[Demonstration of left-ventricular contraction anomalies with the two-dimensional sector-scanner (author's transl)].

Echocardiographic findings were compared with those obtained by cardiac catheterisation in 134 patients. Echocardiography was performed in the standard plane of the long and short axes, as well as from the cardiac apex. After dividing the left ventricle into several segments wall movement was measured in them and compared with those obtained by ventricular angiography in the right and left anterior oblique positions. It was found that in patients with left-ventricular aneurysm, proven by left ventricular angio, there was complete conformity with regard to localisation and extent of the aneurysm between the two methods of investigation. In this group 79% of the demonstrated segments could be adequately assessed and, in 91%, agreed with the radiological findings. In the group of patients without aneurysm only 68% of the segments could be adequately assessed, and 77% correctly assessed wall movement when compared with the laevocardiogram. Tracing recorded from the cardiac apex gave especially good results in demonstrating the ventricle and its segments.

[Digoxin induced changes in the exercise ECG and its relation to plasma concentrations (author's transl)].

The effects of a single intravenous dose (1.5 mg) of diogoxin on the resting and exercise ECG were studied over several days in twelve normal subjects. Maximal ST-segment and T-wave changes were observed 24 h after drug administration. A significant ST-segment depression and decrease in T-wave amplicude were still observed on the 7th and 11th day after the glycoside dose, in spite of undetectable digoxin plasma concentrations. There was no correlation between digoxin plasma concentration and ECG-changes. In order to avoid false positive ischaemic ST-segment responses to exercise, a therapy with digoxin should be discontinued for at least 2 weeks before the exercise test.

[Functional scintigraphy during contraction of the left ventricle. III. Findings in patients with coronary heart disease (author's transl)].

Left ventricular volume curves were measured by a noninvasive technique after equilibration of 99m TC human serum albumin in 27 patients with angiographically proven coronary heart disease. Compared with normals, patients with coronary insufficiency showed a significant reduction in the ejection fraction and of maximal emptying and filling times of the left ventricle during rest. In mildly affected patients with normal ejection fractions, maximal filling was already reduced in keeping with the well known loss of compliance of the left ventricle due to coronary disease.

[Aplastic anaemia and thymonal].

The combination of aplastic anaemia and thymona was observed in a 49-year old woman. Concomitant with severe normochromic anaemia and reticulocytopenia was a cell-rich bone marrow smear in which erythropoietic precursors were almost absent. Immunological phenomena could not be demonstrated. The erythropoietin level was maximally increased. Pathological anatomy typically revealed a predominantly spindle-cell thymonal with only minor capsular invasion.

[Coronary fistula into the sinus coronarius. Hemodynamics, clinical aspects, therapy].

Coronary artery fistulas are rare, inborn anomalies which will be diagnosed more often as the diagnostic method of coronary arteriography is increasing. This report concerns a preoperatively diagnosed coronary arteriovenous fistula of the right coronary artery to the sinus coronarius; Frequency, pathological physiology, clinical symptoms, differential diagnosis, prognosis and treatment are discussed. The importance of using selective coronary arteriography to outline the anatomy of these malformations is stressed. Surgical treatment is the only effective way of therapy and should be performed after diagnosis, even if in an early stage of the disease the ECG does not show any changes in spite of a large fistula.
