms had a 3.88 (odds ratio) or 2.27 (col-2 risk) higher risk of PID than those with negative cultures or exclusively apathogenic organisms. There were no significant associations between the microbiologic results and individual parameters of inflammation. The microbiologic results did not reflect the course or severity of the disease.(ABSTRACT TRUNCATED AT 250 WORDS)

Conservative treatment of ectopic pregnancy with local injection of hyperosmolar glucose solution or prostaglandin-F2 alpha: a prospective randomised study.

In a prospective randomised study, 31 patients with an unruptured tubal pregnancy were treated either with local and systemic prostaglandins or with local instillation of a hyperosmolar glucose solution. Prostaglandin therapy was successful in 13 of 15 patients and glucose therapy in 16 of 16. 9 women treated with prostaglandins had cramping abdominal pains postoperatively. No side-effects were noted in those treated with glucose. At subsequent hysterosalpingography 5 of 6 patients treated with prostaglandins and 7 of 8 treated with glucose had normal tubal configuration and patency. 3 patients treated with glucose later had a normal intrauterine pregnancy, demonstrably through the affected tube in 1 case. These results suggest that local instillation of hyperosmolar glucose solution is an option in the laparoscopic management of unruptured tubal pregnancies.

[Obstetrics in Graz. A comparison 1887-1987].

We compared obstetric data from the archives in Graz for 1887 with those of 1987. In 1887, 85% of all deliveries took place in the home or with a midwife--compared with only 0.46% in 1987. Maternal mortality in 1887 was 0.75%; two-thirds of the deaths occurred in the puerperium. Perinatal mortality was 11.0% in 1887 and 1.03% in 1987. The intrauterine fetal death rate, including deaths during delivery, was 6.7% and 0.35%, respectively; 2.7% versus 0.26% were macerated. The percentage of stillbirths among unwed mothers is higher today than in 1887. Perinatal mortality in 1887 was frequently associated with prematurity, abnormal presentations, and operative vaginal deliveries. 14.4% of the children of multiple pregnancies died in utero or during delivery. In 1987, 83% of mothers were bearing their first or second child, as compared with 50% in 1887. Grand multiparae were much more common in 1887. The percentage of mothers under 20 years old is twice as high today as 100 years ago.

[Severe fetopathy--toxoplasma despite serologic screening. A case report].

Toxoplasmosis is a special prenatal infection because it is susceptible to medication. Serologic screening and early diagnosis of a new infection is a prerequisite for successful treatment, which protects the fetus from infection. A case of a twenty-two year old gravida-II shows, that infection is possible despite negative initial screening. A new infection was diagnosed because of seroconversion at a routine screening at the end of the second trimester. An ultrasound scan showed severe hydrocephalus, hepatomegaly, and ascites. Labor was induced in the 33rd gestational week. Conatal toxoplasmosis was confirmed by histologic and microbiologic studies of the placenta and membranes. We discuss the difficulties resulting from negative primary serology.

[Diagnosis and treatment of an occult intrauterine device].

Between February 1978 and December 1985 53 women with lost intrauterine devices (IUD) were treated. To exclude unrecognized expulsion and to locate the device sonography was primarily employed. In 90.6% of cases a correct diagnosis was established. Only in 9.4% of cases a plain X-ray of the abdomen was necessary because sonography was not diagnostic. In 47 patients (88.7%) the IUD could be located by hysteroscopy and was removed. Using biopsy forceps or curette. Of the remaining 6 cases there were 4 with perforation and intraabdominal dislocation, 1 spontaneous expulsion in the interval between diagnosis and admission to the hospital and 1 case of endometrial incrustation which escaped hysteroscopic diagnosis.
