Long-term visual results after laser treatment of proliferative diabetes retinopathy in childhood onset diabetes.

Ninety-seven laser treated eyes from 55 highly selected childhood onset diabetics (diabetes before the age of 15, proliferative retinopathy before the age of 31, and a follow-up time of at least 5 years) were examined retrospectively. The eyes were divided into two groups, with or without high-risk characteristics according to DRS classification. They were all treated with a blue-green argon laser in the years 1974 to 1981. Of 34 eyes with high-risk characteristics, 12 (35%) were blind (less than FC 1 M) after 5 years. Of 63 eyes without high-risk characteristics, 8 (13%) were blind after 5 years.

The human choriocapillaris in organ culture.

4 x 4 mm pieces of human eye walls consisting of the sclera, the choroid and the pigment epithelium were kept in organ culture for 1 month and studied with light and transmission electron microscopy comparing the findings with normal tissue. After 1 month the choriocapillaris still had open vessel lumina lined with flattened endothelial cells containing organelles and vesicles on both inner and outer cell membranes. The endothelial cells lost their fenestrations and polarity in regard to the location of cell nuclei. Otherwise the ultrastructure was similar to that of the choriocapillaris in vitro. This study has shown that choriocapillaris can survive in organ culture for 1 month, and it permits further studies on the behaviour of the choroid under controlled conditions, and on the long-term effects of different types of trauma to the choroid in vitro.

The use of sodium hyaluronate (Healon) in the treatment of complicated cases of retinal detachment.

Thirty-six eyes with retinal detachment and various degrees of proliferative vitreoretinopathy were operated with conventional buckling techniques and the additional use of intravitreal injected sodium hyaluronate (Healon). Eighteen eyes (50%) had re-attached retina 6 months post-operatively. The re-attachment rate depended upon the severity of the proliferative vitreoretinopathy. Three of 4 eyes with grade B, 4 of 5 with grade C-1, 8 of 13 with grade C-2, 2 of 11 with grade C-3 and 1 of 3 eyes with grade D-1 were reattached. Major complication occurred in 3 aphakic eyes where persistent intraocular pressure rise to 60-70 mmHg necessitated removal of Healon to normalize the intraocular pressure. In one eye Healon passed through a retinal rupture to the subretinal space.

Retinal detachment after intraocular lens implantation.

Between August 1982 and July 1985, 19 eyes were operated for pseudophakic retinal detachment (PRD) (Anterior chamber lens in 13 eyes, posterior chamber lens in 3 eyes and iris supported lens in 3 eyes). The characteristics of these pseudophakic retinal detachments were very similar to those following intracapsular cataract extractions. Retina was re-attached in 18 eyes (95%), in 14 eyes, after one buckling procedure. In 4 eyes, altogether 12 surgical procedures were needed to re-attach the retina. Problems to visualize the peripheral retina, made it necessary to remove the IOL in 3 eyes (2 iris fixated and one in the anterior chamber).

Pars plana lensectomy.

The results of pars plana lensectomy (PPL) in 36 eyes are reported. The average follow-up time was 9 months (2-24). The optical media became clear in all eyes. Post-operative visual acuity (VA) was greater than or equal to 0.6 in 31 eyes. Two eyes developed serious complications (retinal detachment in 1 eye and neovascular glaucoma in 1 eye). PPL represents minimal risks for the anterior segment, excludes problems with secondary cataract, presents optimal conditions for early contact lens fitting and for necessary treatment in the posterior segment. In the hands of an experienced surgeon PPL is effective and safe and offers an alternative treatment for patients up to the age of 30-35 years.

Light perception in diabetic vitreous haemorrhage and its prognostic value in pars plana vitrectomy.

Ninety-two years with a pre-operative visual function of light perception 6 m or less, were treated with vitrectomy because of diabetic vitreous haemorrhage. The post-operative visual function after 6 months was improved to 5/50-5/5 in 37 eyes and counting fingers from 1 to 5 m in 32 eyes. In 23 eyes the postoperative visual function was unchanged or decreased, and among these were all 7 eyes with pre-operative light perception at 1.5 m or less. No eye with pre-operative light perception at 1.5 or less benefitted from vitrectomy due to extensive retinal damage.

Vitrectomy in double perforating eye injuries.

The results of 41 consecutive cases of double perforating eye injuries, operated with vitrectomy between December 1975 and December 1982, are reported. Fourteen eyes (34%) obtained a visual acuity of 5/10 or better, and 23 eyes (56%) obtained 5/200 or better. Twenty-eight (68%) developed retinal detachment, and 12 of these were attached by surgery. In this series the pre-operative visual function and the type of injury had a significant effect on the final result. No beneficial effect from early (within 14 days) vitrectomy could be shown.

Prophylactic argon laser photocoagulation in retinal detachment fellow eyes.

Over a two-year period (1975-76) 329 patients were operated for rhegmatogenous retinal detachment. 301 patients had unilateral detachment and in 62 of these prophylactic photocoagulation with argon laser in the fellow eye was performed. During the observation time one of these (1.6%) and 7 untreated (2.9%) fellow eyes developed retinal detachment. The indication for prophylactic photocoagulation of fellow eyes and the results are discussed.
